Charge Capture and Charge Interface Enhancements and Fixes To be compliant with the 5010 requirement, iKnowMed must report NDC codes and HCPCS codes with 5010 formatted amounts and units of measurement. Prior to this release, iKnowMed displayed the NDC and related information as read only in the Dispense Details dialog, and this information could only be modified in Lynx Mobile. 1.1 NDC Codes Editable in Dispense Details This release adds a Filter by Manufacturer field and a NDC field, and makes most Dispense Details fields editable. The drug’s Service History records changes to the Dispense Details. The All Visit Data Note, Audit Note and Charge Capture Report display this information, and Charge Interface messages include this information. 1. 2. 3. 4. 5. 6. 7. 8. Use the alphabetically sorted Filter by Manufacturer field to filter the NDC field. This field is not related to the Manufacturer field in the Admixture Details window. Enter or modify an NDC. Tab or click into the NDC field, and then press the Backspace key to immediately see an alphabetically sorted list of NDCs. When unfiltered by Manufacturer, this list includes NDCs for all non-obsolete, brand and generic forms. This list excludes negative NDC numbers. Each row must have a unique NDC. Enter or modify the Dispense amount, Waste amount, Lot Number and Expiration Date. Click “Add another NDC” to create a new Dispense Details row. The Filter by Manufacturer value is copied to the new row, but it can be changed. The Total row sums the Dispense and Waste amounts, and can be used for error-checking against the Charge Capture Report. If MAR Update is enabled, then Lynx Mobile provides the total Dispense and total Waste. Click the X on a row to delete a row. Deleted rows are permanently deleted after the Save button is clicked. Click the Save button to submit changes. Click Cancel to close the window without saving. 4 Copyright © 2013 McKesson Specialty Health. IV Hydration code (96360) is no longer missing from Charge Capture Report. This only occurred when multiple admin records were charted on a hydration only order and the first admin record's duration was less than 30 minutes. The Charge Capture Interface now sends the Seen By provider so that the Seen By is not overwritten by the Attending. In particular, this resolves instances where a Mid-Level Provider (MLP) and a physician update the progress note on the same day, and, in the charge message, there is no indication that the MLP saw the patient. 5 Copyright © 2013 McKesson Specialty Health. All rights reserved. 2. Lynx Mobile Integration 2.1 Return a Dispensed Drug When a drug is dispensed in Lynx Mobile, the iKnowMed Admixture queue displays the Status “D” for that drug. With the 6.5.0 release, when a dispensed drug is returned in Lynx Mobile, the Status “D” is removed. In the case of a partial Return, the Status “D” is retained and the Dispense Details may show positive and negative transactions. 2.2 Informational Notifications of Ordered Dose not Equal to Dispensed Dose A new notification displays when a dose does not equal the amount dispensed. No action is required on these notifications, and they are merely informational. These notifications display in the following areas: Dispense Details Cycle Day Drug Administration Drug Administration Record Charge Capture Report Charge Interface 2.3 PRN Orders in Lynx Mobile “PRN” is added to the medication name in Lynx Mobile when the order’s Frequency in iKnowMed is PRN. 3. Admixture 3.1 New Mixed By Field To help practices meet regulations in some states (specifically, Texas regulation 291.133 (e) (2) (A) (iii & iv) Pharmacies Compounding Sterile Preparations), the new Mixed By field in the Admixture Details window allows pharmacies to track who mixed the drugs, and get that person’s sign-off on the printed Admixture Report, which can be stored off-line. The Mixed By field: Is below the Pharmacist field; Is not required; Is sorted alphabetically, although “Other…” always appears last; Does not have a default for new admixtures, but defaults to the saved Mixed By name when editing an admixture; Only permits: letters A-z, space, comma, period, apostrophe and hyphen; and Is unique to each practice to keep the list short. 6 Copyright © 2013 McKesson Specialty Health. To update this field, choose a name from the list, or choose “Other…” Choosing “Other…” opens the Modify Mixed By window to create, replace and remove Mixed By values. 3.1.1 Add a Name To add a name to the Mixed By list: 1. Type a value in the New field. 2. Click the Add button. Clicking Add saves the New name to the Mixed By list. If the New name exactly matches an existing one, then the list does not change. 3.1.2 Replace a Name To replace a name in the Mixed By list: 1. Select the name in the Value drop-down list. The New field is set to the selected Value. 