ADVANCED NON-INVASIVE TECHNIQUES IN EARLY DETECTION OF ORAL CANCER PRESENTED BY: Abhishek Ghosh 3rd prof. B.D.S. Dr.R.Ahmed Dental college &Hospital GUIDED BY: Dr. Jay Gopal Ray H.O.D. Dept. Of Oral & Maxillofacial Pathology ORAL CANCER : SOME DEADLY STATISTICS • Every hour 1 American dies of oral cancer • Death rate is higher than cervical cancer • 25%cases have no risk factor IMPORTANCE OF EARLY DETECTION 70% cases are diagnosed in late stages It leads to 5 year survival rate of 57% patients Oral cancer is 90% curable when detected early Why non-invasive techniques? • Painless and bloodless technique • Biopsy is not possible in screening examination • Helpful for follow-up cases • Easy and simple for operator • Patient’s medical status is fragile for biopsy • Patient refuses for biopsy EXFOLIA -TIVE CYTOLOGY INTRA ORAL VITAL STAINING IR THERMO -GRAPHY VIZILITE EXFOLIATIVE CYTOLOGY It is the histopathological examination of cells obtained by their physical removal, followed by placement on glassslide and then appropriately stained. Preparation of ‘Pap-smear’ VIZILITE Simple painless noninvasive technology to visualize suspicious areas at earlier stages what we may not see by naked eyes .It’s based on the principle of CHEMILUMINESCENCE ACTION OF VIZILITE Under vizilite normal healthy tissue will absorb light & appear dark. Abnormal tissue will appear white. HOW VIZILITE WORKS? Mouth rinse • Removes mucosal glycoprotei -n • Increase N-C ratio Light stick • Differentia -te between normal & abnormal tissue STEPS Encourage the patient after routine exam Rinse with 1%acetic acid for 30-60 sec Shake vigorously to mix content of light stick Bend outer light stick breaking inner vial Insert it into open end of retractor & assemble Make the room dark & re-examine the oral cavity with vizilite Intra-oral vital staining With Toluidine blue helps to identify dysplastic and malignant mucous membrane in the oral cavity. STEPS Drying of mucosa with gauge Mouth rinse with 1% T-blue for 1-2 min 1 min rinse with 1% acetic acid for decolourisation Malignant cells have more metaboli -c rate They retain the stain Normal mucosa decolourises VIZILITE &TOLUIDINE BLUE VIZILITE TBLUE SITE FOR BIOPSY IR THERMOGRAPHY It uses the heat of our body to aid in making diagnosis. It is completely safe and uses no radiation. 2 parts-1.IR CAMERA 2.HIGH RESOLUTION COMPUTER MECHANISM Detection of breast cancer Use in oral cancer –on way Malignant tissue has more vascularity Raised surface temperature Detected by thermograph ADJUNCT TO SURGICAL BIOPSY Visual examinati -on Screening Biopsy Confirm diagnosis CONCLUSION These screening techniques help in early detection of malignancy with an accuracy of 90 % and thus % THANK YOU
