POSITION DESCRIPTION Position Title: Senior Pharmacist – General Medicine Award Classification: Grade 3 Award / Agreement Name: Medical Scientists, Pharmacists and Psychologists (Public Sector - Victoria) Award Current Effective Date: February 2017 Next Review Date: February 2019 Reports to: Associate Director of Pharmacy – Maroondah Hospital 1. POSITION SUMMARY The Senior Pharmacist- General Medicine will be responsible for the direction, planning, provision, and review of clinical pharmacy services to the General Medicine Service across Eastern Health. Page 1 of 7 21/10/2016 Based from Maroondah Hospital, the post-holder will maintain a clinical load within the general medicine directorate, providing a high level of service to patients and staff, whilst managing staff at the site working in the specialty. In addition, the post-holder will work in collaboration with the Senior Pharmacist- Education and Training, to deliver clinical education and will be responsible for overseeing the mentoring and development of rotational clinical pharmacists (including residents and fellows) and interns working within the General Medical Services across Eastern Health. Research and quality improvement are an essential part of pharmacy services at Eastern Health. This position would also be expected to contribute to the research and educational activities of the General Medicine teams as well as pursue their own research. As part of the senior pharmacy management team at Maroondah Hospital, the role also includes involvement in all aspects of Maroondah Hospital Pharmacy services. 3. MAJOR DUTIES AND/OR RESPONSIBILITIES The following list of activities is to be read as a guide to position responsibilities. It is not intended to be exhaustive. Act as specialist practitioner and role model in General Medicine as well as broader clinical pharmacy practice Co-ordinate, develop and monitor the activities of the clinical pharmacy service to the General Medicine Service Provide support for the General Medicine service including provision of advice, medicine information and education sessions Provide advice and implement strategies to improve the cost-effective and quality use of medicines (QUM) within the specialty Provide pharmacy input at multidisciplinary team reviews Ensure relevant pharmacy staff compliance with pharmacy and hospital procedures relating to their responsibilities including but not limited to Drug Formulary and Medication Guidelines Oversee reporting and review of medication related clinical incidents for areas covered within the position portfolio Demonstrate an ongoing commitment to and participate in Quality Improvement activities, including the development and reporting of key performance indicators. Develop and contribute to practice based research and service improvement projects within the position portfolio Actively maintain and extend clinical knowledge by participating in continuing education, seminars and conferences Encourage and foster a culture of enquiry, research, continuous improvement and life-long learning Contribute to the strategic plan for pharmacy services which addresses hospital priorities, emerging pharmaceutical trends, technological change, and patient care objectives. Lead, support and mentor pharmacists, residents, fellows, interns and students in the General Medicine Service Conduct performance appraisals, ClinCat assessments and other competency based assessments for staff within the specialty Identify training and educational requirements and priorities in conjunction with the Senior Pharmacist- Education and Training In collaboration with the Senior Pharmacist- Education and Training, participate in the organisation, development and/or delivery of department education programs for medical, nursing, pharmacists, pharmacy students, pharmacy intern, pharmacy technicians and consumers Representation of pharmacy and/or the health service on relevant committees & working groups as required Develop in conjunction with other Eastern Health stakeholders relevant local guidelines including standard operating procedures. Regularly communicate with staff and Department on matters relating to respective division Page 2 of 7 21/10/2016 Rotate to other sites as deemed appropriate Participate in the weekend, public holiday and on-call rosters (post credentialing) at Maroondah Hospital Participate in activities deemed appropriate by the Director of Pharmacy or Associate Directors of Pharmacy Contribute to the overall management of the Pharmacy through active participation as a member of senior management. Key results areas: Clinical pharmacy and pharmacy education services for General Medicine are conducted in accordance with all relevant legislation, standards and guidelines, including APAC, ACHS and ACSQHC Incidents are reviewed within identified time frames and actions implemented Participation in the pharmacist intern tutorial program Preparation and/or oversight of one SHPA conference poster or presentation per year (or equivalent) Annual performance appraisals and SHPA ClinCAT assessments for clinical pharmacists and interns rostered in the area Oversee the general medicine clinical pharmacy performance board Provision of accurate and timely information for management purposes to ensure that the systematic provision of services is achieved Provision of an end of month report to the Associate Director of Pharmacy Participation in relevant Eastern Health meeting and committee structures Ongoing CPD Maintained and documented at a level applicable to the post. This post is one of continual development. Responsibilities will be reviewed periodically in line with service priorities and duties may change or new duties be introduced after consultation with the post holder. 4. PERFORMANCE DEVELOPMENT A Performance Review, that includes agreed targets, will occur six (6) months from commencement and then annually on the basis of the duties and responsibilities outlined in this position description. This is an opportunity to review personal and the allocated work unit’s service performance, facilitated by the setting of objectives/goals and ongoing evaluation of performance and achievement. Objectives will be developed annually, documented, discussed and agreed with the immediate line manager, who will act as the assessor.
