o many of your
patients complain
of itchiness and
a scratchy feeling
in their eyes? How about
redness,
irritation
and
watering? If so, you’re not
alone. Optometrists report
that the majority of people
who visit their offices report
these symptoms. Fortunately,
new research into dry
eye syndrome (DES) now
offers practitioners important
information that can help
to alleviate the condition.
Persistent dryness, scratchiness, red eyes and a burning sensation are common symptoms of
dry eyes. Another symptom is a “foreign body sensation” - the feeling that something is in
your eye.
DES can also cause watery eyes because dryness on the eye’s surface sometimes
over-stimulates production of the watery component of the tears as a protective mechanism.
DEFINING DRY EYE
It has been a challenge for international groups of eyecare specialists to adequately define
DES. A 2007 report published by the Dry Eye WorkShop (DEWS), an international team of
academics and industry representatives, found that a 1995 definition of dry eye was wholly
inadequate when it said the disease is, “a disorder of the tear film due to tear deficiency or
excessive tear evaporation which causes damage to the interpalpebral ocular surface
associated with symptoms of ocular discomfort.” The report noted the lack of international
consensus on the condition’s symptoms and said there is no single diagnostic test for dry eye,
nor is there an internationally accepted treatment protocol 1.

“
With evaporative dry eye, the tear film
evaporates too quickly. The most common
cause of evaporative dry eye – meibomian
gland dysfunction (MGD) – is responsible
for over 60 per cent of DES cases. With
MGD, the oil glands that line the upper
eyelid and secrete lipids to the eye become
clogged and inefficient, reducing oil output
and leading to excessive evaporation of tears.
If left unmanaged, MGD can cause or
exacerbate dry eye symptoms and eyelid
inflammation. The oil glands become
blocked with thickened secretions.
Chronically clogged glands eventually
become unable to secrete oil, which results
in permanent changes in the tear film and
dry eyes.

moisture-secreting glands of the eyes and
mouth are usually affected first, resulting in
decreased production of tears and saliva.
Although Sjogren’s Syndrome can develop
at any age, most people are over 40 at the
time of diagnosis. Treatment focuses on
relieving symptoms.
Sjogren’s syndrome is very under-diagnosed,
Caffery says. “The rule of thumb is that one
per cent of the North American population
is affected by it. When a post-menopausal
senior complains of dry eyes, asking
whether her mouth is also dry could lead to
a diagnosis.”
SJOGREN’S SYNDROME
CAUSES OF DRY EYE
Dry eyes can be symptomatic of Sjogren’s
(SHOW-grins) syndrome, an auto-immune
disease in which the white blood cells attack
the moisture-producing glands. Sjogren’s
syndrome is identified by its two most
common symptoms – dry eyes and a
dry mouth. Ninety per cent of Sjogrens’
sufferers are women.
• Aging
DES affects women far more frequently
than men, possibly due to hormonal
fluctuations. A recent study also indicates
that the risk of dry eyes among men
increases with age.

For anyone who spends a lot of time looking
at computer screens, blinking is important
for re-spreading the tear film. This doesn’t

“I don’t think any other device is so clinically
friendly,” says Caffery. “The key is the
thoroughness as well as the frequency of
blinking. Blinking is involuntary – many
people don’t completely close their eyelids so
their eye surfaces are not completely covered
and wetted. With a full blink, the lids meet,
which helps with oil secretion.”

Dry eye occurs as a side effect of many
medications, such as antihistamines,
antidepressants, certain blood pressure
medicines, Parkinson’s medications and
birth control pills.

wearers.

