Date of commissioning, e.g. Month/Year. To be asked from the end customer if
the commissioning date is unknown!
Type of Frequency Converter Type of the whole frequency converter, e.g. ACxxxxxxxxxx.
Note: For Machinery drives, give the type of the whole unit e.g.ACSM1-01AS-05A0-4.
For Standard drives, give the type of the whole unit e.g.ACS550-01-038A-4.
For Industrial drives, give the type of the whole unit e.g.ACS800-07-0750-7.
With cabinet assembled drives the product type field xx in ACS800-xx should
contain the type of the main unit, e.g. -07, -107, -67, -17, -77, etc.
Serial No
Sales ID / Project No
Type of Module / Option
Serial number of whole frequency converter, e.g. 1020100584 (always 10
digits). If this is not available then the Sales ID must be given.
For cabinet drives enter the sales ID, e.g. 27391. For P&P** projects, enter the
project number e.g. 1234XF.

Description of Fault
Choose the fault which best describes the problem. If none of the given choices is good enough, please
choose Other fault, and describe the fault in the More description of the fault and fault history info field.
More description of the fault and fault history info
More details about the fault and behaviour. E.g. Is the fault occasional or is it active all the time?
Description of service
A detailed description of the service done to make the system/part functional. Reason for changing the
parts. Please also describe the outcome of the repair.
Has the drive been changed or repaired before?
Has the unit been repaired before? This can be asked from the customer!
Software version of the drive/unit
See version information from the Control Panel / PC-tool.
How long had the drive been running?
Check the total running time (in hours, if possible) from the Control Panel / PC-tool in the actual value group.
Environmental condition
If the installation environment is really bad, the drive should not be repaired under warranty.
Electrical room
Is the unit placed in an electrical room or is it somewhere in the plant?
Estimate how clean the environment is
Dirty:
Lots of dust inside the unit/room. Boards and components are covered by dirt.
Medium: Only some dust can be found on boards, components or inside the unit/room.
Clean:
No dust inside the drive/room.
Air conditioning with cooling
Is an air conditioning unit installed to keep the temperature of the room within the specified limits?
Estimate the risk level of corrosive gas, moistness and conductive materials for the drive
High:
Marks of corrosion, moistness or conductive materials are found easily.
Medium: Only few marks of corrosion, moisture on conductive materials can be found.
Low:
No marks can be found.
Broken components
Parts or components changed to make the system functional.
* SD = single drive, MD = multidrive
** P&P = Pulp & Paper
