SUMMARY
Mission
The mission of JoshProvides is to improve the quality of life for those living with epilepsy or other seizure
disorders. The organization does this thru education, awareness, volunteerism, and fundraising activities. In
addition, JoshProvides strives to eliminate the negative stigma associated with epilepsy and to provide financial
and benevolent assistance for people living with epilepsy and seizure disorders, their families, and the
community at large.

BACKGROUND & NEEDS
Impact Statement
Our top accomplishments this year are:
• The establishment of a support group in West Palm Beach and the continued success of the JoshProvides
support group in Sarasota
• The awarding of financial grants as described in the Program Section of this Profile for the benefit of qualified
people living with epilepsy;
• The establishment of collaborative relationships with many surrounding organizations, including Epilepsy
Services of Southwest Florida, Epilepsy Foundation of Florida, Epilepsy Services of Central Florida, the
Manatee County Department of Health, and many others
• The continued success of our annual epilepsy awareness walks: The fourth annual "Family and Dog Walk for
Epilepsy Awareness" will be held in November in support of National Epilepsy Awareness Month. The main
walk is in Sarasota and additional walks will be held in West Palm Beach, Ft. Lauderdale, and other areas in
Florida and throughout the country.
• Our accomplishments are supported by the many testimonials which can be read in the Testimonials listed in
"Other Documents" as well as those listed on the JoshProvides website.
Needs Statement
The needs of JoshProvides during the 2016 fiscal/calendar year include:
1.Financial support for client services: This includes providing seizure alert monitors and seizure response dogs
for individuals and families - $100,000
2.Funding for the creation and support of new support groups; The expenses for a year of operations that
includes a counselor, snacks, and activities for youth is approximately $5000/group.
3.Funding for transportation: The JoshProvides transportation program supports youth and young adults who
are unable to drive due to seizures and need transportation to college and individuals of any age who are in
need of transportation for epilepsy treatment - $35,000
4.JoshProvides is launching a new earned income initiative to support its seizure response dog program. This
initiative will be self-funding within three years, however, has start-up costs for which the organization is
seeking funding. The needs equal approximately $75,000
Background Statement
Shortly after Josh passed away from a fatal seizure, Sandi, Nicole and Bruce Chapnick (the Chapnick Family)
established with the Community Foundation of Sarasota County (CFCS) the Joshua David Chapnick Memorial
Fund (JCF) in honor and memory of Josh. The JCF was organized to assist people living with epilepsy with their
transportation expenses and seizure medications. The fund was established because when Josh had a seizure
he was not allowed to drive for six months and then, only if he could demonstrate he was taking his medications
and had no other seizures during that period of time. Josh was living by himself in West Palm Beach, FL, and
incurred transportation costs averaging $200.00 per day for someone to pick him up in the morning, drive him to
school, pick him up after culinary school and then drive him to work at Seasons 52 and then pick him up after
work and drive him home late at night. His medication often cost over $500 for a thirty day supply of Depakote.
Josh realized he was fortunate that his family could financially assist him with these costs and the Chapnicks
knew there were many families who could not afford to incur these additional expenses. The fund was
established to assist these families. Originally, the fund benefited Epilepsy Services of Southwest Florida. As
the fund grew, requests for assistance began pouring in from individuals and families.
On March 22, 2013, the Chapnick Family organized the JoshProvides Epilepsy Assistance Foundation as a
Florida public, not-for-profit corporation. The Foundation was organized with a thirteen member Board of
Directors in order to complete an effective succession plan for the Chapnick Family to ensure the perpetuation
of the charitable purposes established by the Chapnick Family. On July 19, 2013, the Foundation signed
agreements with Community Foundation of Sarasota County to handle the investments of the Foundation, by
establishing both an endowed fund and a non-endowed fund.
On July 24, 2013, the Foundation registered under the Florida Solicitation Contributions Act and received
Registration Number: CH39093. On September 24, 2013, effective March 22, 2013, the Internal Revenue
Service, following a review of the Foundation’s application for tax exempt status, determined that the
Foundation is a Section 501(c)(3) tax exempt public charity.

Statement from the Board Chair
While JoshProvides is a new and growing foundation, our impact is great for those we help. We fill a temporary,
and often critical, need for these individuals. As an example, we recently approved the purchase of seizure
medication due to a loss of family insurance coverage. Without having medication, this individual would be
subject to a sudden onslot of seizures. This incident happened on a weekend. We are able to react quickly
when time is of the essence.
When we provide assistance, we follow up and maintain relationships with our recipients. They become part of
the JoshProvides family. In this manner, we can continue to provide support to them, as well as, maintain
accountability for our financial assistance.
The need of individuals with seizure disorders is great. What might seem like a small amount of financial
assistance has a significant and immediate affect on these individuals and their families.