PREVALENCE
Dry eye is significantly under-diagnosed,
says Caffery. “We’re so busy making sure
there are no serious eye diseases that we
sometimes forget about problems that are
symptomatic. ECPs should always ask if the
eyes are uncomfortable or dry, especially
among those with risk factors, including age,
gender and computer screen use. Opticians
should also ask about dry mouth, especially
in older women. If that’s an issue, the ECP
can perform a great service by screening for
dry eye and Sjogrens.”
Recent research indicates that contact lens
wear and dry eyes can be a vicious cycle. DES
makes contact lenses feel uncomfortable
and evaporation of moisture from contact
lenses worsens dry eye symptoms. Newer
contact lens materials and lens care products
can help reduce contact lens dryness. (See
sidebar)
DIAGNOSING DRY EYE
Dry eyes can be diagnosed through a
comprehensive eye examination. Testing,
with special emphasis on evaluating the
quantity and quality of tears produced by
the eyes, may include the patient’s history,
external examination of the eyes, evaluation
of the eyelids and cornea, and measurement
of the quantity and quality of tears for any
abnormalities. Using this information the
ECP can determine if the patient has DES
and advise on treatment options.
TREATING DRY EYES
DES is a chronic and typically progressive
condition that may not be completely
curable (depending on the cause). However,
the symptoms can be managed. Because
there are many different causes, a variety of
treatments are used.

Optometry has a role to play in educating
patients about proper lid hygiene, says
Caffery. Regular warm compresses that melt
the waxy portion of the oil in the meibomian
glands and clean lid margins are especially
helpful to those prone to MGD. It is very
important not to apply any pressure to the
eye while performing this procedure.
TheraLid, an eyelid cleanser that contains
a small amount of tea tree oil, can be very
helpful for this purpose, particularly if
Demodex follicularim is involved at the lid
margins. Over-the-counter (OTC) products
that sterilize the lids can also help.
For a hot compress, fill the sink with hot
water and use a cloth to gently apply it to the
lid margins while the eyes are shut. There
are also devices that can be warmed in the
microwave and used lying down, says
Caffery, adding, “I encourage patients to go
to bed with them on.”

recent University of Waterloo study on the effects of makeup deposits on contact lenses showed
that some makeup removers and waterproof mascaras changed the shape, optical performance
and wetting ability of some silicone hydrogel lenses1,2. Further research is needed to fully

hen it comes to dryness
and contact lens wear,
there are many studies
and few clear results,
says Dr. Barbara Caffery. “I have some
severe dry eye patients who can only
function with contact lenses and mild
dry eye patients who can’t tolerate
them.”
Caffery says. “The Leis Vision lens
and the Proclear material have proven
track records. Some silicone hydrogels
like Acuvue® Oasys and Dailies Total
One have shown clinically better
performance in dry eye. But you
have to try it and see what happens.
Many visits and lots of patience are
required.”
The sensation of dryness may not be
actual dryness, she adds. “Something
like blurred vision can make contact
lens patients aware of their eyes in a
way that makes them seem dry. As
clinicians, we must pay attention to
factors beyond lens type, including
lens condition, solutions used, whether
the patient cleans their lenses after
removal, how the lenses fit, and even
whether they see well or not.”

Those who oppose UPP, including Costco,

directly sell a particular manufacturer’s

patients are more likely to buy from them
if the purchase price is the same. They also
claim that UPP will result in higher prices
for consumers and that practitioners will
limit lens choices for their patients because
of potential revenue bias.
ECPs are part of the multi-billion-dollar
contact lens industry. As we watch the
UPP drama unfold in the U.S., we must be
mindful of who, in the end, will exercise
market power. Will it be manufacturers,
who have rising flexibility to influence
future price increases of their products
downstream, which in turn can impede the
buying/selling of premium contact lenses
and reduce the patient benefits associated
with their purchase? Or will it be the
online discount retailers who can continue
discounting products, with unknown
economic consequences, such as decreased
quality or variety of contact lens products
developed and sold?
Eyecare practitioners need to communicate
the value of contact lens fittings, eye exams
and progress checks to their clients. They
need to inform the public and lawmakers
that contact lenses are medical devices
designed to correct vision. They need to
emphasize the importance of supporting
their right to prescribe contact lenses,
based on a patient’s visual needs, including
lifestyle considerations, for optimum
ocular health and safety. They also need
to educate contact lenses wearers about
potential risks and complications that
require regular follow-up care.

of the purchasing and decision-making
equations for both patient and practitioner.
The value of the contact lenses is increased,
accompanied by a continued investment
in service levels and in the contact lens
products being sold. In this way, the
negative perception that, “it’s too expensive
to purchase from my eye doctor,” is also
eliminated and the focus can be entirely
directed to patient care, visual need, best
lens choice and safety.

