Since October 2013, Dr. Jed Shimizu and his team have been piloting a Geriatric Evaluation and Management program, to identify seniors not coping in the community and connect them with community supports and services. As a Care of the Elderly Physician, Jed explains that seniors often present to the Emergency department without having medical issues related to aging, such as dementia and functional decline, addressed. As part of the Geriatric Evaluation and Management program, the care team ensures this vulnerable population gets directed to the support they need. “We are meeting unmet needs of seniors coming into the hospital that may not be coping great in their community, but aren’t sick enough to be kept in hospital,” says Jed, explaining that follow-up care and community supports, such as fall prevention and seniors’ day programs, are pivotal for success. Jed Shimizu, Care of the Elderly Physician, and his team have been piloting a Geriatric Evaluation and Management program in the Misericordia Emergency department, to identify seniors not coping in the community and connect them with supports and services. Emergency department transition co-ordinators screen patients 65 years and older for geriatric issues such as cognitive impairment, mobility and falls, and multiple medications to identify if they would benefit from geriatric intervention. If these patients are identified as at-risk, they receive a comprehensive geriatric assessment by the Geriatric Evaluation and Management team. This includes medical, mental, psychiatric and functional assessments. To date, the team has assessed over 300 patients. Margery’s 77-year-old husband came to the Misericordia Emergency department after he had taken a fall and suspected his hand may be broken. After seeing an emergency room physician, he was referred to Jed for an assessment. According to Margery, her husband had been especially forgetful lately. “Going through this assessment and being reassured that my husband’s problems were a mild cognitive impairment, not Alzheimer’s or dementia, was quite a comfort.
