fyou are looking for excellence in FOOT and ANKLE care; if you are
looking for a doctor who practices state-of-the-art Medicine; if
you are educated enough to understand that you may only want
an ORTHOPAEDIC SURGEON to touch your feet, then you owe it to
yourself to make an appointment.
There are many people with very serious problems of the foot and ankle
who go from doctor to doctor and remain undiagnosed or poorly treated.
The typical patient coming to see Dr. Samotin has been evaluated by many
other doctors. The patient usually finds that Dr. Samotin is the last doctor
they have to see.

surgery, and previous patients were having wonderful success. Deciding to go the route of adult stem
cell therapy, his eight ounces of blood was drawn
and hand couriered to Israel to undergo cultivation
while he prepared to travel to the Dominican
Republic just 5 days later.
Howard Lindeman
In 2009, Regenocyte™ gave Mr. Howard Lindeman
an alternative to open heart surgery.
While being admitted to the hospital and informed
of needing quadruple bi-pass surgery, Howard
Lindeman was shown another option for treating his
failing heart - adult stem cell therapy. Regardless of
what the therapy entailed, it sounded vastly less
nerve-wracking than the traumatic heart surgery.
Reaching the hospital in the Dominican Republic,
Mr. Lindeman was pleased to find what he rated as
an outstanding hospital facility. The courteous staff
and amenities relieved the stress of the unknown
and the short procedure was underway quickly.
Returning to the United States only a few days after
his therapy, he resumed his working schedule,
which just so happened to be engineering the sound
for the Pope's papal visit to New York City. Careful
to give his body time to adjust, he was slowly
growing excited as his energy level was gradually
increasing. Now a year later, Lindeman is racing
through airports to reach his next working destination more than pleased with his decision to have
adult stem cell therapy.
The fascinating procedure of adult stem cell therapy
is quite unique. Simply stated, adult stem cell therapy
is a process in which ones own stem cells are
extracted from the patient via a simple blood draw
then isolated, therapeutically strengthened, and
returned to the body where the cells begin to repair
the damaged area. The blood is drawn from the
Regenocyte™ offices in Bonita Springs, Florida. It is
then hand couriered to a state of the art laboratory in
Israel where the separation process of stem cells from
the blood occurs. Once the stem cells are extracted
and have heen strengthened, they are couriered to the
hospital where the insertion process is completed.
As a sound engineer that travels around the world
bringing the latest in sound technology to various
productions, concerts, and public addresses, Mr.
Lindeman was extremely busy and open heart
surgery was going to be a major time complication.
The idea of adult stem cell therapy had many
positive sides to consider for the man where time is
of an essence: the stem cells were his own, the procedure could be completed within one week, the
recovery time was vastly shorter than open-heart
heart dead, Neim Malo realized that his life expectancy was shortened and his quality of life diminished. Fortunately, he reached out and grabbed hold
of the right solution - adult stem cell therapy.
Armed with his risk-it-all personality, Malo traveled
overseas to undergo his stem cell therapy. Malo's
heart was going to be repaired by his own cells
therefore eliminating any chance of the body
rejecting the treatment. Before the adult stem cell
therapy, Malo had an Ejection Fraction (EF) rate of
28% and it was dropping. Simply put, his heart was
severely damaged and was operating at a dangerously low level.
Living was limited to observing - slttmg and
watching others do what he longed to do himself.
Malo referred to his pre-therapy days as, "living as
half a man."
Having undergone the treatment four years ago, it is
safe to say the therapy worked! With his Ejection
Fraction rate dramatically climbing, he reached a
much healthier 50% EF rate. Now, he dashes
around the restaurant meeting and greeting diners
with enhanced enthusiasm.
Both men had heart conditions that needed to be
treated directly, efficiently, and quickly if they were
to continue their fast paced lives. Due to Dr. Grekos
and the team at Regenocyte™ committed to the
cause of adult stem cell research and the belief that
the therapy can change many lives, two men have
lives to live and stories to tell.

TAKE (LONG-TERM) CARE
Don't overlook this essential part of retirement planning.
By George T. Leamon, CLTC
ost retirement planning is
about means, not ends. It's
about making sure you have as
many options as possible, not telling you how to
spend your money. This month, however, I want
to suggest that you set aside a significant amount
of your monthly budget for a purchase that is
often considered somewhat controversial: longterm care insurance.
Long-term care includes medical and nursing
services beyond your typical visits to the doctor,
treatment for illnesses and short hospital stays. It's
the kind of ongoing help you need when you can't
handle activities of daily living, such as bathing,
eating, or dressing. It can take place in your home,
assisted living facilities (residences with services
that monitor your health and provide your meals), or
nursing homes.
The case for insuring yourself and your family
against the costs oflong-term care is straightforward
and essentially statistical. First, however healthy and
hearty you may be today, you might need help
tomorrow. Americans age 65 and over have a 40
percent chance of entering a nursing home at some
point during their lives, according to the Department
of Health and Human Services. Meanwhile, today
55 percent of Americans age 85 and over are
impaired seriously enough to require long-term care,

according to America's Health Insurance Plans, a
trade group. Many of these seniors will get help at
home, but a good portion of them will have to move
into a facility.
Second, this kind of care is extremely expensive and
getting more so every year. The annual cost of
private nursing-home rooms averages a whopping
$75,190 in 2006, up 7.3 percent since 2004, according to the Met Life Mature Market Insurance. The
average cost of 12 months in an assisted-living
facility was $35,616 in 2006, up 17.6 percent since
2004-and that doesn't include extra charges that
some impose for dementia care.
Third, for most Americans, long-term care isn't
covered by other types of insurance. Don't be
confused by the fact that Medicare pays for "skilled
nursing care." This refers to short-term help you may
require to get over an injury or acute illness. If you
break your leg, for instance, Medicare will cover the
cost of "medically necessary" care, like pbysical
therapy, for 20 days. (It will then cover part of your
costs for another 80 days, after that you're on your
own.) But, if you are cbronically sick or permanently
incapacitated, Medicare will NOT help. Neither will
Medigap or Medicare Advantage policies.

