Livostin Pt 01-2013 SUB 2 מידע חשוב על חלק מהמרכיבים של התרופה התכשיר מכיל 0.15מ"ג במ"ל בנזלקוניום כלוריד בדומה לטיפות עיניים אחרות המכילות משמר זה הנקרא בנזלקוניום כלוריד ,אין להרכיב עדשות מגע רכות בזמן הטיפול בליבוסטין. .3כיצד תשתמש בתרופה: תמיד יש להשתמש לפי הוראות הרופא. עליך לבדוק עם הרופא או הרוקח אם אינך בטוח. המינון ואופן הטיפול יקבעו על ידי הרופא בלבד. המינון המקובל בדרך כלל במבוגרים וילדים הוא 1 :טיפה של ליבוסטין לעין ,פעמיים ביום .אם התסמינים מטרידים מאוד ניתן לטפטף טיפה בעין 3או 4פעמים ביום. אין לעבור על המנה המומלצת אין לבלוע תרופה זו מיועדת לשימוש חיצוני בלבד אופן השימוש: ליבוסטין טיפות עיניים הינן תרחיף ועל כן יש לנער היטב את הבקבוקון לפני כל שימוש. לפני השימוש הראשון בטיפות העיניים יש להסיר לחלוטין את כיסוי הטבעת החותמת את הפקק. .1נער היטב את הבקבוקון לפני הסרת הפקק. .2הטה את ראשך לאחור ככל שתוכל .3לחץ את הבקבוק ובזהירות טפטף טיפה אחת לזווית הפנימית של העין .משוך את העפעף התחתון כלפי מטה בזמן שראשך עדיין מוטה לאחור כך שהטיפה תוכל להגיע לאזור שבין החלק התחתון של העין והעפעף התחתון .כתגובה אוטומטית לכך תמצמץ ,וכך הטיפה תתפזר בכל העין. .4חזור על שלב 3בעין השניה. אין לגעת בעין עם הבקבוקון ,כדי ששום מזהמים לא יחדרו לנוזל שבבקבוק .יש להמשיך בטיפול עד שהתסמינים ישתפרו. QUALITATIVE AND QUANTITATIVE COMPOSITION Levocabastine hydrochloride equivalent to 0.5 mg levocabastine per ml For excipients, see Section 6.1. 3. PHARMACEUTICAL FORM Eye drops 4. CLINICAL PARTICULARS 4.1. Therapeutic Indications Allergic conjunctivitis (classic and vernal) Livostine eye drops is indicated for the rapid and long lasting relief of eye complaints such as itching, redness, and watering eyes associated with allergies, for example to grass, pollen, moulds and dust. 4.2. Posology and Method of Administration Adults and children: the usual dose is 1 drop of LIVOSTIN eye drops per eye, 2 times daily. The dose may be increased to 1 drop 3 to 4 times daily. Treatment should be continued as long as required for symptom relief. LIVOSTIN eye drops should be used within one month of the first opening of the bottle. Patients should be instructed to take appropriate measures to avoid contamination of the bottle. 4.3. Contraindications Hypersensitivity to any of the ingredients. 4.4. Special Warnings and Special Precautions for Use As with all ophthalmic preparations containing benzalkonium chloride patients are advised not to wear soft (hydrophilic) contact lenses while under treatment with LIVOSTIN eye drops. Pregnancy and Lactation 4.6.1. Use during pregnancy No significant embryotoxic or teratogenic effects were observed in animals (see Non-clinical Information). In mice, rats and rabbits, levocabastine, at systemic doses up to 2,500 times (on a mg/kg basis) the recommended maximum ocular clinical dose, did not reveal any embryotoxic or teratogenic effects. In rodents, levocabastine at systemic doses beyond 5000 times (on a mg/kg basis) the recommended maximum ocular dose, teratogenicity and/or increased embryonal resorption were observed There are limited postmarketing data on the use of levocabasine eye drops in pregnant women. The risk for humans is unknown. Therefore, LIVOSTIN eye drops should not be used during pregnancy unless the potential benefit to the woman justifies the potential risk to the fetus. 4.6.2. Use during lactation Based on determinations of levocabastine concentrations in saliva and breast milk in a nursing woman who received a single oral dose of 0.5 mg levocabastine, it is expected that approximately 0.3% of the total ophthalmically administered dose of levocabastine may be transferred to a nursing infant. However, due to the limited nature of the clinical and experimental data, it is recommended that caution be exercised when administering LIVOSTIN eye drops to nursing women. 4.6.3 Fertility Animal data have shown no effects on male or female fertility. 4.7. Effects on ability to drive and use machines LIVOSTIN eye drops do not produce sedation nor do they impair psychomotor performance. Adverse reactions such has eye irritation, pain, swelling, itching, redness, burning sensation in the eye, watery eyes, and blurred vision which may impair vision have been reported. Livostin_eye_Dr_Jan_13_sub Page 3 of 8 Therefore, caution is advised when driving or operating machinery following application of LIVOSTIN eye drops. Therefore they can be used by patients driving cars or operating machinery. 