SanofiAventis advise that piportil is being discontinued from the end of March 2015 due to problems sourcing one of the ingredients. Please review all patients currently receiving piportil and consider a suitable alternative. Nitrofurantoin is now contraindicated in patients with an estimated glomerular filtration rate (eGFR) of less than 45ml/min/1.73m2 (this is a reduction from the previous rate of 60). Denosumab has been associated with a risk of osteonecrosis of the jaw (ONJ) and with a risk of hypocalcaemia. Before starting denosumab treatment, a dental examination is recommended. Lurasidone will remain non-formulary. Lurasidone a new second generation antipsychotic has recently been evaluated by the mental health guidelines and formulary committee. The conclusion from the committee was there is currently insufficient long term safety and efficacy date to recommend its use. Other antipsychotics with similar properties are also already available. Lurasidone will remain nonformulary. The decision will be reviewed when new evidence becomes available. Are now available on the trust website. This includes guidance on how to switch from other antipsychotics. Please note consultants psychiatrists wishing to use the aripiprazole long acting are requested to complete the application form for approval found at: http://www.southernhealth.nhs.uk/knowledge/medicines-management/forms/?assetdet1450445=78951 Self-administration guidelines: (SH CP 168) has recently been updated and now covers trust wide services. Wards are requested to check they are working to the new guidelines. The procedure on checking and filling a compliance aid (SH CP 170) has recently been updated and is now available on the trust website, Oral Anticoagulants online e-learning. MRHA have just launched an online, e-learning module which outlines the key risks of these widely prescribed medicines. To access the learning module, you will need to create an account. The medicines management team recently undertook a medicines waste audit across in-patient units. Please can all in-patient units ensure they have controlled drug denaturing kits available, a list of cytotoxic and cytostatic medicines attached to the purple waste bin and that the blue medicines waste bin is locked away. Omission audit results. A recent omission audit looking at how nursing staff sign prescription charts returned the lowest rate of blanks yet recorded. Looking at over 5000 individual drugs administered 1.2% were not signed for and left blank. This is an improvement from 1.7% in 2013 and 9.2% in 2012. Reminder…… Patient Group Directions (PGDs).