The question people ask most often is
"What is MY risk?"
It's a valid question, but the fact is your real risk of
needing long-term care at some point in your life is
either going to be 0% or 100%.
Either you will need long-term care or you won't.
Averages merely show how many people out of every
100 have a need. Nice information to know, but no
prediction of your individual real risk and need.
Everyone is at risk.
Once you understand that a risk exists, your two big
decisions are: I) how do you handle the risk .. .. and 2) if
long-term care insurance is an option, how much protection is appropriate. The more protection you want, the
more coverage costs. IMPORTANT NOTE: An experienced long-term care insurance professional can help
you take advantage of discounts and show you ways to
maximize your coverage even when budgets are limited.
Since no one can predict your true risk, the best we can
do is provide information on what happens when
someone with long-term care insurance has a claim.

n estimated one million people are
newly diagnosed with gallstones
each year. Out of these patients
diagnosed approximately 500,000 will undergo
gallbladder removal surgeries (cholecystectomies).
Gallbladder disease is caused by gallstones, or a
poorly functioning gallbladder (biliary dyskinesia).
If a person has gallstones, the stone can block the
outflow of the gallbladder and cause it to become
greatly inflamed and possibly rupture. In a poorly
functioning gallbladder, it fails to contract well and
many times a patient will have vague right upper
quadrant pain and excessive belching especially
after meals. Many times patients live with this condition for many years because their abdominal ultrasound fails to show any gallstones. It is not till your
doctor orders a study called a HIDA scan that your
poorly functioning gallbladder can be diagnosed.
Gallstones or a poorly functioning gallbladder can
cause right upper quadrant pain, nausea and
vomiting and fever. These symptoms usually begin
right after meals and sometimes resolve several
hours later. If you exhibit these symptoms you
should see your doctor immediately so he can order
the appropriate tests. The earlier you diagnose gallbladder disease the greater chance you have to get
your gallbladder removed laparoscopically.
Single incision (scarless) surgery is the latest technique in gallbladder removal. Single incision
surgery is considered scarless because the incision is
hidden in the navel and is rarely visible. The gallbladder dissection for removal is the same as the traditional way, but the ports are all placed through the
navel. I have found that patients have the same pain
as the traditional manner, but return to work the
same time if not sooner. The best part is that friends
or family members can rarely see the incision
hidden in your navel. I performed the first single
incision
gallbladder
removal
surgery
(cholecystectomy) at Gulf Coast Medical Center
and Cape Coral Hospital in 2009 and have performed hundreds since that time. The response has
been incredible from patients and their family
members because the incision is not visible and their
pain was minimal.

bile acids in the digestive track. Your body usually
adjusts to the bile salts, but if you continue to have
diarrhea several weeks after surgery Welchol or
Questran has been proven effective. Please speak to
your doctor before taking these medications.
One of the greatest misconceptions of gallbladder
surgery is that a patients diet will be limited after
the surgery. This is not the case. A patient can
slowly resume their normal diet within one to two
days after surgery. You will not be able to lift heavy
weight (greater than 40 Ibs) till 3-4 weeks after
surgery. If you have a desk job, many patients
return in 7-10 days. A patient may have diarrhea
after gallbladder surgery and this due to excessive
Gallbladder surgery has evolved from a large foot
long incision below your right rib cage to four
small laparoscopic incisions to one incision hidden
in your navel. It is incredible how quickly minimally invasive surgery has evolved. If you have
gallbladder disease don't hesitate and please call
today to schedule your appointment. Please see my

Hernias: Who Has Them and How To Fix Them
I By Robert Tomas, D.O., FACOS - Board Certified General Surgeon
he repair of an inguinal hernia is the
general surgeon's most common
elective surgery. There are many different types of hernias and many people develop
hernias simply from lifting heavy objects every day.
I am going to talk to you about the two most
common hernias I encounter in my office: inguinal
and umbilical hernias. Inguinal hernias are defects in
the in lining of the abdominal wall that do not close
properly during development. The defect is at the
entrance to the inguinal canal (groin) and causes
problems to the patient later in life. The problem
arises when someone tries to lift something heavy
and the bowel or fat can protrude through the defect
and enter into the inguinal canal. The patient
develops a bulge in hislher groin that can resemble
an egg. A patient who becomes obese can form an
inguinal hernia by allowing the defect to stretch
larger in the groin because of the amount of weight
that the abdominal wall must support. This stretching
allows the intestine or fat to escape into the inguinal
canal and this causes a bulge in the groin. Umbilical
hernias are defects in the lining of your abdominal
wall in the region of the navel. This is the area where
your umbilical cord was during development. These
muscles fail to unite and a hernia develops. Umbilical hemias are more common in infants than adults.
These hernias in infants usually close by the age of
one. If an umbilical hernia does not close by the age
of four or if it appears during adulthood it should be

repaired. In an umbilical hernia you will see a bulge
or protrusion in the navel or bellybutton that gets
larger with lifting, crying or even coughing.
So what do you do if you find a bulge or mass in
your groin or bellybutton? Well, first see your
family doctor or local surgeon and ask him to
examine you while standing up. It is very difficult
to detect a hernia on examination if the patient is
not standing. Why? Gravity always brings your
abdominal contents down and if there is a defect in
your groin or navel it should be detectable. Your
doctor will stick his fmger in through your scrotum
up to your groin if you are a male and place his
finger on your groin if you are female. At that time
he will ask you to cough to see if he feels the sac
push against his finger in order to make the diagnosis. It is the same thing for an umbilical or bellybutton hernia. Standing always exhibits more pressure
in the area of the navel and your doctor will ask you
to cough in order to produce pressure in your
abdominal cavity. This will then cause your umbilical hernia to protrude making a clear diagnosis.
So once the diagnosis is made the repair is the interesting part. We used to approximate bad tissue to bad
tissue in multiple layers with mesh until we found out
that recurrences continued to rise. Now, the most
effective treatment is a tension free repair. We have
developed many great meshes that sit above and

in place and this requires a minimum number of
sutures. Less sutures means less pain and no one
likes pain. I have been using this type of double
layer mesh in the groin and the umbilicus for many
years now and have seen incredible improvement in
results. I trained with laparoscopic repair and have
abandoned the laparoscopic technique because I can
do this new type of hernia repair with intravenous
sedation. That's right, general anesthesia is not
required with this type of repair. Therefore, I have
performed numerous hernia repairs in our U First
Surgical Center with intravenous sedation alone.

