I, the undersigned, certify that:

Health Information
I. The fish are sourced from registered farm under Department of Fisheries Malaysia and not
associated wih any significant diseases or pest within the previous 6 (six) months.
II. The consignment was inspected by DoF 7 days prior to shipment.
III. The live fish have been subjected to a health surveillance program according to the
procedures as described in the “Diagnostic Manual for Aquatic Animal Diseases” from Office
International Des Epizooties (OIE).
IV. The consignment originated from OIE listed diseases free zone. There is no any disease
outbreak for a period of two years prior to export.
V. The consigment are not subject to any prohibitions due to unresolved increased mortaliy.
VI. The fish being held for 14 days prior to export at the exporter premises and show no clinical
signs of disease.
VII. The fish are not being kept in same water with farm foodfish (fish farm for human
comsumption including recretional fishing) or koi carp.
VIII. Adequate safeguard are in place to maintain certified fish health status until export.

I.
The consignment was inspected by Department of Fisheries 7 days prior to
shipment.
II.
The fish were exported from a premises approved to export by competent
authority.
III.
The fish are not sourced from a population associated with any significant
infectious disease or pests and there have not been any unusual or substantial
outbreaks of infectious fish disease or pests in the areas from which the fish have
been collected during the 6 months prior to collection.
IV.
The fish were collected from an area at least 5 kilometers from any food fish
aquaculture operation and the fish in the consignment have not come into contact
with water, equipment or fish associated with farmed food fish (fish farmed for
human consumption including recreational fishing).
V.
The fish are wild caught and have not been bred or hatched on a farm or other
premises.

Description of content: Fisheries Biosecurity Division under the Department of Fisheries
is enacting "The notice of the model and applicability of the new health certificate of live
fish " (available in English, 3 pages) according to the Section 40(2) on Control of Live
Fish, Fisheries Act 1985 (Act 317) stated that:
"The Director-General may impose such condition as he thinks fit in the permit, including
conditions concerning the state of cleanliness of the fish to be exported, imported or
transported and measures to avoid the spread of communicable fish diseases, or to avoid or
control the release into the natural environment of non-indigenous species of fish."
The purpose of this notification is to inform all countries of the model and applicability of
the new health certificate of live fish.
The main contents of the Announcement are as follows:

New model of health certificate of live fish from Malaysia for export.