2. Modify the New name. 3. Click the Replace button. Clicking Replace saves the New name in place of the selected Value, and the fields are cleared so that more names can be added or modified. 3.1.3 Remove a Name To remove a name from the Mixed By list: 1. Select the name in the Value drop-down list. The New field is set to the selected Value. 2. Click the Remove button. Clicking Remove permanently deletes the selected Value, and the fields are cleared so that more names can be added or modified. 3.2 Held Drug Invalidates Entire Admixture Prior to this release, holding a drug removed that drug from the Admixture queue, but the held drug was still referenced by the other drugs with which it was mixed. In 6.5, holding a drug removes the held drug from the queue and also invalidates the admixtures for the other drugs with which it was mixed. 7 Copyright © 2013 McKesson Specialty Health. Home Page: News Section, Taller Roster, Change Roster, Patient Phone Numbers The redesigned Home Page is more usable and efficient, and keeps you informed of iKnowMed related events. This release adds a News section to the Home page’s lower-left corner. The News section can list scheduled maintenance times, links to releases notes and other information pertinent to those who use iKnowMed. The numbers in the News section header indicate the quantity News items that are listed, and of those the quantity that are important. Important News items have a red background. This list is managed by the iKnowMed Help Desk. The Home page's Patient Search field, Roster Name and Roster Date occupy less space so that the Roster can show more patients. The Home page Roster can be changed on the Home page making switching rosters easier for those who monitor multiple rosters. The patient's phone numbers (i.e., Home, Work and Cell) are added to the appointment information that is available by hovering on the patient's name. The patient's preferred method of contact is also indicated. 8 Copyright © 2013 McKesson Specialty Health. All rights reserved. 5. Encounter Notes: “…patient is seen for...”, “Subjective”/”Objective”, Multiple Diagnoses, Etc. The statement, "The patient is seen for the diagnosis..." no longer appears on any encounter note. Removed the Subjective and Objective headings from the Referral Letter and Basic notes. When a patient has multiple hem-onc diagnoses, the encounter note lists the visit's Primary hem-onc diagnosis first. When a visit has multiple Primary hem-onc diagnoses, then the encounter note lists the most recently entered diagnosis first. This change provides consistency around the order of diagnoses on encounter notes, and helps move documentation closer to the relevant diagnosis. Patient name, MRN and visit date are added to pages after page 1. Blood draw appears on the Basic Note. 6. Improved Dictation Error Handling with Updated ActiveX Control Improved dictation save error handling. Installed ActiveX controls (version 10.0.0.13) must be updated to take advantage of these improvements. 7. Progress Note: Duplicate Service Error and Dragon Improvements In rare cases, the progress note can contain a duplicate service, which prevents the progress note from saving. A new message alerts the provider that a duplicate service should be removed. This release improves saving Dragon entries after Dragon is used to navigate/click buttons. 8. E-Sign: Pink Highlighting Softened and Queue Count Improved The pink highlighting used to indicate transcription by another provider was too bright, and made it difficult to read the highlighted text. The bright pink was changed to a softer, rose color. Queue counts no longer include Review MLP items created before the Review MLP feature is turned off at a practice. The E-Sign Queue Counts no longer include a visit that moved from the E-Sign queue to the Transcriptionist queue after more dictation is added to the visit. 9 Copyright © 2013 McKesson Specialty Health. Attachments 9.1 Attachment Review Performance Increase, Print Multiple Attachments and Other Improvements The Sign button moves next to Print button to make it easier to click the Sign button after navigating within the attachment. iKnowMed now prefetches PDFs to speed up viewing attachments. When prefetching a PDF, a prompt may appear asking, "Do you want to load ActiveX content?" To permanently dismiss this prompt, check the box "Always perform this action for this file type", and click OK. This prompt may continue to appear for DOC, XLS and other documents because there is no option to permanently dismiss the prompt for those documents. Attachment Review defaults to a 30/70 split when the width of the window is greater than 1850 pixels. Otherwise, Attachment Review defaults to a 50/50 split. The split position is remembered using a browser cookie, so, to affect this change, please delete cookies. Providers can sign off attachments if patient does not have