That’s why the CDSPI Group Benefits plan (offered through Quikcard) allows dentists to protect themselves and their immediate family (through their professional corporations) — or everyone employed in their practices — with employee group benefits. With the CDSPI Group Benefits plan, everyday health care costs, such as prescription medications and extended health can be covered. These benefits can be provided on an insured basis, or through innovative Health Spending Accounts which provide low administration fees and maximum flexibility — while allowing cost certainty for the dentist. To manage expenses for more serious circumstances, a full range of employee group insurance products is also available, such as life, disability and travel insurance. Choosing affordable Health Spending Accounts for the health benefits provides flexible coverage employees want and cost-effectiveness that dentists need. Flexible Health Benefits The plan offers a range of health benefit options, including coverage for: •Vision: Prescription eyeglasses and contact lenses •Extended health: Health costs beyond prescriptions, such as hospital stays, ambulances, diabetic supplies and more •Dental: Basic, major and orthodontic dental services •Prescription drug coverage: Long-term medication or monthly maintenance prescription requirements •Stop Loss: Prescription drug coverage in the case of an unexpected medical event that necessitates costly drug treatment Dentists create their own custom packages by selecting the types of health services they want and the limits for employees. They can choose these benefits on an insured basis, or through innovative and affordable Health Spending Accounts — which can also cover preventive and alternative therapies. Health Spending Accounts Health Spending Accounts (HSAs) are not insured products. They are fully funded by the employer (or if desired, also in part by employees). The funds are administered by Quikcard and used to pay for health claims for participating employees. The best part is that most health treatments are eligible for coverage under an HSA. HSAs are cost effective. There are no sign up fees, no transaction fees and no medical questionnaires to answer. Administration fees are only charged on the claimed amounts and the unused portion of the contributions are returned or rolled into the next year. Fast and Convenient Service Under the plan, trusted CDSPI Advisory Service Inc. and Quikcard advisors provide dentists with the information and advice they need to make the best decisions for their practices and help employees sign up quickly and conveniently. Quikcard offers easy administration, superior customer service and prompt claim payments. Employees can check the status of their Health Spending Accounts online, and administrators in the practice can quickly add or remove members from the plan and view an up-to-date company statement through the Quikcard website. Eligibility You are eligible to apply for CDSPI Group Benefits if you are a licensed dentist resident in Canada practising outside of Ontario or Quebec, who is a member of a participating provincial or territorial dental association. In order for a dentist to obtain CDSPI Group Benefits for themselves and their families only (and not employees of their practice), they must have a professional corporation or be a sole proprietor. Long term disability insurance is not available to dentists under CDSPI Group Benefits, but is available to dental office staff. Dentists wanting disability insurance can contact CDSPI for information about the Long Term Disability Insurance plan available through the Canadian Dentists’ Insurance Program. Conditions and Limitations For each of the components available under the CDSPI Group Benefits plan, coverage details, terms, conditions and exclusions will apply based on the specific component selected. Participants will be provided with coverage information by Quikcard. HSAs are typically set up with some limits on individual expenses or on the type of expenses that can be claimed. These limits are determined by the employer, meaning every plan is customized. Superior Insurance Plans The CDSPI Group Benefits plan offers a range of employee group insurance products* backed by leading group insurers in Canada, including: •Life and dependent life insurance: For the employee and for the employee’s dependents •Short term disability insurance: Protection of an employee’s income in the event of illness, accident or hospitalizations for up to 17 weeks of continuous disability •Long term disability insurance†: Protection of an employee’s income in the event of illness, accident or hospitalization after 120 days of continuous disability •Accidental death and dismemberment insurance (AD&D): Coverage in the event of accidental death or dismemberment (loss of eyes, limbs etc.) •Critical illness insurance: A lump sum payment in the event of a serious illness or condition such as heart attack, stroke or cancer •Medical travel insurance: Covers employees, their spouses and dependents for unforeseen medical expenses while travelling on trips of 60 days or less * Insurance products are provided by selected Canadian insurers and are arranged for by Quikcard Benefits Consulting Inc. and CDSPI Advisory Services Inc. † Not available under this plan for dentists, but available for dental office staff. 15-460 12/14