Colorectal Cancer
I By Dr. Michael Smith
olorectal cancer is the fifth most
common cancer and third leading
cause of cancer related death in the
United States. Since I in 12 people will be diagnosed
with colorectal cancer it is likely that either you or one
of your friends or family members will be affected. For
that reason, it would be helpful for you to understand
some basics about the treatment options.
We believe the most common type of colorectal cancer
originates in polyps, especially adenomatous polyps,
which are mounded up growths of abnormal cells on
the lining of the bowel. Over time, some of these
polyps can degenerate into cancers which can invade
the bowel wall and spread via the blood and lymphatic
systems. By using routine screening colonoscopy to
fmd and snip out these polyps before they get a chance
to degenerate into cancer many lives are saved!
If a cancer is found at the time of colonoscopy,
surgery is performed to remove the tumor and assess
whether the tumor has spread into the lymph nodes.
Depending on the findings, chemotherapy either as
a pill or through the veins for up to one year is often
recommended.
A special situation arises when the cancer occurs in
the final part of the colon near the anus. This portion
of the colon is known as the rectum. Tumors in this
area are often treated with a combination of chemotherapy and radiation to shrink them before surgery.
The most commonly used type of radiation is high
energy x rays. These are the same type of x rays used
in chest x rays or CT scans, except they are high
energy to they penetrate in to kill cancer cells. Radiation therapy is given once a day, five days a week for
about 5 weeks. Today, the delivery of the radiation is
highly controlled so that it is focused in to the tumor
containing area along with the nearby lymph nodes
and hits a minimum of normal tissue. The most
accurate means of delivering radiation involves use of
image guided intensity modulated radiotherapy in
which a cone beam CT scan is performed each day to
provide the ultimate in accuracy. About 2/3 of the
radiation effect is due to the formation offree radicals
which then damage the DNA and other components of
the cells. The remaining effect is due to direct damage
to the DNA itself. Fortunately, normal cells are better
able to repair this damage than cancer cells are.
Side effects of the radiation are generally mild.
These include the possibility of skin reaction, such as
skin redness and itching much like a mild sunburn.
This is temporary and goes away after the treatments
are over. Some patients then have a subtle tanning
within the treated area, but this too goes away with
time. Fatigue is also common, but is temporary and
also goes away. These effect vary substantially from
patient to patient.

the most common result is a hip fracture.
It is important that we take steps to help to
prevent bathing disability before it occurs.
Walk in bathtubs (Safety Tubs) are specially made
with a non-slip floor. This helps in avoiding
common bathroom accidents due to slipping. Safety
Tubs are a great help to people suffering from
certain injuries, specific disabilities and difficulties
due to old age. These tubs are different from traditional tubs. To enter in these tubs, users don't have
to climb in. There is a special door for entering into
the tub. Elderly and disabled can't climb in easily.
Moreover, climbing can be accidental for them.
Thus entering in and walking out of the tub through
a door is more convenient.
People suffering from severe pains, back aches,
arthritis, rheumatism and various other pains are
also comforted by these bathtubs. This is possible
because of the bench like seat present in them. It is
easier to sit on the seat than lying on hard surface
of the conventional tubs. It is very painful to lie
down and get up for people suffering from pain.
Getting up requires a lot of pressure that increases
the pain.
Safety Tubs with a built in bath seat allows you to
enjoy bathing and feel safe getting in and out of
the tub. Bathing is the number one way for people
to maintain a clean, fresh feeling while helping to
lower blood pressure and reduce stress. Doctors
recommend bathing in warm water as a way of
improving arthritis and many other conditions.
If you (or a loved one) are facing a health or
mobility challenge, you want to live independently and manage your health and pain challenges... don't you? A Safety Tub will not only
help you to bathe safely, but can actually offer life
altering health benefits.

EYELID RE UVENATION
T
I By
Stephen J. Laquis MD, FAGS
he most vital and valued function of
the eyes is that they provide us with
the gift of vision. Beyond that, they
are an essential part of how we convey emotion and
engage with others. Tbey are often the first thing
noticed by others and contribute greatly to one's
overall aestbetic appearance. Rejuvenation of the
eyelids is the most commonly performed elective
surgical procedure and serves not only to improve
ones appearance, but also bas the essential benefit of
.
.
..
Improvmg VISIOn.
Upper and Lower Blepharoplasty - Before
UPPER EYELID REJUVENATION
While age is strongly correlated witb "heavy
eyelids", genetics play a vital role in determining
your predisposition for this condition. Furtbermore,
the skin of tbe upper eyelid is the tbinnest skin in the
body. It can easily stretch from the mere act of
blinking or as a result of medical conditions that
cause recurrent eyelid swelling such as allergies,
heart failure, or sleep apnea. One or a combination
of these factors will cause the skin to stretch to such
a degree tbat it ultimately will start to cause
symptoms. Most commonly the excess skin of the
upper eyelids will affect peripheral vision by
extending over the eyelasbes. Many times this goes
unnoticed until it is very advanced as the process is
gradual and most people develop adaptive mechanisms. Otber symptoms are also common such as
tired or heavy feeling eyes or difficulty performing
activities of daily living that require concentration
such as driving, reading or computer work. Intervention therefore is many times medically beneficial.
Upper Blepharoplasty - Before
After
The surgical procedure to correct this condition is
called an Upper Eyelid Blepharoplasty, and is
covered by most insurance policies, including
Medicare. A simple photograph and visual test is
required for approval and can be done right in our
office. Once submitted to your insurance company,
the procedure is often deemed medically necessary, and authorization is obtained. The procedure
is performed in an outpatient setting with both a
local anesthetic and "twilight" type intravenous
sedation. The procedure is quick and virtually
painless. Recovery time is minimal and usually
limited to 1-2 weeks of swelling and limited
bruising, which usually subsides quickly with a
post-operative regimen that includes the application of cold compresses. It is generally recommended that strenuous activities be avoided for at
least ten days after surgery.
LOWER EYELID REJUVENATION
Similar to upper eyelids, age and genetics can cause
changes to the lower eyelids. While excess skin may
be an issue with the lower eyelids, often the problem
is primarily the result of fatty pockets protruding
forward. This is addressed with a procedure called a
Lower Eyelid Blepharoplasty, one which can
achieve dramatic results. Usually, this procedure
does not require an external incision, which means
no scar to worry about. It is accomplished through a
small incision on the inside lining of the eyelid, fatty
pockets are sculpted or removed. A chemical peel is
many times applied to the external lower eyelid skin
to tighten the skin and smooth out fine lines. Occasionally removal of lower eyelid skin is also necessary. Recovery time is 2-3 weeks, and requires cold

younger looking, refreshed YOU! While this is considered a cosmetic procedure, the overwhelming
response from our patients is that it was worth every
penny. Many times patients choose to combine procedures and have an Upper and Lower Blepharoplasty during the same surgical time ... the benefit of
course is tbat you then just bave one recovery period
and can save on surgical costs.

