CLINICAL AGENCY REQUIREMENTS It is the student's responsibility to obtain drug screening results and criminal record check. Submission of documentation to www.CertifiedBackground.com is required prior to the enrollment of the first graduate nursing course. Failure to enroll and submit documents to Certified Background will jeopardize enrollment in graduate nursing courses. DRUG SCREENING All students must have on file results of drug screen testing according to Certified Background.com Satisfactory drug screening is a negative result for all tested groups or a statement from a healthcare provider who prescribes the controlled substance. The prescription must be filed with Certified Background. If the drug screen comes back positive for any one of the drug categories, the student may not be allowed to begin the graduate nursing program. CRIMINAL RECORD CHECK All students must have a completed criminal record check on file with Certified Background. Certain criminal convictions do prohibit individuals from sitting from licensure/certification exams and therefore may be grounds for dismissal from the program. If a conviction appears on the criminal record check, students will be asked to confer with the Graduate Program Chair for follow up information and action. CPR REQUIREMENTS All students are required to have current "Healthcare Provider" CPR. A copy of the signed CPR certification card must be on file with Certified Background. Students who are not certified by the beginning of each semester will not be admitted to clinical agencies. HIPPA REQUIREMENTS The USI Nursing Programs and Health Professions comply with HIPPA standards for patient confidentiality and personal health information. Students must complete the USI HIPAA education program as directed by faculty and file exam results with Certified Background. OSHA REQUIREMENTS The USI Department of Nursing is in compliance with OSHA standards for infection control and exposure. Students must complete the USI OSHA education programs as directed by faculty. Students must adhere to the USI CNHP Infection Control Program. ZACHARY LAW COMPLIANCE POLICY To comply with the state and federal regulations, potential and current students and faculty in selected programs within the College of Nursing and Health Professions, will be required to have a criminal records check relating to sexual and violent offenses against children. In accordance with the state of Indiana's revisions of Zachary's law made in January of 2003, the College of Nursing and Health Professions will verify if the student is registered with the registry for convicted sexual and violent offenders against children, and will continue to do so at least annually for as long as the student remains in the program. Should the student's name appear in the registry, the student will be denied admission to or progression in the nursing program. If the listing is the result of an error, it will become the student's responsibility to correct the error before admission or progression in the program will be permitted.