“Our ultimate goal is to make life easier for those living with epilepsy. This is what Josh would want us to do”. –

There is no loss like losing a child or a sibling. In the English language, when you lose both parents you are an
“orphan” and when you lose a spouse you are either a “widow” or “widower”. There is no term when you lose a
child or a sibling. It has now been 8 years since Josh passed away from a fatal seizure. Sandi knew right away
we needed to do something for families who could not afford transportation expenses and the cost of their
prescription seizure medication.
Look how far we have come and there are many miles yet to go.
We have taken a tragedy and established support and a chance for those less fortunate. We often receive
requests for help from parents telling us their children are sleeping with them because they fear not being able
to hear them if they have a seizure at night. Today, there are methods, medications, dogs and devices that can
save lives when someone has a seizure. A gift from JoshProvides that purchases a monitor for a family is also
proving them with peace of mind and the comfort of knowing they will be alerted should something happen while
their loved one is sleeping. JoshProvides is proud to be able to assist these parents and their doctors with the
financial assistance they may need.
Josh is a special child, no different than our daughter and each of your children. Josh loved being a chef. It was
his way of providing for people. Because of Josh’s persona, his epitaph states: “You open your hand
and providefor all of the living”. Hence, in furtherance of our responsibility to all families living with a family
member who has epilepsy or another seizure disorder, and to perpetuate Josh’s persona, we named our
Foundation with the words “JoshProvides”. This is our brand and is what we do. We thank all of our many
donors and volunteers who have helped JoshProvides to be a caring and supportive arm to the epilepsy
community at large.
We pledge to you our continuing efforts to do what Josh would want us to do to continue to advocate for a
“seizure free day for all”.

Description
JoshProvides assists individuals of all ages who have epilepsy or another
seizure disorder with:
Financial assistance for • Transportation expenses to and from home, school, medical services
and employment, through the purchase from third parties of gas cards,
bus passes, taxi cab cards, or other third party transit options
• Medical services
• Prescription anti-seizure medication
• Seizure response dogs
• Seizure alert monitors (when recommended by the patient’s medical
doctor and not covered by insurance, other reimbursement plans, or
government-funded programs)
• Other expenses which are within the spirit of the Foundation’s charitable
purposes approved from time to time by the Foundation's Assistance

Short Term Success
JoshProvides provides itself in providing prompt response to inquiries for
assistance. Our review process includes an applicant completing a
Request for Financial Assistance Application and it is signed by the
patient or the parent if the patient is a minor. The Application is also
signed by the patient's doctor approving the specific request. If the request
requires seizure medication, the pharmacy must also sign the Application.
The Application and background information is then submitted to an
independent Assistance Review Committee for review. The response time
is generally within forty-eight hours thereafter. This is important especially
if the request is for seizure medication as it is time released and if the
body does not receive medication based on the patient's regimen, the
patient may be subject to uncontrolled seizures. Time is of the essence.
We are fortunate that this process works efficiently.
Long Term Success
Our grant award program allows both parents and patient independence
and the ability to have a normal daily life. For example, we purchase: (a)
seizure detection devices that set off an alarm or send a signal to a parent
or friend; (b) a seizure response dog to protect the patient from injury; and
(c) devices which minimize seizures. We are also exploring underwriting
pilot studies that seek a cure for epilepsy. The ultimate success is
determined by a person having a seizure free day. Today, there is no cure
and we must do what we can to help the families who are living with the
"FEAR": "Will I have a seizure today?" "Will I be called to school to pick up
my child following a seizure?" "Will my child die from having epilepsy?"
You cannot predict when a seizure will take place. Statistics show the
mortality rate for people with epilepsy is two or three times higher - and
the risk of sudden death is 24 times greater than that of the general
population. This is why we do what we do.

Program Success Monitoring
We believe our Program is successful based on our receipt of favorable
testimonials from our applicants. We also believe our success is on-going
as evident with the cross referrals we receive from epilepsy organizations
which do not provide the services we do and/or cannot react as quickly as
we can. The cross-collaboration among the various epilepsy foundations
is what makes this work. See our Testimonials in the Other Documents
Section of this Profile to observe how appreciative these families are.
Giving them peace of mind is worth its weight in gold. When a family can
say they have peace of mind, we know we have done something correctly.
Program Success Examples
Our program is doing well as reflected in the Testimonials listed in the
Other Documents section of this Profile. The following summaries
demonstrate our success: (1) I cannot begin to express into words how
thankful I am for you providing Joseph with an EMFIT seizure monitor. At
17 yrs old, Joey wanted his privacy. After reading Josh's story, I contacted
you for help. Joseph now has his privacy. I have peace of mind knowing
that Josh is keeping a watchful eye on Joey.
You have given me more than you know. May God bless JoshProvides;
and (2) We wish to express our gratitude for the generous financial aid
you provided us when my daughter’s insurance coverage was abruptly
canceled. Your assistance paid for the prescription cost. The caring
manner in which you quickly responded to our initial contact and the
expeditious processing of your financial contribution is a true blessing.