Dental Prophylaxis and the Periodontal
Maintenance Visit
Dental Hygiene Month
D
I By Juan Teodoro, D.M.D.
first few visits to make sure the patient is periodontally stable. Once the patient has a good history of
stability, then the 3-month maintenance visits start
alternating with the general dentist. This allows the
periodontist and the general dentist to work as a
team in maintaining the patient's periodontal health.
entists refer to a dental cleaning
appointment as a "dental prophylaxis". The main purpose of this
visit is prevention of dental disease and patient education. This is likely the most important and
valuable dental visit a patient may ever make to the
dentist. A dental prophylaxis is done on patients
that display gum pockets of3mm or less, no significant gum inflammation and no
heavy
calculus/plaque under the gum line. A routine
dental cleaning typically includes: an oral hygiene
evaluation, home care instructions and a demonstration on proper hrushing & flossing. Removal of
plaque and tartar above the gum level is also done
along with a topical fluoride treatment after polishing the teeth. Many hygienists may also review
nutrition habits or smoking cessation if warranted.
Some patients require further gum care if they
display signs of periodontal disease. If so, the
dentist or hygienist would recommend a comprehensive periodontal evaluation and treatment by a
periodontist. A periodontist is a dentist who specializes in the prevention, diagnosis and treatment
of periodontal disease and in the placement of
dental implants. Periodontists are also dentistry's
experts in the treatment of oral inflanunation. They
receive extensive training in these areas, including
three additional years of education beyond dental
school. Periodontists are familiar with the latest
techniques for diagnosing and treating periodontal
disease. Depending on the patient's severity of
disease, a periodontist may recommend scaling and
root planing, gum surgery, regeneration or PerioLase laser therapy.
After a patient has been diagnosed and treated for
periodontal disease, a standard cleaning (dental prophylaxis) is no longer appropriate. While a standard
cleaning addresses scaling and polishing of the
teeth above the gum line, a person who has experienced periodontal disease requires meticulous care
below the gum line - a maintenance visit. A periodontal maintenance procedure is not the same
treatment as a regular cleaning even though a
hygienist may perform both services.
A periodontal maintenance procedure includes:
• An update of your medical and dental history
• X-ray (radiographic) review
• Intraoral and extraoral exam of the cheeks, lips,
gums and throat
• Periodontal Probing (around the tooth, gum and
bone) an examination done by the periodontist
• Review of home care with demonstration of
brushing & flossing
• Removal of Plaque & tartar above & BELOW the
gum level (Scaling & Root Planing)
• Polishing & removal of staining
• Gum and pocket irrigation with medicine, as needed
• Topical fluoride application
The typical maintenance interval, especially immediately after active periodontal treatment such as
scaling and root planing, surgery, regeneration or
PerioLase laser therapy is every three months. This
is usually done at the periodontist's office for the
The recall interval may be lengthened or shortened
over time depending on the patient's history of stability and their effectiveness of home care. The
important thing to remember is that once a patient
has been diagnosed and treated for periodontal
disease they become a periodontal patient for life.
Similar to diabetes, there is no cure for periodontal
disease. The goal is to prevent recurrence and
further progression of the disease, by adhering to a
regular maintenance schedule. It has been shown in
multiple scientific studies that those patients that
do not comply with maintenance visits experience
more periodontallbone degeneration, possible
tooth loss and ended up requiring more surgery.
Some people tend to have more buildup of calculus
than others and some may be more prone to periodontal inflammation or the development of tooth
decay. Patients with inadequate oral hygiene practices require more frequent cleanings.
Dr. Teodoro is a Board Certified
Periodontist. He holds both Diplomate status in the American
Academy of Periodontology and
the Intrenational Congress of Oral
Implantologists.
Dr. Teodoro
served as a Major in the u.s. Air
Force practicing periodontics.
Throughout his Air Force career he served as Chief of
Periodontics, Director of the Implant Board &
Clinical Instructor. He is currently a clinical assistant professor at the Naples dental residency program
for the University of Florida. Dr. Teodoro is still the
only periodontist in both Lee & Collier County
trained in Advanced Laser dentistry.

s parents, we know our children
best. We listen to them talk and we
also listen to their friends and siblings talk, but despite knowing better we often find
ourselves comparing our child's skills to that of their
friends and siblings; especially when it comes to
communicating. This often leaves us asking ... "Is
my child developing at the same pace?"
When we think our child's development may be
slower than their same aged peers, we might ask
others (teachers, friends, pediatricians) if they have
noticed these differences. A common response from
others might be, "My daughter didn't talk until she
was 3 and she's making up for it now" or "Give it
some time, he's still young."
Too often however, kids don't grow out of it. This
can sometimes lead the child to get caught up in a
vicious cycle of always trying to 'play catch-up ' .
These communication delays can impact other areas
such as social development as well as academics.
Communication milestones are reached at different
ages by different children because development is
influenced by many factors. Things like innate
ability to learn, amount of exposure to language and
manner in which people respond to the child's
attempts at communication all impact this development. It can often be very difficult to identify
whether a child is within a typical range of development or actually delayed. Parents need to know
what to do and when to do it.
Speech . . . . . . . . . . . .. .
By age 3 years, if your child cannot:
• be understood by family and/or caregivers.
• correctly produce vowels and such sounds as
p, b, m, w in words.
• repeat when not understood without
becoming frustrated.
By age 4 years cannot:
• be understood by individuals with whom
they do not associate regularly.
By 12 months does not:
• respond to music and/or singing.
• understand simple questions.

