Purpose
Radiology information systems are currently inseparaable part of department
management playing significant role in scheduling, billing, quality management, report
repository and resource handling. Since a typical diagnostic radiology service consists
of an imaging procedure ending in a collection of images and a report, no clinical and
follow up information is captured.
Management of interventional radiology department is considerably different from its
diagnostic equivaent and no regular RIS is capable of handling data management in an
interventional radiology suite.The nature of interventional radiology services mandates
inclusion of additional features in information systems.
The purpose of this paper is to introduce the idea behind development of a specific type
of RIS which is customized to the needs of interventional radiologists.

Methods and Materials
Sample routine procedures performed in a non-vascular interventional radiology suite
were analyzed according to the workflow:
1- Endovascular Laser Therapy of Varices
2- Radiofrequency Ablation of Hepatic Focal Lesions
3- Radiofraquency Ablation of Osteoid Osteoma
4- PRT Spine Injection
5- Vertebroplasty
6- Perutaneous Billiary Drainage
7- Nephrostomy
8- Intra-abdominal Intra-Pelvic Abscess Drainage
Type, frequency and priority of data production, storage and summarization were
formulated for each procedure.
A modified RIS was developed to let radiologist keep not only the admission, reporting
and financial information but also the data needed for monitoring and follow up of patients.
Results
IRIS (Interventional Radiology Information System) is a database management software
which in addition to ordinary patient record and billing, cheduling functions, allows
interventional radiologist to keep record of
1- Pre-intervention Diagnostic Tests
It is possible to capture para-clinical, imaging and clinical information into patient record.
All laboratory tests required for diagnsis and base-line measures for comparison with
post-intervention findings are kept in the system as well as laboratory tests information
required for pre-intervention evaluation like bleeding tendncy. Imaging findings are kept
likewise. Clinical examination or history findings are included too. For example patient

pain scale in spine intervention as well as varicose vein distribution pattern in EVLT
procedures are defined in the system.
2-Detailed Procedure Information
Interventional radiologists need to keep record of the procedure in detail. It will help them
prepare an interventional procedure report for the patient and insurance companies. At
the same time these details can be used for multi-step or repeated procedures in order to
change parameters to acheive a better result. So, not only planning information is kept,
but also characteristics of euipment, devices as well as physical and chemical parameters
of the intervention (like RF ablation parameters or LASER interventions) are recorded.
3-Periodical Post-intervention Follow up Information
Each interventional procedure has its own follow up requirements inluding clinical and
para-clinical assessments. The system is capable of providing appropriate warnings in
the follow up period. So, the radiologists benefits from followup automation and record
keeping for better patient management and research projects.
4-Exporting Data for Statistical Analysis and Research Purposes
Structured nature of captured information and inter-related data including imaging, clincial
and para-clinical findings paves the road for export of complex query tables for statistical
analysis. So, the system lets interventional radiologist to easily transfer the medical
records into research projects.
Conclusion
Customized RIS may help interventional radiologist improve the management of his/her
department and raise the quality of interventional procedures.
Despite diagnostic radiology information systems, IRIS needs multi-step information
management and cutomized data-entry forms for each intervention. Work-flow based
nature of IRIS helps radiologist and his/her assisstant make benefit of the system as far
as the intervention proceeds in each step.
Intervetional RIS needs to be able to capture and handle image files (including DICOM
and light images) that makes is far beyond ordinary RIS.
