JOB PURPOSE To funon as a lead member of the multi-disciplinary team by co-ordinating and providing a high quality and effective specialist nursing service for patients with skin cancers throughout the course of their illness, integrating both acute and palliative care. To provide professional and clinical leadership in the speciality of skin cancers across NHS Grampian, and act as a recourse of clinical expertise for nursing and medical colleagues. The individual is expected to motivate staff to provide high standards of care by acting as a role model. To be responsible for providing specialist education and training programmes for health care professionals. To ensure practice if clinically effective and informed in accordance with the NMC code of Professional Conduct. To implement and undertake research and audit activities to ensure evidence based practice in the specialist area. 3. SCOPE AND RANGE The postholder has responsibility for the provision of care for newly diagnosed and existing skin cancer patients in NHS Grampian under the supervision of Dermatology and Plastic surgery consultants. In Grampian, approximately 100 new cases of malignant melanoma are diagnosed per year. The number of non-melanoma skin cancer cases in Grampian is approximately 800 – 1000 per year. The postholder has direct responsibility for developing and implementing nurse led services within the overall service provision for patients with skin cancers. 5. MAIN DUTIES/RESPONSIBILITIES Advanced Nursing Practice: Improves the process of care for patients with skin cancer throughout their pathway. Applies specialist knowledge, competencies and clinical decision making skills to enhance patient care. Assesses, diagnoses and manages complex problems with agreed protocols. Leads and acts as a specialist resource in skin cancer nursing by promoting the service and increasing the awareness of the cancer in acute and community to enhance standards of care. Responsible for ensuring that the quality of care is reviewed, assessed, implemented and monitored to maintain standards of care. Enables patients to take responsibility for their health, wellbeing and future lifestyle by practising in an open, transparent and inclusive manner, thereby ensuring patients have the relevant information to participate in decisions about their care. Leadership and Management Acts as an exemplary professional role model for leading skin cancer nursing services. Take a lead role in the development and implementation of guidelines and protocols for skin cancer are to promote consistency in delivery of evidence based practice. Provide nursing expertise to advise on reviewing and enhancing skin cancer nursing services, were concerns exist about quality. Direct and develop, in conjunction with others, the future service provision and planning for skin cancers. Education Lead on the assessment of training and education needs relating to care of patients with skin cancers, and facilitate the development of educational provision to ensure that nurses are professionally competent to deliver care. Develop, deliver and contribute to multi-disciplinary education and training programmes to promote a wider understanding of skin cancers in both acute and community settings. Teach, advise and patients about the condition, treatment options and management of symptoms and treatment side effects. Promote and advise on health and lifestyle activities to enhance health and well-being. 3 Research, audit and evaluation Lead clinical audit of skin cancer nursing services to inform practice, and service development and redesign. Act as a clinical collaborator for research programmes to support best practice which leads to continuous improvements in care. Critically analyse the relevance of appropriate research related to practice and encourage dissemination and utilisation at a local level. Evaluate effectiveness of role in relation to patient outcomes, service needs and professional requirements. In collaboration with colleagues, implement actins arsing from NHS Quality Improvement Scotland, cancer reviews and cancer guidelines. 6. SYSTEMS AND EQUIPMENT Computer and Microsoft packages – email, letter writing documents. Specialist database – collect and input patient data which allows postholder to determine workload and activity. Powerpoint presentation for teaching purposes. Intranet/internet – access to local and national NHS sites, government sites, and nursing and medical databases to continually develop evidence based practice Videolinking. Laboratory and radiology – systems – access to clinical results Telephone & mailbox – answering machine, fax, bleep, photocopier. Medical devices, various infusion systems 7. DECISIONS AND JUDGEMENTS The postholder is expected to make clinical and professional autonomous decisions on a daily basis, The postholder is expected to work closely with dermatology and plastic surgery consultants to provide counselling, education and certain types of treatments within the multi-disciplinary team. Informing clinical decision making with regard to patient care, through monitoring of the patients’ condition and acting on clinical judgements with active and continuous liaison with respective consultants. Assesses, plans, implements and evaluates individual complex needs with the provision of practical, informational and emotional support and symptom control for patients and their carers at all stages of the skin cancer pathway from pre-diagnosis, through diagnosis and treatment, to palliative care, end of life care and into bereavement. 8.