FULFILLS HIS DESTINY IN
EYEWEAR DESIGN
Working as a sculptor in wood and stone would seem to be an almost
ideal background for creating frames in acetates and metals. But little
did the teenage Frederic Beausoleil guess where his destiny lay.
“I
always wanted to be an artist,” says Beausoleil, president
of Frederic Beausoleil haute lunetterie. “And sculpture
was my first love in my teen years. I wanted to attend art
school but my parents felt I should have a career at which
I could earn a living first.”
Beausoleil’s mother was the first female optician in
France and a successful retailer of high-end designer frames
from her two stores in Nantes. She suggested that he train as
an optician. Frederic had worked in her stores selling frames
during the summers. “I remember perfectly the beautiful frames
available during those years,” he says. “The subtle sophistication
of frame design was not lost on me and I enjoyed handling the
materials.”

It was young love that took Beausoleil
as a newly minted optician to Paris, where
he was soon to embark on his career.
“I decided to move to Paris with my
girlfriend and wanted to create a pair of
optical frames for her as a gift, so I did the
drawings and was looking for someone to
manufacture them when I came upon a
small workshop in Le Marais. The 72-year
old owner had been making tortoiseshell
and buffalo horn frames by hand since
1946. It was there that I caught the magic
and first envisioned a career as a frame
maker. He took me on as an apprentice for
six months and then sold me the workshop.
I well remember my first day on my own in
the workshop: I knew how to craft frames
but I didn’t yet know how to design them.
I spent 18 months teaching myself. Then, I
was ready for my first exhibition.”
Silmo 1989 was the stage for Beausoleil’s
introduction to the optical industry. To
say that his first collection of 26 models
was a success is an understatement. “I was
approached by leading optical retailers
and designers from around the world,” he
explains. Robert Marc (NYC), Josh
Josephson (Toronto), Barbara McReynolds
and Gai Gherardi (l.a. Eyeworks), and
Oliver Peoples founder Larry Leight were
among Beausoleil’s first fans. His order book
was overflowing but he couldn’t produce
all the frames needed in his tiny workshop.
“I sought out a manufacturer but I was
new in the business and no one trusted
that I would pay them. So I built my own
factory,” he explains.
Now one of only nine eyewear factories
left in France (down from 150 when
he started in the business) Beausoleil’s
Nantes-based factory employs 26 people
and four robots. Here they produce the
acetate collections – Sellier, Bague, Galet,
Bi-Chomatique and Stradivarius (part metal
with hinge production in Japan) – with
exclusive Beausoleil-designed Mazzucchelli
material. Every Beausoleil piece is
hand-assembled and hand-polished, with
robots doing the intial cutting and sharp
sculpting. Each frame is slightly different,
due to the hand finishing and Beausoleil
knows his clientele appreciate the attention
to detail that is integral to his design
aesthetic.
One of the benefits of building his own
factory is that Beausoleil was able to set it
up to manufacture one of his innovations:

of André-Charles Boulle, a French
cabinetmaker who originated the technique
of inlaying tortoiseshell and brass in
furniture in the 17th century. Other
innovations include frames made from 100
per cent linen, an ergonomic bridge for
metal frames, and original hinge designs.

hearing aids embedded in the
temples. He was asked to design
them by Siemens, the world’s
largest hearing aid manufacturer.
“These are perfect for men who
don’t usually have hair that’s long
enough to hide hearing aids,” he
explains. Beausoleil is a partner
in a new type of retail store that
sell both regular and hearing-aid
equipped frames, along with
traditional hearing aids. Acuitis
is the name of the new company
and he is very excited by its
growth and potential.