By 4 years does not:
• understands comparisons.
• make inferences.
What can we do
By age 5 years cannot:
as parents?
• be understood in all situations by most
listeners.
If you are concerned about the growth of your
child's speech andlor language, you should consult
with a Speech-Language Pathologist (SLP) for a
professional evaluation. SLP's are specially trained
individuals that assess communication development
in various ways. An SLP can conduct evaluations in
areas such as expressive/receptive and social
language, articulation, fluency, andlor processing.
The following are some helpful guidelines that
should prompt a speech andlor language referral:
By 5 years does not:
• understand complex directions.
• correctly produce most speech sounds.
• understand concepts.
• be asked to repeat without exhibiting
frustration .

Many more women are using massage during labor
to relieve tension and promote relaxation. If a
woman is relaxed during childbirth then she can
fight off tbe pain cycle, making it possible to bave a
minimal or pain-free birth experience. Also, the
more a woman is able to relax during her labor, the
quicker ber labor proceeds. Tbere is a direct connection between relaxing the skeletal muscles and
dilation of tbe cervix. Tiffany offers Labor Massage

What is a Daily Money Manager?
I By Sheri Samotin - LifeBridge Solutions, LLC
From time to time in this space, Sheri will answer readers' questions.
lfyou have a question, please e-mail [email protected].
Dear Sheri:
My husband recently passed away and he
always handled our household bills and budget.
I don't even know where to begin to get everything in order. Is there someone who can help
me with this?
Intimidated Irene
Dear Irene:
First off, please accept my condolences to you on
your loss. It may help to know that your situation
is not at all unusual. Often, one spouse handles
the family finances. This works out well from the
standpoint of division of labor, but leaves the
other spouse at a real disadvantage when the situation changes, especially if it is a sudden and unexpected change. As a new widow, I'm sure you
have a lot on your mind, not to mention the piles
of paperwork you're probably handling to settle
your late husband 's estate.
F
ortunately, there are professionals
who specialize in helping people in
your situation. They are called Daily
Monday Managers, or DMMs .
DMMs offer a range of services, including
helping you to manage your mail and set up a system
for paying your bills, assisting with check writing and
maintaining bank accounts, helping you create a realistic budget and stick to it, and developing a tax planning
and organization system. In addition, many DMMs
have expertise in medical billing advocacy and can
make sure you are paying only those medical bills that
have been correctly billed and processed. A DMM can
also help you with the many tasks that are necessary to
settle an estate, including obtaining various records,
closing accounts, and gathering items needed by attorneys and accountants.
One of the most important services a daily money
manager offers is helping you consolidate all of your
important information so that if something happens to
you, the person you've entrusted to act on your behalf
will be able to take action. For example, if you pay your
bills on line, have you documented your login and
password information and does the person who holds
your Power of Attorney know where to find that information? Are all of your insurance policies stored in two
places, at least one of which is not in your residence?
Have you made a list of all of your accounts (bank, brokerage, retirement, credit cards, etc.) with contact information and have you given each institution written
permission to discuss that account with the person
you'd expect to handle these matters for you if you were
incapacitated? If not, a DMM can help you pull this an
together into one physical or electronic repository.
MASSAGE THERAPY
& Skin Care
Be sure to choose a daily money manager who is
bonded and insured. Also, be sure the person
you choose specializes in working with older
adults and their families and is not simply a
bookkeeper for businesses. Many DMMs are
members of a national organization called the
American Association of Daily Money
Managers. You can learn more about this profession, as well as search for a DMM to help you, by
visiting their website at www.aadmm.com.

veryone wants to look and feel their
best. Often standing in the way is
finding a dentist with whom one can
feel comfortable and who provides quality care and
attention. Dr. Alan N. Rembos IS one of those dentists.
"We provide traditional care as well as functional
and cosmetic dentistry in the most relaxing environment we can," he explains. "Cosmetic dentistry consultations are referred to as smile design, or smile
improvement. We can change the color and shape of
the teeth while restoring sound functional and oral
health condition designed to last a lifetime."
"Some people don't like the color of their teeth, or
they come in with rotated, chipped, or worn teeth,
asking what can he done to improve their smile. Every
patient is provided a comprehensive examination and
a treatment plan best suited to their individual needs.
The follow-up consult then reviews treatment options,
timeframes, and costs of desired treatments."
Whatever the decisions made between Dr.
Rembos and his patients, comfort and quality
are always a priority.

Better Smiles
Advances in cosmetic dentistry have led to the
development of several techniques for brightening
smiles, and in-office whitening is one of the easiest,
notes Dr. Rembos.

"There's no way to predict who will respond most
favorably to whitening but we consider all options
to achieve the best results for each patients."
Dr. Rembos has found that the most popular inoffice system is the ADVANCED POWER ZOOM
2 treatment. "This Advanced Power Zoom system is
the latest version, with even more dramatic results
than the original Zoom systems," says Dr. Rembos.
"We are constantly upgrading to the very latest techniques available so our patients can always choose
from among the best options."

"While the patient rests comfortably with headphones and a pillow headrest, the whitening gel is
applied, the light is activated, and the procedure
begins," he describes. The entire process lasts
about an hour from start to finish.
"The solution doesn't damage the tooth enamel ,"
assures Dr. Rembos, "but really gets into those tiny
spaces and cleans out the discolorations that have
accumulated over the years."
Dr. Rembos explains that although many patients
feel their results are dramatic, individual results
will vary, as programs are tailored for each
patient's individual needs. Some patients' teeth
darken with time from simple staining, whereas
others have additional discolorations as a result of
illness or the use of certain medications. Stains
respond more easily to whitening than do medically induced discolorations.
Northwestern University Graduate
Many patients are pleased to discover that they can
get their old silver fillings replaced with a newer
material made from either resin or porcelain composite, a significant improvement and often a
prelude to whitening treatment.

