JOB DESCRIPTION JOB TITLE: Clinical Glaucoma Fellow DEPARTMENT: Glaucoma GRADE: Fellow RESPONSIBLE TO: Mr Nick Strouthidis, Service Director, Glaucoma Service ORGANISATIONAL CONTEXT Moorfields is one of the world’s leading eye hospitals, providing expertise in clinical care, research and education. We have provided excellence in eye care for more than 200 years and we continue to be at the forefront of new breakthroughs and developments. We are an integral part of one of the UK’s first academic health science centres, UCL Partners, and recently celebrated five years as one of the country’s first NHS foundation trusts. Our main focus is the treatment and care of NHS patients with a wide range of eye problems, from common complaints to rare conditions which require treatments not available anywhere else in the UK. The unique case-mix and volume of our larger services means that our surgeons can develop expertise in different areas of the sub-specialty. Our 1,800 staff treat and provide support for patients at our main hospital base in London’s City Road and at 20 other sites in and around the capital, which enables us to provide expert treatment closer to patients’ homes. As a measure of our success, more patients that ever before chose us for their ophthalmic health care and in 2010/11, we recorded almost 500,000 outpatient attendances in our clinics and 74,000 attendances at our specialist A&E department. We carried out nearly 30,000 inpatient and day-case procedures, more than 8,500 laser treatments and around 10,000 treatment procedures for age-related macular degeneration. We continue to improve the quality of our services and me the key operational targets that matter to our patients, referrers, and commissioners. We delivered a financial surplus of £10 million, maintaining our long-term viability, while the organisation continues to evolve to meet the changing healthcare environment and the challenging economic climate Moorfields has contracts with more than 80 primary care trusts across the country, making us a truly national centre for eye care. We are also a major postgraduate teaching centre and a national centre for ophthalmic research involving, with the UCL Institute of Ophthalmology, the largest ophthalmic research programme in the world. We are one of only 11 sites nationally to be awarded National Institute for Health Research biomedical research centre status for translational research. This has recently been renewed for another r5 years with a substantial increase in research funding. We have recently completed the development for our 10 year clinical strategy – Our Vision of Excellence, which continues to develop our networked model of service delivery for hospital and community based care. Moorfields directly manages three commercial divisions: Moorfields Private and Moorfields Eye Hospital Dubai. Established in April 2007, the Centre is funded by the Department of Health via the National Institute for Health Research. The objective of the Centre is to conduct ‘translational research’ that is designed to take advances in basic medical research from the laboratory to the clinic enabling patients to benefit more quickly from new scientific breakthroughs. The research of the Centre falls within six major “themes”: Age-related Macular Degeneration; Diabetes; Glaucoma; Ocular Repair; Regeneration & Pharmaceutics; Ocular Surface Disease; Paediatric Ophthalmology and Inherited Eye Disease. Ophthalmology at Bedford Hospital Moorfields Eye Hospital provides comprehensive ophthalmic services at Bedford Hospital under an agreement with Bedford Hospital NHS Trust. These services are provided in dedicated ophthalmic outpatient and operating facilities. The catchment population is drawn largely from Bedfordshire Primary Care Trust which has a population of approximately 300,000. The outpatient facilities are modern and well-equipped, including Argon, YAG and Diode Lasers, Digital Lasers, Digital Anterior and Posterior Segment Photography, Digital Fluorescein Angiography, 3 D OCT Scanner, a Video-Indirect Ophthalmoscope, a Heidelberg Optic Disc Analyser as well as Visual Field and modern Biometry equipment. There is a well-equipped ophthalmic theatre staffed by trained ophthalmic theatre staff and 8 major operating lists are run weekly. The Post The Glaucoma Service welcomes a Fellow from outside the North Thames Ophthalmology Training Scheme who wishes to improve their knowledge and skills in Glaucoma management. Applicants should still be in an Ophthalmology training post or have recently completed their higher specialist training programme in Ophthalmology. The Clinical Fellows rotate six-monthly allowing Fellows to work with a number of the consultants during their stay. All rotations will involve placements in at least one of Moorfields’ outreach sites, including surgical sessions. Some rotations over the two years will incorporate Saturday clinics and evening clinics (at any site) as part of the regular timetable. Fellows rostered to Saturday sessions at City Road must also be available to review emergency cases from eye casualty during clinic hours. Fellows on Moorfields South rotations (St George’s and Croydon) may take part in the general on call rota for Moorfields South. This will be in lieu of a Saturday clinic. The rotation will provide experience in glaucoma and its subspecialties, including paediatric glaucoma. The post will commence as soon as possible, for a fixed-term period of 2 years. All posts are eligible for recognition for training by the Royal College of Ophthalmologists. The successful candidate will need to arrange the necessary approvals prior to taking up the post. Applicants from outside the EU can apply and may be eligible for full GMC registration and a work visa through the Royal College’s Dual Sponsorship scheme. The Glaucoma Service Medical Staff include 12 Clinical Fellows, three North Thames ASTO/TSCs, 5 Specialist Registrars/Specialty Trainees sub-specialising in Glaucoma, a number of Research Fellows, Associate Specialists and Clinical Assistants. The service sees approximately 6,000 