According to the Canadian Association of Optometrists, dry eye, or the inability to produce
sufficient tears for lubrication and nourishment, affects as much as 30 per cent of the
population – or a significant percentage of the clients visiting optometry practices and optical
shops. As Watson notes, the condition is particularly troubling for those who wear contact
lenses, as the lack of tears or poor tear quality can lead to the accumulation of debris, causing
eye irritation.
One of the primary culprits in the prevalence of dry eye is increased computer use.
In addition to emphasizing the role opticians, like all eyecare professionals, can play in
“re-teaching customers to blink more often and lubricate their eyes” while working on
computers, Watson also highlights the important role a quality pair of spectacle lenses
designed for computer users (so-called computer progressives or office lenses) can play in
reducing eyestrain. Typically, these lenses are designed with enhanced near and intermediate
segments to provide optimal vision for wearers working on computers in an office setting;
while they do not address dry eye specifically, they can increase visual comfort during the
performance of computer-related tasks.

Another spectacle product with potential dry eye-related
benefits is anti-reflective (A-R) coating. According to
Ron Dorey, optician and owner of Cornwall Optical
in Regina, glare, halos and reflected light can add to
eyestrain and exacerbate dry eye. He has also found
success with coatings designed to filter low-wavelength
visible blue light (from computer screens, for example),
which has been shown to be particularly problematic for
those with dry eye.
“In outdoor situations, a good pair of sunglasses with a
wrap not only prevents an overabundance of sunlight
and UV light from entering the eyes, but also keeps
wind, dust and other foreign debris away from the eyes,”
he adds. “These are all contributing factors to dry eye.”
Indeed, Alan R. Boyco, OD, owner of Image Optometry,
a chain of optometric practices in B.C., says wrap lenses
have historically been referred to as “moisture retention
glasses” because the shorter vertex distance affords
enhanced eye protection from debris and other
environmental factors linked to dry eye. Like Dorey,
Dr. Boyco is also a big believer in specialized,
light-filtering coatings.

“The theory [of these products] is that an ophthalmic
lens that cuts out UV and blue light can, over time,
cumulatively reduce the likelihood of sun damage and
the potential for the development of pingueculum and
pterygium, both of which have a strong correlation
with dry eye due to the disruption of the eye’s natural
tear film,” he explains. Pingueculum is a thickening of
the corneal sclera while pterygium (or “surfer’s eye”) is
a wedge-shaped growth on the sclera that impacts the
cornea.

Dr. Boyco also stresses the importance of patient
education in managing dry eye – and all eyecare
professionals can play a role here as well. He notes that
there are certain medications (including beta-blockers
and antihistamines), for example, that have been linked

With three Eye Health Centres in Saskatchewan
and Alberta, Dr. Diana Monea can’t be
everywhere at once. It helps that her 36
employees, including six optometrists, are
empowered to make decisions on their own.
“I see empowerment as giving people the
necessary skills, experience and guidance to
show leadership,” she says. “We have office
policies but I consider them guidelines.”
Decisions should depend on the circumstances
of the particular situation and Monea’s
employees can make those decisions without
having to consult her first. If, for instance, a
customer purchased a pair of children’s
sunglasses for $20 and they break, her employees
would replace them without having to get
permission first. That’s important because the
alternative might lead to the loss of the patient.

says Sacco. “As employers we have to
be budget- and profit-conscious so it
makes sense to get the most from the
people who work for us. The more we

Canada are poised to
receive significant tax
relief as a result of the
recent federal budget.
On April 21, Federal Finance
Minister Joe Oliver announced
a proposal to cut the smallbusiness tax rate by 0.5 per
cent in each of the next four
years. Between 2016 and
2019, the rate will be reduced
from 11 per cent to 9 per cent,
a move that is expected to
provide $1.2-billion a year
in tax relief when fully
implemented. The lower rate
applies to a business’s first
$500,000 in qualifying income.