Veneers are a more advanced cosmetic option and
may be appropriate for a variety of dental conditions seen in front teeth, particularly gaps between
teeth, stained fillings, and pennanently discolored,
chipped, or crooked teeth.
Dr. Rembos points out that a patient does not have
to have every tooth treated with a veneer or crown
to achieve a good cosmetic result.
"For example, patients who want to brighten their
smiles may have several veneers placed on their
front teeth and then have the rest of their teeth
whitened to match the vibrancy of their new
veneers. When appropriate, laser technology is
often used to achieve the most appealing final
cosmetic result."
With more than twenty years of experience and a
Fellowship in Implant Dentistry, Dr. Rembos is
aware of every technique available to his patients.
One important development over the years has
been the widespread availability of dental implants
for patients with one or more missing teeth.
"After the procedure is complete, most patients
comment about how simple and comfortable the
entire process was," adds the doctor.
Convenience + comfort
Many dental practices are so overburdened that
when calling for an appointment, most new
patients or those with dental emergencies can't be
seen for several weeks. Dr. Rembos makes sure
that doesn 't happen in his practice.
"We pride ourselves on accommodating emergencies as well as our regular patients for quality
dental care," he emphasizes, "and new patients can

In addition to a wide range of cosmetic, restorative,
and general dental services, Dr. Rembos offers
patients a state-of-the-art way to feel relaxed and
comfortable during their dental procedures.
" We exclusively use a computerized anesthetic
delivery system called the WAND," says Dr.
Rembos. "Our patients love it, and we offer it for
all our procedures at no additional cost to the
patient. The system senses the pressure in the
tissue to allow for more comfortable injections,
and provides comfort both during and after treatment. Consequently, patients don't experience the
tenderness and soreness associated with conventional methods." To ensure a relaxed environment,
additional comforts such as stereo headsets and
head pillows are made available to every patient.
"We always use an intraoral wireless camera to
show patients what's going on in their mouth on a
large screen. Our panoramic x-ray machines take a
picture of the entire mouth and jawbone in twelve
second without having to place multiple films in
the mouth. Our patients love these features and
conveniences. By having available such equipment, patients no longer have to fear gagging on
multiple films to obtain a full mouth view.
"Our patients often remark that dentistry has come
a long way - that it's not the way it used to be,"
reports Dr. Rembos. "That's when we know we 're
doing things right."

Neurological and Vestibular Rehab
Dizziness and Balance Center of Southwest Florida
I By Sheila LoBianco, MSPT
Are You Headed For A Fall? Dizziness and balance problems can lead to falls, which are the
number one cause of death from injury among those 65 and older. Fortunately, there is help.
ccording
to
the American
of
Orthopedic
Academy
Surgeons, more than one-third of
the population over the age of sixty-five experience
falls each year, and many patients who suffer serious
fall-related injuries die within two to five years.
"Fifty percent of all falls are due to vestibular
involvement," observes Sheila LoBianco, MSPT,
Clinic Director of Dizziness and Balance Center of
Southwest Florida, "and eighty-five percent of all
dizziness problems are related to the vestibular
system. "
The vestibular system is the part of the inner ear
that helps control balance.
"Determining the type and degree of vestibular
dysfunction is critical in developing a treatment
program that will decrease a patient 's fall risk,"
points out Sheila, "yet recent statistics show that
patients with vestibular disorders will see an
average of six doctors before receiving a diagnosis.
"However, those averages can be overcome."
Jack Nuber says his balance had been deteriorating: "I tried to get help. I don't even remember how
many doctors I saw ... but no one could help me .
.. " Then Jack was treated at Dizziness and Balance
Center of Southwest Florida. He says, "I wish I'd
discovered this five years ago." He is now back on
the tennis court.
The number one indicator for fall risk is a previous
fall. Other indications include symptoms of dizziness, decreased activity level, taking one or more
medications, decreased sensation in the legs or
feet, and fear offalling. "Falls are typically not the
result of a single cause or risk factor," educates
Sheila. "More often than not, they are the consequence of a combination of factors. Many gait and
balance disorders are the result of neurological disorders such as stroke, ataxia, foot drop,
Parkinson's disease, Multiple Sclerosis, and
others."
Benign Paroxysmal Positional Vertigo (BPPV) is a
disorder that causes vertigo, dizziness, and other
symptoms precipitated by a change in head
position. Getting out of bed and rolling over in bed
are two common "problem" motions. About fifty
percent of dizziness in people over age sixty-five is
due to BPPV
Frances Houghton, who suffered with BPPV for
years until she consulted with Dizziness and
Balance Center, says, "In one treatment my vertigo
was gone. It's miraculous ... " Not all balance
problems can be solved in just one treatment,"
acknowledges Sheila. However Dizziness and
Balance Center of Southwest Florida specializes in
helping patients regain their balance and offers a
comprehensive evaluation, individualized treatment, and intensive patient and family education.
"That is what makes our program successful, while

Aging B~!:~cefully
I
t usually starts in the mirror either some
morning or a time of day when we see ourselves in unflattering light. What happened
to our hair? My clothes don't fit. Why does my
skin look like crinkled aluminum foil? What
happened to the young body that looks like the
way I feel?
Read a few magazines or watch TV for a while and
it's apparent that we are a society that values youth
and also seeks happiness. Young is good, old is
bad. Being young and having things make us
happy, being old and having less makes us sad.
Part of the reason that midlife can be so difficult is
that it brings up our fears about aging. We fear that
life is almost over and we still have not yet
attained our long held goals. It would seem inevitable that as our muscles weaken, our hearing and
vision fade , and as our skin gets wrinkled things
would really get depressing. Bring on the antidepressants and the meds!
Yes, it does seem dark and dismal, but a recent
Gallup poll has found that by almost all measures
people get happier as they get older. How can this
be? And it doesn't always look so good for those
young folks. Another study found that enjoyment
and happiness decrease once we pass our "invincible years" gradually until we hit 50, rise steadily
for the next 25 years, and then decline very slightly
Loren said, "There is a fountain of youth: it is your
mind, your talents, the creativity you bring to your
life and the lives of people you love. When you
learn to tap this source, you will truly have
defeated age."
Often times our minds go where our bodies are
unable to follow. Physical conditioning and
strength is not about youth and beauty, it is about
having your body being the friend and vehicle it
was meant to be.
at the end. The decline can usually be associated
with health issues, death of loved ones, and the
typical problems of being a senior citizen.
A commitment to your body through exercise and
diet is as essential as financial planning. While good
fortune and genetics can playa big part in anyone 's
physical condition, more often our health is a result
of the hard work we commit to ourselves so that we
may be there with those we love and do the things
that we love. Eleanor Roosevelt said, "Beautiful
young people are accidents of nature, but beautiful
old people are works of art."
Researchers have also found that with age, many of
us learn to live more comfortably with ourselves,
and accept our human imperfections and frailties.
We stop comparing ourselves to others, and begin
to appreciate what we have.
So since we can't avoid aging except by dying and
since it appears that aging is not something to be
feared, shouldn't we be focusing on how to age
"gracefully"?
Get out and get moving!
What is aging gracefully? Is it being a silver
haired person, impeccably dressed, and driving
down the highway in their convertible? Wouldn 't
it be better to be someone who feels good in their
skin, is able to be active and do the activities they
love without pain, and can live thinking that their
body is not their enemy but rather a part of their
life that they listen to and understand? As we can
see from these recent studies,

