The Cancer Journey Better cancer services every step of the way provincial primary care and cancer network A strong primary care system is the foundation of a strong healthcare system. Cancer Care Ontario (CCO) has brought together a Provincial Lead, 21 Regional Primary Care Leads (RPCLs) and nine Regional Aboriginal Cancer Leads (RACLs) to form the Provincial Primary Care and Cancer Network (PPCCN), an innovative solution to meaningfully engage primary care in all aspects of the cancer system. The role of the RPCLs primary care psychosocial and palliative care • Partner with their Local Health Integration Network (LHIN), Regional Cancer Program and primary care colleagues to: • share their knowledge about cancer and the cancer system • provide continuing professional development (CPD) events • Work with individual primary care providers to improve cancer outcomes and to ensure that patients receive high-quality care every step of the way • Work with CCO to ensure that program design includes primary care engagement Dr. Suzanne Strasberg Provincial Lead, Primary Care Cancer Care Ontario [email protected] The role of the RACLs recovery/ survivorship end-of-life care For more information please contact: Primary Care Centre of Practice [email protected] • Champion the Aboriginal Cancer Strategy vision and goals in collaboration with the Regional Cancer Programs • Identify and respond to systemic issues relevant to First Nations, Inuit and Métis (FNIM) healthcare needs • Ensure that a primary care perspective relevant to FNIM people is included within regional strategic planning and program design Together, the members of the PPCCN work to bring the voice of primary care into the cancer system in a meaningful way, and give primary care providers access to peer contacts who can provide CPD and significant expertise in primary care’s role in cancer care. Dr. Suzanne Strasberg is a comprehensive family physician and physician lead of the Jane Finch Family Health Team in Toronto. She also has active staff privileges at Humber River Regional Hospital and is a graduate of the University of Toronto’s Faculty of Medicine, class of 1992. She served as president of the Ontario Medical Association from 2009 to 2010. LHIN 1 Erie St. Clair Regional Cancer Program | ERIE ST. CLAIR LHIN 5 Dr. John Day [email protected] REGIONAL PRIMARY CARE LEAD Dr. Praveen Bansal is a family physician in Brampton, practicing comprehensive care, including obstetrics. He is also involved with teaching/mentoring residents and is the faculty Development Coordinator for the Brampton site. Dr. Mark Tomen [email protected] Dr. Sanjeev Goel [email protected] REGIONAL ABORIGINAL CANCER LEAD Dr. Mark Tomen worked as a general practitioner in hospital care, emergency departments and surgical assisting in London, Mississauga, and Grand Bend until 1989, at which time he returned to Kent County and took over a family practice in Chatham. He has been affiliated with the Tilbury District Family Health Team since 2010. Mark is also medical director of a large nursing home in Chatham. LHIN 2 Dr. Sanjeev Goel is a family physician and co-founder of A Healing Place in Brampton. Sanjeev graduated from the University of Toronto’s medical school in 1995. LHIN 6 Dr. Mike Vreugdenhil [email protected] Mississauga Halton Central West Regional Cancer Program | MISSISSAUGA HALTON Dr. Paul Philbrook [email protected] Dr. Paul Philbrook is a family physician, practicing at Streetsville Medical Centre. Paul also works in long-term care, as Medical Director of Leisureworld Streetsville Nursing Home. He is on active staff at the Credit Valley Hospital and in July 2013 Paul became Program Chief and Medical Director of Primary Care, Rehab/Complex Continuing Care, and Seniors’ Services. Paul supervises residents at THP’s two Family Medicine Teaching Units, teaches at the Mississauga Academy of Medicine, and is Assistant Professor in the University of Toronto’s Department of Family and Community Medicine. REGIONAL ABORIGINAL CANCER LEAD Dr. Mike Vreugdenhil worked in family practice for four years before joining the Southwest Ontario Aboriginal Health Access Centre (SOAHAC). Through SOAHAC, Mike worked on the Chippewa of the Thames First Nation for over ten years to promote wellness, prevent illness, and provide care for various health concerns. LHIN 3 Waterloo Wellington Regional Cancer Program | WATERLOO WELLINGTON Dr. Rachael Halligan [email protected] REGIONAL PRIMARY CARE LEAD Dr. Rachael Halligan is a general practitioner in Oncology, providing care from diagnosis through to survival or palliation. Rachael received her MD from McMaster University and went to Memorial University for her Family Medicine Residency. LHIN 4 Hamilton Niagara Haldimand Brant Regional Cancer Program | HAMILTON NIAGARA HALDIMAND BRANT Dr. Alexandra Ginty [email protected] REGIONAL PRIMARY CARE LEAD Dr. Alexandra Ginty is a family physician in Oakville. She is an experienced teacher and examiner for McMaster and leads with passion as a cancer survivor, author, blogger and ambassador. In 2013 she published Both Sides – When the Doctor becomes the Cancer Patient. LHIN 7 Toronto Central Regional Cancer Programs | TORONTO CENTRAL Dr. Edward Kucharski [email protected] REGIONAL PRIMARY CARE LEAD REGIONAL PRIMARY CARE LEAD (Toronto Central South) Dr. Daniel Kraftcheck [email protected] REGIONAL PRIMARY CARE LEAD Dr. Danny Kraftcheck has served as Chief of Family Practice and Chief of Staff at St. Joseph’s Hospital Hamilton and President of the College of Physicians and Surgeons of Ontario. Danny is an Associate Clinical Professor at McMaster, Chair Section of Family Medicine Hamilton Academy, and a Peer Leader with Ontario MD. Dr. Andrea East [email protected] REGIONAL ABORIGINAL CANCER LEAD Dr. Andrea East was an Emergency Physician at Brantford General Hospital, and in 1987 began private practice