4.8 Undesirable effects Adverse Reactions Throughout this section, adverse reactions are presented. Adverse reactions are adverse events that were considered to be reasonably associated with the use of levocabastine based on the comprehensive assessment of the available adverse event information. A causal relationship with levocabastine cannot be reliably established in individual cases. Further, because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug and may not reflect the rates observed in clinical practice 4.8.1 Clinical trial data The safety of LIVOSTIN eye drops was evaluated in 508 subjects who participated in four placebo-controlled clinical trials and one open-label clinical trial. All adverse drug reactions (ADRs) reported by subjects in LIVOSTIN eye drops clinical trials are presented in Table 1. (4) Because these reactions are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure. Overdose • Symptoms Following accidental intake of the contents of the bottle, sedation may occur. There have been no reports of overdosing with LIVOSTIN. Some sedation after accidental intake of the contents of the bottle cannot be excluded. • Treatment In case of accidental ingestion, the patient should be advised to drink a lot of non-alcoholic liquids in order to accelerate the renal elimination of levocabastine. Pharmacodynamic properties Pharmacotherapeutic group: Decongestants and Antiallergics, ATC code: S01GX02 Mechanism of action LIVOSTIN eye drops contain levocabastine, a very potent, fast-acting and highly selective histamine H 1 -antagonist with a sustained duration of action. After topical application to the eyes, it almost immediately and for several hours relieves the typical symptoms of allergic conjunctivitis (itching, redness, chemosis, eyelid swelling, tearing). 5.2. Pharmacokinetic properties 5.2.1 Absorption After instillation in the eyes, levocabastine is slowly and incompletely absorbed. After ophthalmic application of 15 μg/drop dose, about 6 μg of levocabastine is absorbed. After ophthalmic administration levocabastine reaches peak plasma levels after about 6 hours. 5.2.2 Distribution Protein binding of levocabastine in plasma is approximately 55%. 5.2.3 Metabolism The primary metabolite of levocabastine, an acylglucuronide, is produced by glucuronidation, the major metabolic pathway. 5.2.4 Elimination Levocabastine is predominantly excreted in the urine as unchanged drug (about 70% of the absorbed dose). The terminal half-life of levocabastine is approximately 39–70 hours. The plasma pharmacokinetics of ophthalmic levocabastine are linear and predictable. Special Populations Pediatrics Sparse levocabastine plasma concentrations were measured in children and adolescents aged between 6 and 17 years who received levocabastine nasal spray at various dosing regimens up to a maximum of 0.2 mg four times a day for 4 weeks, some of which were also using Livostin_eye_Dr_Jan_13_sub Page 6 of 8 levocabastine eye drops as needed. Plasma concentrations measured after 2 to 4 weeks of treatment were either undetectable or ranged up to a maximum of 18.2 ng/mL. Based on the limited information available, no firm conclusions could be drawn with regards to a comparison versus adults. Elderly In the elderly, after multiple nasal administrations of 0.4 mg levocabastine, the terminal halflife of levocabastine was increased by 15%, and the peak plasma level was increased by 26%. Renal Impairment After a single oral dose of 0.5 mg levocabastine in solution, the terminal half-life of levocabastine in moderate to severe renal impairment (Creatinine Clearance 10–50 mL/min) increased from 36 hours to 95 hours. Overall exposure to levocabastine based on AUC was increased by 56%. Hepatic impairment The pharmacokinetics of levocabastine in subjects with hepatic impairment have not been investigated. 5.3. Effects were observed only at exposures considered sufficiently in excess of the maximum human dose level to indicate little, if any, relevance to clinical use. Incompatibilities None known. 6.3. Shelf life 2 years. LIVOSTIN eye drops should be used within one month of the first opening of the bottle. 6.4. Special precautions for storage Store at 25° C or below. Keep out of reach of children. 6.5. Nature and contents of container 5 ml plastic bottles containing 4 ml of white microsuspension. 6.6. Instructions for use/handling As LIVOSTIN eye drops are available as a microsuspension, the bottle should be shaken before each application. 1. Shake the bottle well before removing the cap. 2. Tilt your head backwards as far as possible. 3. Squeeze the bottle and carefully apply 1 drop into the inner corner of the eye. Then pull the lower eyelid down with your head still tilted backwards so that the drops can reach the area between the lower part of the eye and the lower eyelid.
