Admissions Requirements

Now Accepting Applications for Fall 2015
Ophthalmic Medical Assistant is a limited enrollment program. Each year a maximum of 20 students are selected
along with 5 alternates. Students are ranked according to the score on the attached admission form. If you are not
accepted and you wish to reapply, you must complete a new Ophthalmic Medical Assistant Admission Form.
After acceptance and enrollment in the program, student must take all OPH courses as scheduled and sequenced.
Prerequisites:
o
Math and English Placement testing is required. Students must be eligible to enroll in ENG111 and have
completed all developmental math requirements
Suggested courses:
o ENG111, COM 120, or 231,110, MED 121 and PSY150 may be taken prior to application and acceptance
into the Ophthalmic Medical Assistant program.
Basic Admission Requirements:
Submit Ophthalmic Medical Assistant Admission Form after completing the following steps.
1.
Graduation from high school or completion of the GED.
2.
Complete the College Admission Application in the Central High Building, at one of the multi-campus locations
or on the internet at www.cpcc.edu

3.
Designate the Ophthalmic Medical Assistant Program holding code (A55280V) as the program of interest
on the College Admission Application.
Submit official transcripts, in person or by mail, of high school/GED and college study to CPCC Student
Records, Central High Room 261; P.O. Box 35009, Charlotte, NC 28235.


For college credits or Advanced Placement (AP) exam credit, complete a Request for Transcript
Evaluation form and turn the form in to the Student Records Office. Form may be obtained from any
campus Registration Center or online: http://www.cpcc.edu/admissions/studentrecords/transcript_evaluation_request.pdf/view
Check with Student Records to make sure that transcripts have been received.
If your high school or college transcripts are not from the United States, you will need an International Transcript
Evaluation performed by an agent outside of Central Piedmont Community College. A list of International
Evaluators is available in the Counseling Department in the Central High Building. There is a fee for this service.
4.
All students must meet with an academic advisor or counselor to review placement test scores.

Complete all required Developmental Studies Courses.

Past Academic Performance- The applicant may select a maximum of 4 (four) courses to convert to points.
These may be high school or college courses or may be a combination of both. Transcripts (unofficial) verifying
the courses and the grades must be attached to the application.

Applicants will receive notification within 4 weeks of submission of a completed application or by May 8, 2015
of their status.

Upon acceptance into the Ophthalmic Medical Assistant program, students will be required to submit results of a
North Carolina and/or national criminal background check at their own expense. Students must also complete a
medical exam, provide record of immunizations, show proof of medical insurance and carry CPCC’s accident
insurance. Current Provider Level CPR certification from American Heart Association or American Red Cross
must be completed by the end of the first semester of the program. The student is responsible for the costs of each
of these. Required forms will be distributed at the Ophthalmic Medical Assistant Orientation session for students
who are accepted into the program.

The student must present evidence of good physical and mental health as specified in the ADA Essentials attached
to this packet. (CORE PERFORMANCE STANDARDS FOR ENTRY LEVEL COMPETENCIES FOR THE
OPHTHALMIC MEDICAL ASSISTANT).
The attached Ophthalmic Medical Assisting Admission Form and required Documentation must be submitted to Liza
Lopes (drop box in HE 3545) on or before the application deadline.
All documentation, including copies of unofficial transcripts, must be attached to the Admission form. The application
will be considered incomplete if documentation and unofficial transcripts are not attached. Incomplete applications will
not be considered for admission. Admission is based on points obtained from the following criteria:
1.
Attendance of an Ophthalmic Medical Assistant Information Session
2.
Past Academic Performance (transcripts)
3.
Current Provider Level CPR certification from American Heart Association or American Red Cross.