JoshProvides Peer Support Group
Description
JoshProvides assists in the establishment and maintenance of strategic
support groups for people living with epilepsy or another seizure disorder
and their families, friends, and caregivers. The Support Groups are
organized with a trained facilitator or counselor to monitor how families are
coping with their own family situation and engaging the other families to
explain how they are handling similar situations. It demonstrates that the
families are not alone and it provides them an outlet for their concerns. A
separate qualified facilitator meets with the youth and young adults to get
them to open up about their own feelings and concerns.

Short Term Success
Our short term success is measured the same as our long term success.
Do our families keep coming back? Yes. Do we want more families to
attend? Absolutely! We generally gain a new family from time to time and
another may not attend again. We encourage our participants to help by
asking families they know who have a family member with epilepsy to
attend. During the summer months there is clearly a drop off in attendance
due to vacations. We started with two families and at one meeting we had
over 20 in attendance. Are we successful? Yes! You see it in the
paricipants' eyes both before and after the meeting. Our sponsorship of a
support group is one of our most important endeavors. One of our goals is
to help organize many more support groups for people of different age
groups.
Long Term Success
The success of our Support Group Program is measured by the feedback
we receive from the participants both parents and young adults. After
each meeting, our participants thank the facilitators for the giving of their
time (an hour and a half) to be with them. Likewise, when you observe a
child who arrives at the meeting completely withdrawn and at the end of
the young adults session is smiling and talking you know we have done
something right. It is a credit to our facilitators and we are only the
catalyst to provide a meeting place for these families to interact with other
similarly situated families. Our success is measured by the relief we
provide these families and letting them know, "You are not alone!"
Program Success Monitoring
As described in Short Term Success, we know we are successful because
our participants thank us each month. They have no other outlet to
discuss their issues with anyone else who is experiencing the same or
similar issues. The children who attend leave smiling and talkative after a
Support Group Meeting because they know they are not alone. Epilepsy
can be socially isolating. As the Executive Director, I can speak from
personal experience.

Program Success Examples
The Support Group Meetings speak for themselves. Everyone leaves the
meetings feeling positive about themselves and their new friends. Helping
someone without expecting something in return is extremely gratifying and
rewarding. People who can share their experiences and situations with
someone else who experiences the same issues is worth its weight in
gold. Unless you have lived through it, it is hard to imagine what parents
and the children who have epilepsy go through on a daily basis. The
"FEAR" discussed at out meetings includes: "Will I have a seizure today in
front of my peers?" or as the parent: "Will I be called to school because
my child has had a seizure"; or "How do I tell my 16 year old you cannot
drive for 6 months after a seizure and what the next 6 months will be like".
Our families share these stories. They provide an outlet, hope and support
for each other, especially new parents of a child recently diagnosed with
epilepsy. This is how we are making a difference.

Our short term success is the same as our long term success goal. It is a
24 hour per day 365 days a year responsibility.
Long Term Success
Epilepsy Awareness is a 24 hour per day 365 days a year responsibility.
Each program we sponsor includes as its underlying theme epilepsy
awareness. It is our goal to continue to use social media, the press, Public
Service Announcements, e-blasts and general letter writing campaigns to
educate the public about epilepsy and seizure disorders. We forget that
many of our brave troops coming home from Afghanistan and Iraq suffer
from seizure disorders. We must educate the public that: (a) epilepsy is a
disorder and not a disease; (b) the incidence of epilepsy is equal to that of
breast cancer and takes as many lives; and (c) newly diagnosed cases
are greater than cerebral palsy, multiple sclerosis, muscular dystrophy,
and parkinson's disease combined. We annually celebrate November as
National Epilepsy Awareness Month. Success will be determined when we
can say all people with epilepsy have "Seizure free days" and there is no
longer a negative stigma associated with epilepsy.
Program Success Monitoring
We know our Epilepsy Awareness Program is working when we meet new
people and they advise us, "yes, we are very familiar with the statistics
regarding epilepsy and what must be done". Our education includes not
only the general public but also our schools and teachers. Epilepsy is not
contagious, it is a disorder.
Program Success Examples
Examples of our Awareness Programs include: (a) disclosure at each of
our fund raising events; (b) recipients of our grants advocating epilepsy
awareness by email and social media; (c) our own email and mail
campaign, website at www.joshprovides.org, social media and one on one
conversations. At each of our events, we try and have one of our
recipients talk about their experience with epilepsy. When people stand up
and applaud, you know we have reached them.

