The Upper Peninsula Health Plan’s pharmacy formulary for Medicaid, CSHCS, MIChild and
Healthy Michigan members is available through Epocrates® drug-reference software at
www.epocrates.com.
Epocrates® is a software vender that integrates clinical drug information with drug formulary
information. Epocrates® can be used to check formulary status, prior authorization
requirements, formulary alternatives, generic substitutions and quantity limits.
The following formulary status codes provide detailed drug information in Epocrates®

Yes, this on the formulary.
GP
Generic preferred- Only the generic product will pay.
ST
Step Therapy criteria applies.
PA
Prior Authorization is required.
N
Non formulary- Needs a prior authorization.
Covered by UPHP for MIChild.
Carve Out to MDCH First Health for UPHP Medicaid.
Not covered by UPHP. Carve Out to MDCH, First Health

The basic Epocrates® product is available for free and is the only one necessary for access to the
Upper Peninsula Health Plan formulary. Epocrates® also offers expanded products for a fee.

o Follow instructions for Free Download
Choose Online Products for computers
o Click “Go to Epocrates® Online”
o Follow instructions to register for Epocrates®.

From the Epocrates® Online page, click on “Edit Formulary List”
Follow the instructions there to build your formulary list
Choose “Upper Peninsula Health Plan”
Add “Upper Peninsula Health Plan” to list
Click on “close” in the upper right hand corner to return to “Drug Lookup”