boarding flights? No more taking water, perfume,
cologne, shampoo, conditioners, mouthwash, toothpaste, or any other objects that may be considered
dangerous; like tweezers, shaving razors, nail
clippers or anything else someone could possibly
use to hijack or bring down the aircraft.
Snacks and beverages used to be complimentary,
but now there are usually standard charges for any
food or drink. There is now a charge for using
headsets to watch & listen to in-flight movies or
entertainment. [fyou would like to use a pillow or
blanket during your flight; good luck, most airlines
have discontinued them altogether. Most airlines
also are charging for any extra baggage, especially
if the items exceed a certain weight limit.
Coping with all of these extraneous factors can be
very stressful, especially if travelers are dealing
with any other mental or physical handicaps. One
disability, hearing loss, can prove to be extremely
difficult and stressful when traveling, creating
anxiety while trying to listen to instructions, or
flight information while traveling. Arrival times,
flight numbers, concourse information, gate
numbers, aisle and seat numbers, and departing
times are crucial to successful and timely travel.
Hearing loss can make a person feel disoriented
and anxious in some situations, especially when
obtaining crucial information while traveling.
Misunderstanding of vital data may prove not only
to be very disheartening but costly as well, especially if an individual misses his or her flight.
Obtaining another flight to the originally planned
destination may not even exist.

that do not wear hearing devices. Other benefits
noted by hearing instrument wearers, is they are
more likely to participate in social activities,
concerts, theaters, and groups. Users also have
fewer feelings of isolation and depression, frequently associated with hearing impaired individuals. Also, most users note improved relations in
their marriage, with friends and relatives, their
mental health, sense of safety, relations at work,
sex life and life-overall , since communication is a
key factor.
The use of hearing devices has many benefits
tangible, and intangible. The newest digital technology has forever changed the way hearing
impaired individuals can not only hear, but understand speech now and into the future. Don't get
left behind!

Doctors Talk About VAX-D
"I've never found anything to work
so well for back pain patients in my 39
years in medicine. VAX-D changes people 's lives
and frees them from the misery of chronic low back
conditions; it repairs damaged discs that cause
excruciating back pain and sciatica. We have been
able to fix the post-surgical train wrecks that no one
else could repair. VAX-D should be used before
surgery. VAX-D has lowered patients' needs for
pain medicines-and that is a good thing." E.
Michael Gutman, M.D. -Orlando, FL.
Physician, Pain Specialist, Forensic Neuropsychiatrist,
Back Pain Institute of Orlando
•
When I first saw the VAX-D system
a few years ago, I had to find out if it
worked because what the manufacturers were
claiming was revolutionary and exciting. If it were
true, r could help my patients without injections or
surgery. What a dream come true! r looked at the
medical studies performed on VAX-D, as well as
the patients. r spoke with physicians who used
VAX-D on their patients. I looked at the competition who claimed they were better than and
improved over VAX-D, which cannot be since
VAX-D is patented. The others may look very
impressive, even sci-fi! Unfortunately, they are
missing the patented science that is behind VAX-D.
r became excited about VAX-D as a result of my
in-depth study of the product. However, nothing
compared to discovering for myself through using
VAX-D in my own practice, that it works and can
be the miracle cure that people are looking for. " A.

VAX-D adds a new dimension to my
practice by allowing me to noninvasively treat patient neck and back injuries that
otherwise might require surgery. It also addresses
the disc areas of neck and back pain that I cannot
reach with prolatherapy injections. Further, combining VAX-D with other therapies, creates a synergistic approach that increases our patients'
chances of complete recovery from pain and return
to function." Daniel Royal, D.O.-Henderson, NV
Physician, New Hope Medical Centers
Patients have traveled to our clinic
from allover the United States to be
treated with VAX-D. My partner and r had a very
successful multidisciplinary medical practice in
Arizona. In 1994, after reading about VAX-D in a
trade journal, we decided to travel to a clinic in California to see this "Medical Breakthrough" with our
own eyes. We sat in the reception area for an hour
and listened to the patients rave about the "magical"
table. After a brief visit with the doctor, we flew
home and bought the first VAX-D table in Arizona.
Almost immediately, we too started to see seemingly miraculous results on the majority of our
toughest cases. That was thirteen years ago, and I
amjust as excited about VAX-D now as r was then."
Thomas E. Blankenbaker, D.C. - Phoenix, A2
Chiropractic Physician, Spine & Disc Medical Center
"I have treated literally hundreds of
patients over the years from ages 17 to 84 with
this revolutionary, non-invasive, non-surgical and
non-drug treatment. The addition of a VAX-D clinic
to my practice in 1999 clinically has been a most
impressive and rewarding experience. I am thrilled
to offer a treatment that has led to a reduction in use
of many medications and an increase in quality of
life." Gerald B. Weiss, M.D.-Norwalk, CT