There are over 350,000 people in Florida with epilepsy. Although we cannot help everyone, we want to be the
Foundation people know that can provide prompt assistance when called upon. The programs and services of
JoshProvides immediately impact the lives of those with epilepsy. Whether it is a lifesaving device, a dog that

Experience
With over thirty-nine years of legal and business experience, Bruce is a member of the business and corporate
planning group and estate and gift tax planning group in the Sarasota law firm of Icard, Merrill, Cullis, Timm,
Furen & Ginsburg, P.A. He received his undergraduate degree in 1969 from West Virginia University, a Masters
degree in 1970 from The Ohio State University and his law degree in 1974 from West Virginia University. Bruce
serves as a member of the Business Law, Real Property, Probate and Trust Law Sections of the American Bar
Association and The Florida Bar. He is also an active member of the American Bar Association, The Florida
Bar, and the Sarasota County Bar Association; and an inactive member of The Ohio State Bar Association and
West Virginia Bar Association.
Bruce’s experience includes working in financial services, developing creative and sophisticated estate, gift and
charitable tax planning techniques and retirement benefit planning. He also served as Executive Vice President,
Secretary and Director of American Waste Services, Inc. and was responsible for coordinating the initial public
offering of the Company in October 1990. For fifteen years, Bruce practiced inYoungstown, Ohio, wherein his
practice emphasis was on business and corporate planning, mergers and acquisitions, capital financing and
lending, business succession, and estate and gift tax planning.
Bruce has spoken at a number of notable conferences and seminars on business organizations and planning,
estate and gift tax planning, tax shelters and security matters. He has taught advanced estate and gift tax
planning on behalf of the American College, of Bryn Mawr, Pennsylvania.
Bruce has served as a Board Emeritus for Epilepsy Services of Southwest Florida and is a Co- Founder of
JoshProvides Epilepsy Assistance Foundation organized in memory of Joshua David Chapnick
(www.joshprovides.org).

JoshProvides has collaborated with Epilepsy Foundation of Florida, Suncoast Epilepsy Association, Epilepsy
Services of Southwest Florida, Epilepsy Services Foundation, Inc. (Tampa), Chelsea Hutchison Foundation,
Epilepsy Foundation of Florida and National Epilepsy Foundation to assist epilepsy patients referred to us for
services including medication, surgeries, transportation, seizure detection devices, and seizure detection dogs.
JoshProvides also has collaborations with hospitals that include Sarasota Memorial Hospital, Doctors Hospital,
Palm Beach Gardens Medical Center, Cleveland Clinic, and Mission Healthcare.

staff person was added. Requests for support and assistance have increased and it is the goal of JoshProvides

Financial Comments by Organization
There are national organizations everyone in this country knows by name or even by their initials. JoshProvides
is a smaller, community based organization born out of the need to fill gaps for those living with epilepsy. There
are diseases people readily support and understand. Children and adults with epilepsy are shunned because of
their seizures. Friends and co-workers become scared and distant. The challenge for JoshProvides is removing
the negative stigma associated with epilepsy, encouraging communities to wrap their arms around those
affected by epilepsy, and to tell our story in a way that enables us to generate the funds to meet the needs of
those served by the organization. Our success can be measured by the testimonials we receive. For example:
Hello to all of you @ Joshprovides.org. We eagerly await receipt of the Emfit Monitor which you’ve agreed to
graciously provide. Our Emily has DX of Lennox-Gastaut Syndrome, a very severe intractable form of Epilepsy.

She bravely battles seizures most days. The effects of the seizures and the mental retardation, along with the
risks for injury from falls make a very difficult life at times for all of us. Her nights are often disrupted, which
means concerns for her health and safety are always upon us. When I became aware of the monitors I began
to wonder…… could that bring us peace of mind? From the first contact with your organization, I have been
treated respectfully and in a caring manner. I so appreciate that the process has been straightforward and easy
to access for your consideration. I want to sincerely express our gratitude and hope to report back very soon
about how this is helping us to establish timely and appropriate care response for our daughter’s needs and a
peace of mind to us, her parents and caregivers. Your focus toward giving to families who have genuine needs