new patients per year and 60,000 follow-up patients. Approximately 4,000 operations are performed each year. Programme Fellows will attend general and specialist Glaucoma Clinics at Moorfields Eye Hospital, City Road and at outreach sites. Two operating sessions are allocated. Teaching sessions are held for all members of the Glaucoma Service on Wednesday mornings in the Clinical Tutorial Centre at Moorfields City Road. Specialist interest clinics in Glaucoma include Uveitic Glaucoma, Paediatric Glaucoma, Angle Closure Glaucoma and Low Tension Glaucoma. The department has an interest in Epidemiology. There will be opportunities for research during the course of the fellowship and one session weekly will be set-aside for this purpose; however the duties of the posts are predominantly clinical. Teaching The post will have a teaching role, contributing to the current service teaching programmes of the A&E and Glaucoma Services and the on-going assessment and teaching of the clinical and surgical skills of the ST’s and Fellows in training. Research at Moorfields Eye Hospital The department has an international reputation for excellence in research in a wide variety of glaucoma topics. Areas of research expertise include imaging, psychophysics, wound healing, epidemiology, angle closure and clinical trials. Clinical research is undertaken within the NIHR Biomedical Research Centre at Moorfields Eye Hospital NHS Foundation Trust and UCL Institute of Ophthalmology. Research Intellectual Policy Moorfields is an important research institution and as a result the Trust has a policy conveying intellectual property, which will apply to the successful applicant. The policy can be found in the Medical Staff Handbook or copies can be obtained from the HR Department. Study and Training Study leave is granted prospectively by the Study Leave Sub-Committee, following Consultant approval and subject to agreement by the Clinical Management Board. The Committee meets once a month and applications must be received at least two months in advance of the proposed leave. Study leave application forms are available on the Trusts’ Intranet page. A minimum of two months’ notice is required as for study leave to permit adjustment of any clinical research projects or planned clinical work. The Clinical Tutorial Complex, with well-equipped Seminar Rooms and a large modern lecture theatre, provides the principal location for courses and lectures organised by the Post Graduate Medical Education Centre. There is also a skill laboratory, which provides microscopes and other equipment for surgical training. Annual Leave Entitlement 32 days per annum, by agreement with your Consultant supervisors, the Fellow must ensure that adequate cover is available when taking leave and inform the relevant service timetable administrator, typically the Senior Service Fellow and administrative staff. Clinic Clerks must be given at least two months’ notice to adjust clinic profiles. The Fellow shall not be away when their Consultant is away unless appropriate cover has been agreed and obtained. Sickness One of the Senior Residents and the HR Department must be notified as soon as possible on the first day of sick leave. You are required to provide a self-certificate on the fourth day of any period of sickness. If this continues a medical certificate must be obtained from the seventh day of absence, including weekends and holidays. Working Emergencies and Unforeseen Circumstances Fellows are expected to perform duties in clinical emergencies and unforeseen circumstances at the behest of their Service Consultant Staff. Salary Scale The salary for this post is currently £40,000 + £10,000 per annum. Conditions of Appointment Possession of the Membership of the Royal College of Ophthalmologists or equivalent is essential. Applicants should have completed their higher surgical training in Ophthalmology or be close to doing so. Possession of a Higher Degree, while not essential, would be an advantage. The successful candidate must be registered with the General Medical Council (with a Licence to Practise), be an appropriately subscribed member of a recognised Medical Defence organisation and be eligible to work in the UK (documentary evidence will be required). Revalidation requirements All doctors are responsible to maintain a portfolio of their career over the last 5 years in order to satisfy GMC Revalidation criteria. If successful, you will be required to bring along your supporting information portfolio including recent appraisals documents when joining the Trust. Doctors holding joint and/or various fixed term appointments throughout their appraisal year are also required to demonstrate evidence of an exit report and supporting information from the different appointments they hold throughout their appraisal year. This information must be brought with them when starting a new appointment within The Trust. It is recommended that doctors collect this information before leaving their previous employers. Main Conditions of Service The Trust follows the majority of Terms and Conditions as described by the Medical and Dental Staffs Whitley Council with the exception of the General Staffs Whitley Council for the Health Service, pay protection and salary or by such agreement as may from time to time be amended by the Trust Board. There are a number of other Trust policies which apply to your appointment, including Confidentiality, No Smoking, Intellectual property etc. Copies are available from the Human Resources Department and can be found in the Policies and Procedures Handbook. GENERAL DUTIES 1. All staff are required to implement infection control policies and practices, including hand hygiene, waste disposal, staff uniform and occupational health responsibilities, as detailed in the trust intranet. 15. It is the responsibility of all staff to ensure that they have evidence of annual/or otherwise infection control training. 16. All staff are responsible for ensuring that equipment used in the patient environment is cleaned, decontaminated and maintained in line with trust policy. 17.
