 Conduct patient interviews via clinic and telephone encounters to collect necessary patient demographic data
 Provide patient care independent of health care providers to safely adjust warfarin dose to achieve target INR
and minimize bleeding and thromboembolic risk
 Provide new patient teaching of oral anticoagulants
 Identify drug, food, and disease state interactions with anticoagulants
 Develop plan for perioperative management of anticoagulants to minimize bleeding and thromboembolic risk
 Document pharmaceutical care plan in medical record and implement with patient acceptance

Medication Management
 Assess patient compliance with formulary medications to ensure adherence to policies and procedures to
minimize unnecessary treatment costs
 Review non-formulary drug recommendations for safety based on objective and subjective medical data
 Examine patients’ drug therapy to determine appropriateness of drug, dose, drug regimen, route of
administration, as well as possible treatment emergent side effects
 Document pharmaceutical care activities in the electronic medical record
 Respond to daily drug information questions on PRN
Primary Care Clinic
 Establish a collaborative relationship with primary care health team
 Conduct patient interviews to collect necessary information to evaluate drug therapy and treatment of
established medical conditions
 Review past medical history, family and social history and relevant risk factors and medication compliance to
make appropriate treatment recommendations
 Complete medication reconciliation and document other medications that a patient may be taking outside the
VA including over the counter, locally prescribed prescription medications, and alternative and complementary
therapies
 Review vital signs and make treatment recommendations as needed
 Identify goals of therapy for various disease states after accounting for age, co-morbid conditions, as well as
renal and hepatic function
 Identify drug-drug and drug-interactions and make modifications when necessary
 Screen patients for potential adverse drug reactions to current therapies and recommend alternatives when
necessary