THAT MAY CAUSE YOU STRESS!
I
By Myles Rubin Samotin, MD - Board Certified Orthopaedic Surgeon, Fellowship Trained in Foot and Ankle
Y
may have heard the term stress
fracture, but just what is it and how
does one get it? A stress fracture is
exactly what it states it is, a fracture of the bone
caused by "stress" to the bone. It is a "microfracture" of the bone, similar to a crack in the concrete
of your driveway. It occurs due to several causes, but
most of the time due to the muscles that support the
body become overly fatigued and the stress transfers
to the bone, sometimes resulting in a fracture.
OU
The people who get this fracture are people who
either do repetitive exercises, (people who constantly are running, or doing consistent exercises
that overexert the muscles) or people who have
problems with bone quality or density, due to
medical problems, poor nutrition, or osteoporosis.
The muscles are not strong enough and the bone
fractures. As you can imagine, these stress fractures
rarely involve upper extremities, since we don't put
pressure on these bones on a consistent basis. These
fractures almost always involve the tibia (shinbone)
or one of the many bones of the foot, many times
involving a metatarsal bone.
Women are much more likely to suffer these, since
estrogen, which helps support bones, tends to
decrease as women age. Long distance runners, triathletes are very susceptible to these fractures, BUT,
many "snowbirds" will suffer from these when they
first come down to Florida, since they will start a
strong exercise program without properly building
up the supportive muscles.
The symptoms of stress fractures are relatively
simple: PAIN. This pain will increase as you
continue putting pressure and stress on the affected
bone, and will subside when the bone can rest.
The first thing in proper diagnosis is to a see the
proper specialist who can give a correct diagnosis.
Since the bones associated with stress fractures
involve mainly the foot and ankle, then a foot and
ankle specialist, with orthopedic experience is the
proper medical person to see. He will order the
proper tests, to diagnose the stress fracture. Most
normal X-Rays will NOT show any fracture, and a
person may be misdiagnosed because of this.
Special tests such as bone scans and MRI's can
make the proper diagnosis.
When you see your orthopedic foot and ankle specialist, he will order the proper tests and will make the
diagnosis of a stress fracture. He will rest your leg by
placing it into a walking boot to give it the chance to
heal. The most important thing to remember during
this time of rest is that you MUST give it enough time
for the fracture to heal. Several weeks of rest may be
necessary. If you start to use the bone too early, the
fracture may not have totally healed and another
stress fracture may result. Therefore, care must be
given to properly let the stress fracture mend. All
patients must remember that the BONE IS FRACTURED. Give it time to heal.
Anytime that a lower extremity has been consistently
painful over a period of time, the diagnosis of stress
fracture cannot be ruled out. You should seek your
orthopedic foot and ankle specialist. I am the only fellowship trained orthopaedic foot and ankle specialist,
with clinical experience of over 12 years in the
Naples area. I believe my combination of extensive
education and clinical experience along with willingness to use new techniques in a state-of-the-art office
allows me to give the best possible care to ALL your
foot and ankle needs. I know that I can give you the
orthopaedic foot and ankle distinction.

The Grand Buffet Belief System
ur modem day culture has progressed into a society that no
longer adheres to absolute truth,
Whatever sounds good to me is true for me and
whatever sounds good to you is true for you, When
we go out to eat at a buffet, we like to get a little bit
of this and a little bit of that to make one huge plate
full of everything that looks good to eat With our
"have it my way" mentality, we often times form
our belief system in the same way we choose our
food, No longer is it good enough to believe God's
Word for what it says, instead, we believe bits and
pieces of God's Word and we like to mix a little bit
of psychology, a little bit of sociology, and throw in
some new age philosophies and then you get a
grand buffet helief system that is based on nothing
substantial. God's Word says, "See to it that no one
takes you captive through hollow and deceptive
philosophy, which depends on human tradition and
the basic principles of this world rather than on
Christ" (Colossians 2:8)
Our culture and modem philosophies of the day are
like smog, We really don 't recognize we are breathing it in until we step out of it and look back, Our
culture runs rampant with increasing influence
from new age philosophies, psychics, and all sorts
of religions, Something must change because we
are so open to everything that we don't stand for
anything! What do you base your beliefs on? How
do you know what is true? Is it more logical to base
your beliefs on man 's ideas and popular philosophies, or God's Word that has been tested and tried
over the centuries? How can we get to a place
where we make good decisions based on real truth?
We need to start with who and what we allow to
influence our thinking and behavior.
Who Are We Listening Too?
The lens that we see and live life through will determine the life you and I will live! Jesus tells us, "if
you hold to my teaching, you are really my disciples. Then you will know the truth, and the truth
will set you free, " (John 8:31-32) Those of us who
claim to believe in Christ, are we really following

Can't We Mix Worldly Philosophies With God's Truths?
Why would we want to add anything to God's clear
truths found in the Bihle? That is like saying here
is a fresh, cold glass of water, let me add some
sewer sludge to it, and than it will be ready to
drink. That is absurd because we traded fresh water
for disease infested sewer waste, This leads to
physical sickness, as mixing worldly philosophies
to God's perfect word leads to spiritual confusion
and ultimately disease infested sin, That is why
God's word says, "the foolishness of God is wiser
than man 's wisdom, and the weakness of God is
stronger than man's strength," (I Corinthians 1:25)
We must recognize our knowledge is finite,
fallible, and weak while God's knowledge is perfectly accurate, powerful, and unchanging, God's
Word stands alone as the only source that changes
men's hearts, everything else merely cleans up the
surface, The philosophies of the day will come and
go, but God's Word and His truths will stand
forever because they are eternal. God's Word tells
us that, "all men are like grass, and all their glory is
like the flowers of the field; the grass withers and
the flowers fall , hut the Word of the Lord stands
forever," (I Peter 1:25)

Him? Christ tells us that we are set free when we
follow His teachings, not mentally just believe He
is the Son of God. If we say we follow Christ than
we must he guided hy God's Word. That means we
must study God's Word to know how God wants us
to live. We can 't say I am following Christ, and not
know what God 's Word says. The Bible tells us,
"we know that we have come to know Him (Jesus)
if we obey His commands. The man who says, I
know Him (Jesus), but does not do what He
commands is a liar, and the truth is not in him, But
if anyone obeys His Word, God's love is truly made
complete in him." (1 John 1:3-5) When God's Word
guides our life than we grow in discernment and
recognize the worldly teachings bombarding us.
Who are we listening too, God or someone else?
Are we getting our beliefs from God 's Word or
worldly philosophies like Oprah, Dr. Phil, and other
psychologies of our day?
