ophthalmologists. Professor Sebastian Wolf (University Eye Hospital Bern, Switzerland) will review anti-VEGF therapy in wet
AMD, including a review of the VIEW trial of aflibercept. Professor Ramin Tadayoni (Lariboisière University Hospital, Paris,
France) will consider the advantages and disadvantages of different treatment regimens for the treatment of wet AMD;
namely fixed-dose, PRN and treat-and-extend. Finally, Dr K. Bailey Freund (Vitreous-Retina-Macula Consultants of New York,
USA) will review long-term experience with anti-VEGF therapy and highlight new data on patients receiving a treat-andextend regimen.

The incidence of blindness due to wet age-related macular degeneration (AMD) has reduced considerably since the
introduction of anti-VEGF agents, notably ranibizumab. Building on this success requires a deeper understanding of wet

their risk of developing the disease as well as their responses to treatment. In order to minimize risk and maximize vision

Expanding Options for Presbyopic Correction

Both multifocal IOLs and modified monovision approaches employing traditional monofocal IOLs are well accepted
options to enhance the range of unaided vision and reduce spectacle dependence following cataract surgery. A growing
number of studies comparing these approaches highlight the strengths and weaknesses of each. According to market
survey, demand of multifocal IOLs and monovision approach have been increasing and will be increased for the next
several years. Extended depth of focus monofocal IOLs could further enhance modified monovision alternatives thereby
expanding the options to correct presbyopia at the time of cataract surgery.

Assistant Prof. Shizuka Koh, who is in charge of the Dry-eye clinic of Osaka University, will talk about visual disturbance
associated with Dry-eye. Prof. Yufeng Yao, who is one of authorities of corneal transplant in China, will present a new
technique in surgery of Deep Anterior Lamellar Keratoplasty. Prof. Zouguo Liu, who is a pioneer of Dry-eye in China,
will talk about developing a Dry-eye model of mouse induced by topical administration of benzalkonium chloride and
investigating the possible mechanism.

Should Eye Imaging Be Part of a Standard Newborn Examination?

Significant strides have been made to improve the overall standard of care for our patients in the NICU and the management of Retinopathy of
Prematurity (ROP) has been no exception. Four prominent International Pediatric Ophthalmology specialists and one prominent International Pediatric/
Neonatology specialist will share their experiences with regard to ROP management and the imaging of all newborns for ophthalmic abnormalities.
Documenting ROP examinations with digital images helps physicians to better manage ROP both locally and at remote locations. It has enabled the
Pediatric Ophthalmologist to track the pathology longitudinally with objective detailed images instead of subjective drawings and notes. This has
resulted in an improvement in the overall standard of care for the patient.
Recent clinical data has concluded that pathologies other than ROP can be detected in a full-term neonate at birth with a standard ophthalmic exam.
These pathologies include congenital glaucoma, congenital cataract, coloboma, hamartoma, retinoblastoma and nerve abnormalities.
The pathology incidence rate in preliminary studies has ranged from 0.5% to 2.5% which appears significant compared to the incidence level for
childhood hearing loss (0.3%). Nearly 85% of a child’s learning and social development is derived from their eyesight leading many to believe the need
for an ophthalmic exam at birth deserves considerable attention.

Macular Edema Secondary to CRVO: Optimizing Outcomes

Changing the Treatment Paradigm: Ranibizumab for Myopic CNV

Myopia is an increasingly common eye condition in many countries, particularly in East Asia. In its most severe form,
pathological myopia (PM), it is a major cause of visual impairment and blindness worldwide, especially in a younger
population. CNV is the most common vision-threatening complication in PM. Ranibizumab provides transformational
efficacy with a very low number of injections. It is the first and only anti-VEGF therapy licensed to treat these patients. This
symposium discusses clinical evidence supporting the use of ranibizumab in myopic CNV, and introduces the evolving
treatment recommendations for optimal management of patients with myopic CNV.

Ranibizumab: Transformational Outcomes in Patients with DME and RVO

It is increasingly clear that, in DME and RVO, early-stage pathology is driven by VEGF. Consequently, physicians should
endeavor to initiate early treatment using anti-VEGF agents. Comorbidities in patients with DME and RVO may mandate
the use of anti-VEGF treatments with minimal systemic exposure to optimize the benefit-risk ratio. Data from recent clinical
trials of ranibizumab demonstrate significant visual and anatomic improvements and minimal systemic exposure with an
individualized treatment regimen. With the wealth of available evidence and its well-established safety profile, ranibizumab
is a preferred option in the treatment of patients with DME and RVO (both BRVO and CRVO).