an attending. When sending a Chart Message from Attachment Review, the Date Attached and Attachment's Comments are inserted into the message to help identify the specific attachment the message is about. Prior to this release, technical issues prevented multiple attachments from being printed together. With this release, selected attachments are merged into a single PDF document, which can then be printed or saved. A list of errors appears if any attachments could not be merged into the PDF. 9.2 9.3 Updated Show Recent Attachments This release modifies the Show Recent Attachments window by separating the attachment's Comment from the attachment's Status (i.e., Reviewed, Awaiting Review, etc.). Attachments can now be sorted by Status in addition to the other columns. Deleted Attachments Accessible and Fixed Attachments Calendar Deleted attachments are accessible from the Service History. In the Service History, hover on the deleted attachment, and then choose the View Document spyglass option. On the Attachments page: The calendar reverts back to today's date, if the Service Date is not copied forward. Selecting a provider in Signature By sets that provider as the attending, if the patient does not have an attending. 10 Copyright © 2013 McKesson Specialty Health. All rights reserved. 10. Current Medications Allows Updates to Prescriptions In-house prescriptions can be updated in the Current Medications window. Updates here are not new orders, only annotations to the existing orders. The Date Started field defaults to the earliest order date for the term. When updates to a prescription are saved from the Current Medications window "C: " is prepended to the flowsheet value to indicate the prescription is continued. 11. Pick a Dose to Change By Percentage and Weight Change Alert Appears Once When changing a dose by percentage, the Change By Percentage dialog includes a new drop-down field listing all given dose amounts in chronological order, with the earliest listed first. The value visible in the field includes the drug’s admin date and the amount given (not as a formula dose). The default value is the earliest given dose. The percent is applied to the dose chosen in the new field to produce the new dose. The Weight Change Alert no longer appears after drugs are changed due to a previous Weight Change Alert, unless the weight changes by 10% after the last Weight Change Alert. 12. ePrescribe: New Match Confirmation, Inactive Pharmacy Handling, PRN Refills, Etc. To reduce the number of electronic prescriptions in wrong charts, if the patients' Birth Dates do not match, then a match confirmation prompt appears when matching renewal requests to patients. When ordering a new prescription, an electronic prescription cannot be sent to an inactive pharmacy, and a message appears indicating the pharmacy is inactive. This release prepends "(Inactive)" to the pharmacy's name. Inactive pharmacies are not displayed when choosing a patient's pharmacy. During provider registration: 1. Validates provider names do not contain periods, and 2. The application emails Support and Implementation when there is an error during provider registration. 11 Copyright © 2013 McKesson Specialty Health. All rights reserved. Pharmacy Type and 24 hour availability can be used to find pharmacies to add to charts. iKnowMed supports NCPDP SCRIPT 10.6 mu2014 as required by SureScripts. Rollout of NCPDP SCRIPT 10.6 will occur shortly after upgrading to 6.5.0. The following changes will then be implemented: PRN refills are no longer compliant, and the PRN checkbox is disabled, The full eRx renewal message is available by clicking the “View all messsage details” link, and 13. Lab Results 13.1 Requeue Result to Result Review Queue It is sometimes necessary for a provider to change a result's Clinical Significance or Patient Communication fields after the result is reviewed. Version 6.5.0 adds a Requeue button to the spyglass options for results. To requeue a result: 1. Click the Requeue spyglass option, 2. Enter an optional "Reason", and 3. Click the OK button. 12 Copyright © 2013 McKesson Specialty Health. All rights reserved. The Reason does not appear in the Result Review queue, but does appear in the Workflow Detail. The result will be awaiting review and unreleased in the ordering provider’s Result Review queue. Results received after those that are requeued are not visible in the Result Review queue. Entries in the Patient Communication fields and the Clinical Significance are retained and editable. 13.2 Other Result Review Changes Abnormal results and the CS and NCS options are bolded so that they are more easily seen. To show more results without scrolling the Result Review queue no longer shows the laboratory's address for past results. To be CLIA Compliant, the lab address must show next to the result (analyte) being reviewed when the result's lab address is different than the panel's lab address. 