Two fellows work with me at any one given time. One of these fellows is split between GOSH and King’s College Hospital, London. Royal College of Ophthalmologists Tutor NSCAG Ophthalmologist The National Specialist Commissioning Advisory Group (NSCAG) aims to help patients by improving access to uncommon services, whilst at the same time seeking to sustain high levels of expertise by preventing proliferation to too many centres. In addition to my normal workload, I am funded by NSCAG to deliver Ophthalmic care and service strategies to all patients at Great Ormond St Hospital for Children with: 1 Craniofacial Anomalies 2 Epidermolysis Bullosa Service Developments Since commencing my post in January 1999 I have introduced eleven main service developments 1 Penetrating and lamellar keratoplasty service for appropriate neonates , infants and children 2 Ultrasound service : high frequency ultrasound, posterior segment ultrasound and biometry for intraocular implant calculations 3 Management protocol for periocular haemangiomas 5 4 5 6 7 8 9 10 11 Management protocol for Visual function surveillance in Craniosynostoses As a result of demonstrating to the National Specialist Commissioning Advisory Group (NSCAG) the effect of this new management protocol, we received a centrally funded grant of 47000 sterling pounds on a recurrent annual basis in June 2002. Botulinum Toxin for paediatric strabismus Ophthalmic Surveillance for children with Epidermolysis Bullosa As a result of demonstrating improved quality of life with decreased corneal erosions NSCAG provided 2 Consultant Sessions of funding for this service starting April 2004 Eyemovement service: in 2009 I recruited a doctor to set this service up.IN 2010 clinics were set up to provide this service In 2009 a clinical nurse specialist post was developed to allow nurse led clerking of admissions and hence reduced dependency on inexperienced junior doctors In 2009 A joint paediaric neuroophthalmology post was created between GOSH and Whipps Cross hospitals and a candidate successfully appointed In 2010 a second anterior segment surgeon post was created and a successful candidate appointed full time at Great Ormond Street Hospital for Children In 2009 the ‘low vision aid service’ was developed with Lynne Speedwell and a candidate successfully appointed to run this service Although my two colleagues did treat paediatric glaucoma before my arrival, this has been part of my responsibilities since my arrival. I have introduced releasable sutures for paediatric trabeculectomy, Ahmed tube implant (which I learned about while fellow at HSC, Toronto) and high frequency ultrasound guidance for cycloablation. I have gained 7 Clinical Excellence Awards because of my contribution to the National Health Service both at Local And National Levels RESEARCH INTERESTS 1 Craniosynostoses • Extraocular muscles o Structure o Genotype-phenotype correlation o Strabismus Evaluation Treatment • Visual Loss o Amblyopia o Optic neuropathy Causes Treatment 2 Ultrasound 6 • • 3 Normal Ultrasound o Linear Use in biometry and lens diameter estimation o Doppler Vascular anomalies High Frequency Ultrasound o Diagnosis in congenital corneal opacification o Therapeutic use for Cyclodiode laser o Corneal thickness measurement Paediatric Anterior Segment • Cataract o with and without implantation • Keratoplasty o Penetrating Congenital corneal developmental opacification Acquired o Deep Anterior Lamellar Keratoplasty • Amniotic Membrane Transplant o Epidermolysis bullosa • Glaucoma o Goniotomy o Trabeculotomy with Trabeculectomy (MMC and releasable sutures) o UBM guided Cyclodiode laser o Ahmed Tube RESEARCH GRANTS AND FUNDS 1 JS Rahi, IM Russell-Eggitt, KK Nischal, DSI Taylor, CS Peckham Outcomes of congenital and infantile cataract in the United Kingdom and their predictors – national study in collaboration with the British Congenital Cataract Interest Group Guide Dogs for the Blind Association Project Grant. 2001 £94,552 2 KK Nischal Ocular aspects of Craniofacial Disorders National Specialist Commissioning Advisory Group (NSCAG) 2002 Recurring annual funding £64,000 3 KK Nischal 7 National Specialist Commissioning Advisory Group (NSCAG) 2004 Recurrent annual funding £27,000 4 KK Nischal, A Martinez, J Mellerio Ocular phenotype in Epidermolysis Bullosa Dystrophic Epidermolysis Bullosa Research Association 2005 £47,000 5 KK Nischal, C Tuleu, O Rabiu, R Sekhri, W Van’t Hoff Development of cysteamine gel for patients with cystinosis Cystinosis Research Network USA 2006 AWARDS, PRIZES £103,000 1. 1995 Royal Eye Hospital, London Poster Prize at the Annual Congress of the Royal College of Ophthalmologists 2. 1997 Keeler Scholarship 1997- 1998 3. 2005 World Cornea Congress V. There is a fellowship education program which involves orthoptics, electrophysiology and my colleagues. There are fellows’ nights for informal discussion about one subject, regular journal clubs and Socratic style teaching seminars with weekly grand rounds. Teaching is also done for paediatricians and for nursing staff ORBIS I have been privileged to be involved with ORBIS since December 2003 I found teaching the local faculty extremely challenging and very satisfying. I am committed to the vision of Dr Helveston as regards telemedicine and its use as an educator. December 2004 Hospital Based Programme in Kunming, China. Although I do not speak any African languages I have developed a great enthusiasm for African music especially from Mali, Senegal, the Ivory Coast and South Africa. I also enjoy a game of Charom (oriental game similar to billiards but played with discs on a specially constructed board).
