Living With and Beyond Cancer STP Workshop May 2016 Who are we talking about? ‘Survivors are those people who are living with or beyond cancer’ Includes those in remission after initial treatment, those who are cured, and those who may have active and progressing disease LWBC Programme • The National Cancer Survivorship Initiative the Living With and Beyond Cancer (LWBC) Programme was set up in June 2014. • 2 yr partnership between NHS England and Macmillan Cancer Support to embed priority findings and recommendations into mainstream NHS commissioning and service provision. Outpatient Care - 2010 • • • • • • Current services do not fulfil the needs of patient. Poor value for money. Inconsistent across cancer specific groups. Not evidence-based. Unsustainable. Will not cope with increasing numbers. • 2 million UK survivors, increasing by 3% each year (30,000 new survivors per yr) • This doesn’t mean they are living well • Complex and varied needs • 1 in 4 live with ill health or a disability as a consequence of cancer treatment (Macmillan 2013) Programme focusses on • All cancer patients having access to Holistic Needs Assessment; EOT summary; CCR; patient education & support event • Developing and commissioning risk stratified pathways of post treatment management. • Promoting Health & Wellbeing including physical activity. • Understanding and commissioning for improved management of the consequences of treatment. Recovery Package Macmillan 2013 Clinical Risk Stratification • Estimated number of patients in each group based on the test pilots undertaken by Macmillan is: • Supported selfmanagement- 40% (9,592) • Shared care- 35% (8,338) • Complex case- 25% (5,955) Treatment Active & recurrent disease Managing consequences of treatment Promoting recovery Sustaining recovery Aims of the Strategy • More people having a positive experience of care and support. • More people having a better long-term quality of life. • Ambitious & stretching but need to act now Priorities • Transformation in support for people living with and beyond cancer, so that by 2020 every person with cancer should have access to the Recovery Package and follow-up care that responds to their needs. Care providers will also become more accountable for improving quality of life, through the development of a new metric to measure this. STP footprints • Opportunity to embed improving outcomes for people with cancer locally, within the context of the wider population. • Chance to improve local pathways based on information, data and experience from the local population. • Opportunity to streamline our patient pathways 1 model does not fit all Complex Monitoring Self manage LWBC • NHS England working in partnership with Macmillan to roll out the RP nationally • Risk statification • Long term consequences • Holistic needs assessment at point of transition What difference will it make ? • • • • • • • Reduce unnecessary GP visits Reduce unnecessary hospital admissions Provide reassurance and support Enable patients to learn self- management Facilitate and advise on returning to work Provides holistic care Prepares patients for the long term consequences of treatments • Support lifestyle choices & changes Referral to LWABC Late effects & Follow up Health & Wellbeing Surveillance Watch and Wait Recovery Package Clinical examination, screening and monitoring for long term consequences Monitoring tumour markers, physical and psychological support Support Group, Activities, Choir, HOPE course, Arts therapy, Walking group Our Vision • Variety of support services as ‘one size does not fit all’. • Equitable access to the Living With & Beyond Cancer service. • LWBC to be an integral part of all tumour site pathways. • Collaborate working with primary care Summary All patients will have: • • • • HNA’s at key points in pathway Individualised care plan/ risk stratification Access to health & well being initiatives Awareness of long term consequences of disease or treatment RECOVERY PACKAGE