13.3 Historical Clinical Significance No Longer Suppressed The Clinical Significance of historical abnormal lab results is no longer suppressed when the most recent Study patient value is not "Active". 13.4 Lab Trend: Critical Low Changed from “L” to “CL” The Lab Trend Report displays Critical Low as CL, not as L. 14. Orders 14.1 Order Cosignature: Press Enter to Move to Next Patient When reviewing a patient's orders in the Order Cosignature queue, press the Enter key to move to the next patient's orders after clicking any checkbox. 14.2 Order Entry Tool: No Default On Behalf Of The Order Entry Tool's On Behalf Of field no longer defaults to a provider. The iKnowMed application often unsuccessfully defaulted the incorrect provider in the Order Entry Tool's On Behalf Of field, resulting in the wrong provider receiving the orders in the Order Cosignature queue. We considered many options and suggestions about which provider should be defaulted, and decided to not default a provider. 14.3 Order Date Editor: Past Order Dates Do Not Erase Future Order Dates When selecting specific dates for an order, past dates no longer cause future dates to be erased from the order's Date editor window. 14.4 Show Recent Orders: Discontinues Italicized In the Show Recent Orders window, discontinue orders are italicized. 15. Quick Login: Removed Patient Name The Quick Login page displays the patient's MRN or PATIENTID, but no longer displays the patient's name in an effort to keep PHI more secure. 13 Copyright © 2013 McKesson Specialty Health. All rights reserved. 16. Regimens: Enter Historical Dose Once, ROE On Behalf Of Fixed, Etc. The application prompts for a drug's historical dose only once. Before, if you started a regimen, entered the historical dose, then clicked Cancel on the Confirmation page to correct an order in the regimen, you would be prompted to enter the historical dose a second time after starting the regimen again. In the Regimen Order Editor, the current user's name is now excluded from the On Behalf Of list, and the selected name can be removed by backspacing. When changing the Cycle Length with prior unapproved and unheld cycle days a message preventing the change appears. With this release, the message displays the current Cycle Length, so that the Cycle Length can still be viewed eventhough it cannot be changed. Prior to this release, as a result of running the Death Details knowmed template, only 1 in 3 regimens was discontinued. With this release, all active regimens are discontinued. 17. MA Template: Blood Pressure “0” Error Prior to this release, in the MA Template, if “0” (zero) was entered for systolic or diastolic Blood pressure, the MA Template could not be saved. Now, a new error message appears alerting the MA of the problem so that it can be corrected before attempting to save the MA Template. 18. Patient Demographics: Race, Ethnicity, Preferred Language and More A patient's Race and Ethnicity can be seen by hovering on the patient's name in the flowsheet. Ethnicity includes the option ”Declined”, as required by Meaningful Use Stage 2 (MU2). Preferred Language, updated for MU2, now uses ISO 639-2 language codes and is better able to match those codes to data received by interface. The Save button is now disabled on the Patient Demographics window until a change is made. New read only “Created” field indicates when the chart was created and who created it. Placed after “Other Contact Info”. “…by HL-7 Interface“ indicates the interface created the chart, while any other name indicates that chart was manually created in iKnowMed. Because this information was not tracked prior to this release, charts existing before this release will show "information not available". 19. Batch Print: Provider Added to Manifest and Fixed Approved Treatments Printing on Scheduled Orders The provider who dictated appears on the Encounter Note batch print manifest. When batch printing Scheduled Orders, approved treatments also print. 14 Copyright © 2013 McKesson Specialty Health. All rights reserved. 20. Interface Monitor: Resubmit Warnings and Errors and Filter Labels Renamed The Interface Monitor now allows any Warning or Error message to be resubmitted from the Interface Monitor. This is useful for resending appointments and results that failed because a provider or result was not mapped properly. The Interface Monitor stores the last 30 days of messages. The Interface Monitor's filter options: "Show Accepted" and "Show Non Labs" change to "Also Show Accepted" and to "Also Show Non Labs", respectively, to reduce confusion around their functionality. The Interface Monitor shows rejected results by default. Check to "Also Show Accepted" to view accepted messages in addition to rejected messages. Check "Also Show Non Labs" to view appointment and demographics messages in addition to results. 