Evidence of Neuroprotection in Topical Treatment

Approaches to neuroprotection include identifying exogenous agents that alleviate neuronal damage or upregulate
causative intracellular processes. Several topical anti-glaucoma agents reportedly show neuroprotective effects in vitro,
particularly brimonidine that is neuroprotective in several experimental systems. Moreover, topical brimonidine 0.2%
better preserved visual field than timolol 0.5% in the randomized double masked Low-pressure Glaucoma Treatment
Study (LoGTS). In this symposium, the pathophysiology of optic nerve damage and its amelioration via neuroprotection,
particularly with brimonidine, will be addressed.

The Present and the Future of Optical Coherence Tomography for Anterior Segment Evaluation

The precise, quantitative analysis of anterior segment shape is essential for corneal irregularity, primary glaucoma and
cataract surgery. For instance, the applicable clinical situations are the corneal shape evaluation of keratoconus suspected,
the evaluation of anterior chamber angle shape in primary angle-closure glaucoma, and the IOL calculation in patients
with post-refractive surgeries including LASIK, and moderate/severe corneal astigmatism.
Several optical coherence tomographies are used in these circumstances. Among them, the swept source optical
coherence tomography (SSOCT, CASIA, Tomey Corporation) is a novel imaging device that employs a 1310 nm swept laser,
which is the state of the art OCT device for the anterior-segment quantitative analysis mentioned above.
In this seminar, we invite Dr. Naoyuki Maeda, Japan, and Dr. Tin Aung, Singapore, two distinguished clinician- scientists in

For instance, conjunctivitis due to quinolone-resistant Corynebacterium spp. is a Japan-specific phenomenon, probably due
to a 90% share of fluoroquinolones in antibiotic eye drops. Pre-operative povidone-iodine is used as 5% in eye drops in the
US, and as 0.625% in eye wash in Japan.
In this seminar, Dr. O’Brien will present the treatment of conjunctivitis and the prevention of postoperative endophthalmitis
in the US, and Dr. Hatano will present the treatment of conjunctivitis in Japan, and I will present the prevention of
postoperative endophthalmitis in Japan. The implications derived from the comparison of the strategies against ocular

work life. The anomalous trichromat has less restriction than the dichromat. Individuals with congenital colour deficiency

were accumulated until now.
This presentation is the outcome of the treatment using Vancomycin Ophthalmic Ointment 1% obtained from the above
clinical study data and post marketing surveillance.
No detection on MRSA and MRSE with resistance against Vancomycin has been found in ophthalmology field in Japan up
to now.

knowledge of its pathogenesis such as AHT (anterior hyaloid membrane tear).
Dr. Souichi Matsumoto will discuss treatment strategies for posterior capsule rupture on a case-by-case basis, depending
on the degree of the rupture or the amount of residual lens nuclei.
Dr. Masayuki Akimoto will explain the pros and cons of various intraocular lens (IOL) fixation methods for cases in which inthe-bag fixation of IOL is impossible and introduce the intrascleral fixation procedure of IOL.

pathophysiology of ocular surface disorders. When the barrier is intact, the development of allergic conjunctivitis is

eye disease, mucins on the ocular surface play crucial roles in controlling the barrier function

Due to progress of health care, aged population is increasing worldwide and non-communicable diseases (NCDs) are becoming important medical and economic burden.
NCDs develop through interplay between genetic and environmental factors, progressing slowly. It is important, therefore, to prevent disease onset by predicting NCDs
based on genetics and biomarkers and introducing early intervention. This is a preemptive approach which is expected to reduce social and economic burden.

information, perform precise inspections, and adopt a richly imaginative approach when evaluating pathology. In so doing,
we can enable a more accurate diagnosis and effective medical treatment.
Together, we will take a look at actual case studies through videos and images while considering diagnosis points and
medical treatment. Although we only have 60 minutes, we aim to show as many cases as possible and talk about the
essence of our 3i Cornea Practice. We look forward to studying with you.
