JOB DESCRIPTION Job Title: Macmillan Penny Brohn Living Well Services Coordinator Date Prepared: October 2014, updated January 2017 Location: Staffordshire (home based) with travel across Staffordshire Main Purpose of Job: To co-ordinate a programme of Living Well services in Staffordshire in line with the requirements of the Macmillan Penny Brohn Pathway of Survivorship Services project plan. To maintain key relationships with NHS partners and other voluntary sector partners and ensure high quality delivery of Penny Brohn UK Services within Staffordshire. Responsible To: Senior Living Well Development Manager (Midlands) RELATIONSHIPS: The post holder is expected to foster excellent working relationships with other members of the Living Well Services Team at Penny Brohn UK who will be supporting the delivery of Living Well services in Staffordshire, the Macmillan Partnership Manager for Staffordshire and the Black Country and all staff in both organisations. Important external relationships will include clinical staff at University Hospital of North Staffordshire and Stafford Hospital, Staffordshire based GPs, and members of staff from other voluntary sector partners. At all times the needs of the client are paramount. MAIN DUTIES: 1. To coordinate the delivery of Living Well services, including Health and Wellbeing events, Living Well courses and Follow Up days to meet the objectives of the Macmillan Penny Brohn Pathway of Survivorship Services project plan. 2. In the case of health and wellbeing events to work closely with the Clinical Nurse Specialists to develop and deliver regular events which meets the needs of different patient groups. 3. To maximise participant recruitment to Living Well Services through regular contact with health and care professionals, networking, giving talks and attending appropriate events and meetings. 4. To network and raise the profile and represent Macmillan and Penny Brohn UK within professional settings. 5. To support the Living Well Development Manager in engaging key stakeholders and running regular Steering Group meetings. 6. To support the Living Well Development Manager in creating opportunities for regional user involvement in shaping the delivery of the Macmillan Penny Brohn Pathway of Survivorship Services. To provide appropriate support to service users who become involved. 1 7. To contribute to the audit and evaluation procedures for internal (administration of courses and training of Facilitators) and external services. 8. To attend and contribute to relevant meetings with the local Macmillan Team and the Living Well Team at Penny Brohn Cancer Care. 9. To work closely with the Living Well team to develop and evaluate services. 10. To manage flexible working arrangements that may include weekend or evening work. 11. To attend relevant training sessions. 12. To undertake other such duties as may become appropriate from time to time in order to achieve the objectives of Macmillan and Penny Brohn UK. PERSON SPECIFICATION: Essential To have previous successful experience of project coordination and an understanding of effective resource management. To be able to independently prioritise workload to meet project milestones. To have excellent communication skills and a proven track record of network building. To have experience of working with people with cancer or a life threatening illness. To be able to demonstrate good presentation skills. To be able to work well in a team. To be dedicated, proactive and flexible in your approach. To be in sympathy with the philosophy and practice of the integrative approach to supportive cancer care. To have an understanding of the survivorship agenda in cancer care. To have a commitment to personal growth and development. To be open to the requirements of working in a voluntary sector organisation. You must have a full UK driving licence and vehicle insured for business use. Desirable To have experience of home working as part of a remote team. An understanding of cancer services in Staffordshire. An understanding of the impact of a cancer diagnosis and treatment for the individual. To have experience of working in community settings and in partnership with other health care providers. Penny Brohn UK operates a system of regular performance reviews, which includes an Annual Appraisal where the job description, progress against objectives and overall performance in post are discussed. FURTHER INFORMATION: The partnership between Macmillan Cancer Support and Penny Brohn UK Macmillan Cancer Support and Penny Brohn UK joined forces in Staffordshire in January 2015 to launch a new programme of support for people affected by cancer which includes half-day Health and Wellbeing events, two day Living Well courses and Follow-up days. 2 The programme is available to patients and their close supporters in North Staffordshire, Stoke on Trent and Stafford. A significant number of people living with cancer feel abandoned once their treatment is over, often left with a wide range of debilitating problems as a consequence of the disease and its treatment. The programme provides patients with the knowledge and tools to help manage the symptoms and side effects of cancer and its’ treatment, to support them to live well after a cancer diagnosis. Specifically, it provides information around healthy eating, the importance of exercise, emotional support and how to manage stress, and also gives participants the opportunity to share their experiences with others who have been through a cancer diagnosis In the case of Health and Wellbeing events, the service is delivered in partnership with clinical staff at University Hospitals of North Midlands NHS Trust in line with national guidance to transform how patient follow-up services are delivered. The project has initially been funded for 3 years and there is 11 months remaining on the current employment contract for the Macmillan Penny Brohn Living Well Services Coordinator, with the possibility of this being extended. As a Macmillan Professional, the Macmillan Penny Brohn Living Well Services Coordinator can access an extensive range of Learning and Development opportunities provided by Macmillan Cancer Support. 3