21. iKnowMed Supports IE10, LOINC and SNOMED-CT iKnowMed supports the Internet Explorer 10 browser. LOINC and SNOMED-CT codes are now fully supported in iKnowMed. Look for upcoming Content Releases regarding the availability of these codes. Support for these codes is a Meaningful Use Stage 2 requirement. 22. Faster iKnowMed Account Inactivation Inactivating a user account is typically faster because the application only attempts to deregister the associated ePrescribe account if the user account was registered for ePrescribe. 23. Contact the iKnowMed Help Desk Character Limit Raised to 100,000 To encourage use of the Contact the iKnowMed Help Desk window, the description field's character limit is changed from 500 to 100,000. 24. Patch Release Addendums The following sections describe software changes after the initial release of version 6.5.0.b238 (version 6.5.0 build 238). The software changes are first divided by the Build number released. To determine iKnowMed’s current Build number click the Window menu’s About iKnowMed option. The Build number is below the Close button, and begins with the letter ‘b’. 24.1 Version 6.5.0 b247 (July 25, 2013) Fixed a problem that prompted the provider for an E-Sign PIN between signing notes. Providers should be prompted to enter the E-Sign PIN when they attempt to leave or close the View E-Sign window and after they E-Sign the last visit in the list. 24.2 Version 6.5.0 b253 (August 4, 2013) Improved the saving of knowmeds and dictation. Updated the USON Helpdesk phone number when a screen resolution error occurs. 15 Copyright © 2013 McKesson Specialty Health. All rights reserved. Fixed a problem in which a regimen was missing cycles. Corrected a problem that allowed a DC to appear as the latest entry when it was not. 24.3 Removed the Requeue button from the Edit Result window. The Requeue spyglass option is still available when hovering on a result. In Attachment Review, when printing multiple attachments, the “MergeAttachmentsToPdfServelet” error no longer occurs. Corrected a situation that generated vast quantities of charge records. Reverted a change to encounter notes that caused the Assessment and Plan to appear in the middle of the note, instead of the end. 24.4 24.4.1 Pharmacy Miscellaneous Corrected a problem that produced a MAR Update Error when the dispensed dose equaled the order dose. The Time picker now displays when clicking in a Time field in a knowmed. Corrected file skips on the Attachments page. This occurred when clicking Cancel after opening a document from the Duplicate Check list. On the Show Recent Attachments list, the Comment is no longer truncated when certain characters are entered. 24.5 Home Page Roster The patient's zip code defaults into the filter when searching for a pharmacy. To filter by phone number it is no longer necessary to enter the entire phone number into the Area Code field. 24.4.3 Version 6.5.0 b275 (September 13, 2013) Fixed the "Show only my appointments/visits" option in the Roster definition. This problem resulted in inconsistent roster behavior. Enabling the "Show only my appointments/visits" now shows the appointments and visits for the clinicians listed in the Include Clinicians field, and excludes all others, including the currently logged in user. This change can affect providers using rosters other than the Clinician Roster. To correct affected rosters, consider checking or unchecking the "Show only my appointments/visits" option on the roster. Also, for providers who only need to see their own appointments, the “Clinician Roster” is the ideal roster. The Home Page Roster is now optimized to include the recently added patient contact info, which caused the roster to load slower. Rosters should load in the normal amount of time. The Home Page Roster drop-down list is now disabled for a user with the Restrict to Flowsheet permission. 24.4.2 Version 6.5.0 b263 (August 16, 2013) Version 6.5.0 b286 (September 18, 2013) Improved the eRx Message Queue to prevent servelet errors. Changes made in the Patient Demographics window are consistently saved. Queue Count Calculations are improved. This was a problem for busier databases, and sometimes caused a problem when holding a regimen cycle. In the Attachment Review queue, after clicking the Customize button, the provider list shows all providers and no only 50 providers. Also, the list can now be filtered by Most Common, Current Practice and All Practices. 24.6 Version 6.5.0 b297 (October 4-8, 2013) Corrected Admixture related changes, made in the initial 6.5.0 release, which exposed a problem that significantly diminished system performance and caused timeouts to occur. Interface messages no longer match on deleted patients. Fixed two scenarios in which the UOM was not available on the Charge Capture Report and not sent over the Charge Interface.