Other budget proposals include an increase in the lifetime capital gains exemption for
farmers and fishers. It was raised to $1-million from roughly $800,000 for all property sold
on or after budget day.
Dan Kelly, president of the Canadian Federation of Independent Business (CFIB), praised the
budget, calling it, “Perhaps the most decidedly pro small business budget we’ve seen. CFIB
applauds the government for lowering the tax burden on Canada’s small businesses now
that the budget has been balanced. This builds on earlier announcements of Employment
Insurance premium relief, new measures to address credit card fees and balanced budget
legislation.”
Reducing the small business corporate tax rate was viewed by CFIB members as the most
effective measure the federal government could take to strengthen the performance of small
firms, he noted. “We are especially pleased that government intends to legislate the full small
business tax cut plan before the (fall 2015) election.”

!For those with a net yearly taxable
income of $30,000, $1,500 in total savings.
!For those with a net yearly taxable income
of $500,000, $25,000 in total savings.
Kelly also lauded the following budget
provisions:
!Returning the budget to balance and
introducing balanced budget legislation
to protect taxpayers for the future. The
government is projecting a $1.4 billion
surplus for 2015, the first in eight years
for Canada’s federal government.
!An Accelerated Capital Cost Allowance
(CCA) for manufacturers for the next
10 years.
!Several measures to reduce red tape,
including legislating a plan to cut a
regulation for every new one that is
implemented (1:1 rule).
!Confirmation that small firms will save
$550 million in lower Employment
Insurance rates through the Small
Business Job Credit before even larger
premium cuts start in 2017.
!Changes at the Canada Revenue Agency
(CRA), including less frequent
remittances for new firms, a commitment
to honour all written advice, including
information provided by the CRA
website, and a new CRA forum with CFIB.
“In the past, if the CRA gave you bad
advice, their auditors weren’t obligated
to acknowledge it and you were liable for
any penalties that resulted from it,” said
Kelly. “Now, if the CRA makes an error, the
taxpayer won’t be on the hook for it.”
The budget also contains tax breaks for
women entrepreneurs, with a program
to help “connect women with the tools
they need to succeed in business.”
The budget suggests these tools could
include mentoring and network initiatives,
as well as access to $700 million over three
years through the Business Development
Bank of Canada. The program would
also encourage trade missions for
women-led companies to widen exposure to
international markets.
Kelly said the budget’s biggest surprise
was the increased lifetime capital gains tax
exemption for fishers and farmers. “It’s a
big deal for us because it will help to secure
the retirement of many of Canada’s small
business owners. This measure also gives
us something to shoot for in the future:
increasing the capital gains tax exemption
to $1 million for all of Canada’s small
business owners will be one of our goals
for 2016.”

The budget wasn’t perfect from a small
business standpoint, however. “There
wasn’t as much about reducing the size
and cost of government as CFIB would
have liked,” said Kelly. However, the federal
public service, which is in the midst of
contract negotiations, did receive notice
that the government intends to save $900
million by revamping sick-leave policies,
one way or another.
Noting that the government intends to
pass legislation during the coming session
to put the announced business corporate
tax reductions in place, Kelly said this
demonstrates that this is not just a political

commitment.

to attend a full weekend of continuing
education. Since Opticians were able to fulfill
their portfolio requirements all in one
weekend, the program quickly sold out.
Optifair Canada Trade Show, which ran
concurrently during AOE’s CE seminar, was
bustling with activity. Along with all of the
well-known suppliers, many new companies
also exhibited their products and services.
Optometric Assistants were invited to a
training course that featured one-on-one,
hands-on workshops on frame fitting and
sales and service techniques. Due to the high
demand of these workshops, this program all
sold out quickly.
AOE’s fall seminar and Trade Show are
scheduled for Saturday and Sunday October
31 and November 1, 2015. Registration

