•pharmacy can give a refund but drug is not to be
recycled. Nobody can verify what happened when it
went home with pt. must be destroyed.
•inpatient hospital----drug is still packaged and not
tampered with. destroy if opened.
•manufacturer recall---when recommended by FDA
•class I recall----can cause serious problems or
death
•class II recall-----temporary problems or slight risk of
serious nature
•class III recall-----violate FDA regulations but
unlikely health problem

• overstocked?----wholesaler has requirements like
unopened and no pharmacy labels.
• sch II drugs--wholesaler must do form 222.
• destruction----recalled, expired, returned. most
pharmacies hire a company that handles returns on
their behalf.
• CSA destruction----returned to DEA authorized
company. 222 form must be done for sch2 drug.
• pharmacy can send drugs to DEA with a DEA return
form. if DEA lets pharmacy do destruction, 2
pharmacist or more must witness