in Brantford and on Six Nations. Since 2005 Andrea has concentrated her clinical practice on Six Nations. Presently Andrea is a member of the Cancer No More Committee to improve The Cancer Journey for First Nations Inuit Métis (FNIM) people in the Hamilton Niagara Haldimand Brant LHIN. Dr. Marla Ash is a family physician in Toronto with the North York Family Health Team. In addition to her family practice and her role as Regional Primary Care Lead, she is involved with clinical teaching. She previously worked for 14 years in the Charlotte and Lewis Steinberg Familial Breast Cancer Clinic at North York General Hospital. LHIN 9 Dr. Edward Kucharski is a family physician at Sherbourne Health Centre, where he served as Medical Director from 2009-2012. Ed is also an Accreditation Canada Site Surveyor. Central East Regional Cancer Program | CENTRAL EAST REGIONAL ABORIGINAL CANCER LEAD Dr. Jason Pennington is a member of the Huron-Wendat First Nation. Clinically, his community general surgery practice is located at the Scarborough Hospital. He is also the Curricular co-Lead in Indigenous Health Education at the University of Toronto’s Faculty of Medicine. RPCL Pending Recruitment LHIN 10 South East Regional Cancer Program | SOUTH EAST Dr. Hugh Langley has been a family physician in Kingston for 27 years. Hugh is also an Adjunct Assistant Professor at Queens University and the Regional Lead for primary and Aboriginal cancer care, and the Ontario Diabetes Strategy. He also works as the Senior Medical Advisor for OHIP in the out-of-country unit. LHIN 11 Champlain Regional Cancer Program | CHAMPLAIN REGIONAL PRIMARY CARE LEAD Dr. Amanda Hey is a family physician having practiced in Sudbury since 1986, and most recently worked at the Shkaqamik-Kwe Health Centre for 14 years. Amanda currently also works at the Sudbury and District Health Unit and is an Assistant Professor at the Northern Ontario School of Medicine. Dr. Patrick Critchley [email protected] REGIONAL PRIMARY CARE LEAD Dr. Patrick Critchley is a family physician and has been working in Timmins for the past ten years. Patrick has a private practice and recently joined the White Pines Health Network, he also works at the Misiway Community Health Centre in Timmins. Patrick’s interests include geriatrics, Aboriginal and palliative care. Prior to moving to Timmins, Patrick worked for 13 years in palliative care in Hamilton, Windsor (Hospice of Windsor) and Halifax. Lee is past-president of the Academy of Medicine Ottawa, and regional representative to the Section of General and Family Practice of the Ontario Medical Association. Dr. Declan Rowan [email protected] REGIONAL PRIMARY CARE LEAD Dr. Declan Rowan is a rural family physician in Petawawa. Declan has worked with Health Quality Ontario as quality improvement champion. Originally from Ireland, he immigrated to Canada as an international medical graduate in 2000, working in PEI prior to moving to Petawawa in 2006. Danusia is also an assistant professor at the Northern Ontario School of Medicine (NOSM) and recipient of a Women’s Health Scholarship and a Master’s of Health Sciences. She is an experienced administrator and facilitator, and an organizational and systemic change agent. Lisa has experience in research and program development across the cancer continuum especially in screening, early detection, survivorship and palliative care. In her current role, she supports primary care awareness and adoption of new initiatives that improve the cancer journey. John is an adjunct professor of family medicine for the Windsor branch of the Schulich School of Medicine. Mississauga Halton Central West Regional Cancer Program | REGIONAL ABORIGINAL CANCER LEAD Dr. Annelind Wakegijig is a family physician presently working as the Lead Physician for the Baawaating Family Health Team on the Batchewana First Nation near Sault Ste. Marie. Annelind is also an Investigating Coroner with the Office of the Chief Coroner of Ontario. LHIN 14 North West Regional Cancer Program | NORTH WEST Dr. Nicole Zavagnin [email protected] REGIONAL PRIMARY CARE LEAD Dr. Nicole Zavagnin worked in many rural Northern Ontario communities before setting up her family medicine practice in Thunder Bay. Nicole earned her BHSc (Honours) at McMaster Universit prior to completing an MD as part of the NOSM’s Charter Class. Nicole is a member of the clinical teaching faculty at NOSM and serves as a primary preceptor for their residency program. Dr. Claudette Chase [email protected] REGIONAL PRIMARY CARE LEAD Dr. Claudette Chase is a comprehensive family physician working in Northwestern Ontario, primarily serving remote First Nations. Claudette was a nurse from 1974, until entering McMaster School of Medicine in 1989. After completing a rural residency at the University of British Columbia, Claudette started working out of Sioux Lookout and has done that since (except for one year spent working in Ottawa). Connie Foster, NP [email protected] Dr. Shannon Wesley [email protected] REGIONAL ABORIGINAL CANCER LEAD REGIONAL ABORIGINAL CANCER LEAD Connie Foster is the Aboriginal Nurse Practitioner and Manager at Moose Deer Point First Nation Health Centre/Nursing station. Connie has also worked intermittently in the Sioux Lookout Zone and provided care for most of the Aboriginal fly-in communities in the region. Connie is currently a Board Member and Treasurer at the Nurse Practitioner-Led Clinic in Huntsville Ontario and is a member for the Aboriginal Health Circle in LHIN 12. Dr. Shannon Wesley currently practices family medicine with the Superior Family Health Organization in Thunder Bay. Shannon is a leader in the medical community for promoting cancer screening among her peers and the greater community, and advocating for and linking First Nation communities and community members with improved access to and education about cancer screening services.