Optometrists enjoyed a weekend of
educational lectures with topics that ranged
from technological innovations to ocular
surface conditions. The program also
featured a panel of seven ophthalmologists
from the Toronto area that participated in a
grand rounds panel discussion.
On October 30, 2015, AOE will be hosting
its third annual Eye Ball Gala Fundraiser
at the Embassy Grand Convention Centre
in Brampton, Ontario. All proceeds raised
from the gala will go directly to AOE’s
beneficiary – The Foundation Fighting

Canadians Believe UV is Most Dangerous Form of
Light, But …
Most Canadians (87 percent) think sunlight is dangerous for their eyes, but, despite
this, more than half of Canadians falsely believe that you only need to protect your
eyes from the sun’s UV rays in the warmer, summer months. Results from the 2015
“Light and Glare” survey from Transitions Optical show that 87 percent of Canadians
agree that they should protect their eyes from the sun in July, but only 47 percent say
the same for the month of December.
“Unfortunately, the survey results reveal a large group of Canadians who are still
in the dark when it comes to understanding the importance of UV protection for
the eyes year-round,” said Isabelle Tremblay-Dawson, senior marketing manager,
Canada. “We hope this data will urge more people in the industry to talk more
frequently about the ways in which UV rays are present during the entire calendar
year.”
In addition to thinking that sunlight is dangerous to the eyes, Canadians also believe
that blue light from digital devices (50 percent) and overhead lighting indoors
(34 percent) can be harmful to the eyes as well.

“It is interesting that there is so much higher recognition of the danger of UV rays,
versus blue light, because Canadians actually report that they increased their time
spent using digital devices (70 percent) rather than their time outdoors (18 percent)
over the past five years,” said Tremblay-Dawson. “The research suggests that even
though Canadians are not necessarily spending more time outdoors, they still
recognize that UV can be dangerous to the eyes – it’s just not top-of-mind
year-round.”

With the summer season upon us, WestGroupe’s summer collection

CooperVision has named Jerry Warner as president, North America, responsible for
determining the strategic direction of the company’s largest region, driving sustainable business
growth and deeper customer relationships in the United States and Canada.

Warner previously served as CooperVision’s senior vice president of global marketing.
He joined the company in 2012 as vice president, life cycle management.
Reactive Revamped
“Jerry has extensive vision care experience, having spent the past two decades working
hand-in-hand with eyecare practitioners, retailers, researchers, educators, field sales teams and
others in global and regional roles,” said Dennis Murphy, executive vice president of global sales
and marketing at CooperVision. “The depth of his expertise, encompassing strategic planning,
sales, marketing, branding, pricing, and life cycle management will help drive CooperVision
and the entire contact lens industry forward.
“As a key member of our global leadership team, Jerry has helped grow CooperVision’s
worldwide market share by more than four share points over the last three years, and his
insights and knowledge have been instrumental in introducing inventive products and services,
such as MyDay® and LensFerry® that are benefitting customers and patients alike. He’ll bring
that same drive, vision and commitment to North American customers and their business.”
Prior to joining CooperVision, Warner was with Bausch + Lomb for 17 years. During that
time, he held numerous leadership positions, including vice president of marketing/general
manager of global contact lens; vice president of global strategy for contact lens; and director of
marketing for lens care – North America.
Optika Eyewear has totally reworked its
Reactive collection for small faces. Their
top-selling line is perfect for both adolescent and ladies with eye sizes ranging

Expected in August, the new line will
present 15 new styles featuring a mix of
super lightweight stainless steel and acetates. The model RE5251 shows the latest mix of acetate colours by combining
two different acetates and fusing them
together, making a new exciting impact.

The corporation New Look Eyewear will
be re-named New Look Vision Group Inc./
Groupe Vision New Look Inc. The new
designation received shareholder approval
at the annual and special meeting of the
corporation held May 7 in Montreal.
The new name will better reflect the national
aspirations of the corporation and the
multiple banners of its activities. All banners
– New Look Eyewear, Vogue Optical and
Greiche & Scaff – will retain their own
identities and maintain their operations
under the same designations, as in the past.
