Lifetime Dental Implant Warranty & 5 Year Prosthetic Warranty
Many practices offer some warranty as a courtesy, but we put in in writing. We stand by our All-on-4
services with a Lifetime warranty on our Dental Implants. Most practices will charge extra for these
services, even though the cause of this failure is sometimes due to their own error.
In the rare case of implant failure, not only will Dr. Golpa will remove the implant and replace it at no
charge, but any corrective procedures required will also be performed at no cost.
Your All-on-4® bridges and implants should receive special care and maintenance every 6 months. If
you choose to participate in our voluntary maintenance and warranty program a complete
assessment of your dental implants and bridge(s) will be conducted at each re-care appointment
which will include:
Visual assessment of the patient; including examining the patient for signs of inflammation,
bleeding, changes in probing depths, mobility, or exudate around the implant(s), and debridement of
the your dental prosthesis. In addition, we will radio-graphically examine your Implant(s) to
determine whether changes have occurred in bone levels. We will also conduct a careful review of
your current medications, as well as a review of your health status.
Due to the nature of the tissue attachment surrounding the implant(s), Dental Implants may be more
susceptible to inflammation and bone loss when plaque accumulates. Proper maintenance of the
tissues will ensure longevity of the Implant(s). Your home-care routine can significantly influence
Implant success. After your re-care appointment, a patient-specific home-care routine is developed
for you.
Your PMMA/Titanium bridge(s) are warrantied for a period of 5 years. Within the warranty period we
will repair or replace bridges that fail due to workmanship at no charge. This period can be extended
through our Maintenance and Extended Warranty Program.
Your Prettau Zirconia bridge(s) are warrantied for 10 years. Within the warranty period we will repair
or replace bridges that fail due to breakage at no charge. This period can be extended through our
Maintenance and Extended Warranty Program.
Dental Implant & Prosthetic Conditions and Limitations
If a dental implant fails to integrate we will replace it at no cost, one time only. An annual implant
examination including x-rays and regular cleanings is an absolute requirement for any warranty to
apply. The Lifetime Warranty Dental Implant Warranty does not apply to smokers or persons who
have, or who develop, an uncontrolled systemic health problem, such as, but not limited to; diabetes,
autoimmune disorders, severe osteoporosis, or any condition requiring long steroid use. If smoking
or a medical condition is suspected as the cause of implant failure or other complication that requires
treatment, an examination and blood tests will be required for warranties to apply.
10 Year Extended Warranty & Maintenance Program
Our Extended Warranty and Maintenance program is designed to ensure the long-term success of
your All-on-4® restoration. Because of the strength of our restorations we are able to offer exclusive
extended warranties. No other facility can offer this value and protection for you.
Much like buying an automobile, you have made a significant investment in yourself! When you
consider all the hard work that went into this investment into your health and well-being, we’re sure
you would agree that it only makes sense to protect your investment so that you get as much return
on it as possible. You understand that even though your car runs great, performs magnificently, and
looks wonderful on the outside it is imperative that you keep up with regularly scheduled
maintenance to keep it running smoothly. Routine oil changes are essential to the life of your car’s
engine. It would be foolish to sink tens of thousands of dollars into a new vehicle only to neglect the
maintenance and drive it to an early grave.
To protect your investment and to insure your satisfaction for years to come, Dr. Golpa has
developed this valuable warranty and maintenance program. Here’s what is included and how it will
benefit you:
NEW BRIDGES IN 5 YEARS
Overtime, changes in your facial structure, and appearance may occur. You will have the opportunity
for us to design new bridges that may help to correct or enhance these effects of nature. Also, as
advances in dental materials are realized, you will be afforded the opportunity to receive new bridges
made from these materials. (Applies to PMMA/Titanium only)
UNLIMITED CLEANINGS, EVALUATION, AND ADJUSTMENTS
Your All-on-4® bridge/s and implants should receive special care and maintenance every 6 months. If
you choose to participate in our 10 Year maintenance and warranty program a complete assessment
of your dental implants and bridge(s) will be conducted at each re-care appointment which will

Your bridges will also be re-lined and adjusted to insure proper fit, comfort, and function.
AIR, HOTEL, AND TRANSPORTATION
For your re-care appointments we will provide 1 night hotel stay for 2 adults, and provide all ground
transportation. In case of emergencies only we will also provide round-trip airfare for 1 adult.
