1. Opens, dispenses, and transfers items to the sterile field by methods
that maintain the sterility and integrity of the item and the sterile
field.
2. Recognizes that the sterility of an item is event-related and depends
on maintenance of the integrity of the package.
3. Inspects sterile items for processing, packaging, and package integrity
immediately before presentation to the sterile field including
a. checking the expiration date (if provided) and not using the item
after the labeled expiration date; and
b. inspecting the sterilization chemical indicator in the package to verify the correct color change for the sterilization process used.
4. Delivers items to the sterile field in a manner that prevents unsterile
objects or unscrubbed team members from leaning or reaching over the
sterile field.
5. Presents heavy items directly to scrub person, places them securely
on the sterile field, or opens them on a separate clean, dry surface.
6. Opens wrapped supplies by opening the farthest wrapper flap first,
then each of the side flaps, and the nearest wrapper flap last.
7. Secures wrapper edges when supplies are opened and presented to the
scrubbed team member or sterile field.

8. Visually inspects instrument tray wrappers for moisture and integrity
before contents are placed on the sterile field.
9. Presents peel pouches to the scrubbed team member or opens items
onto the sterile field by pulling back the flaps without touching the
inside of the package or allowing the contents to slide over the
unsterile edges of the package.
10. Inspects and opens rigid sterilization containers on a clean, flat, dry
surface by
a. verifying that the external locks, latch filters, valves, and tamperevident devices are intact before opening;
b. verifying that the external chemical indicator has changed as to
indicate that the item has gone through the sterilization process
before opening;
c. opening the rigid sterilization container according to the
manufacturer’s written instructions for use and by
1) lifting the lid of the container up and away from the container
and toward the person opening the container;
2) inspecting the lid of the container for integrity of the filter or
valve and integrity of the filter or valve and the gasket; and
3) considering the container contents contaminated if the filter is
damp or dislodged, or has holes, tears, or punctures;
d. avoiding contacting the unsterile surfaces of the table or container
while lifting the inner basket(s) out of and above the container by
1) examining the internal chemical indicator for the appropriate
color change and
2) inspecting the inside of the container for debris contamination or
damage before instruments are placed on the sterile field.

e. inspecting medications and solutions visually immediately before
transfer to the sterile field and not using if the expiration date has
passed or if there is any indication that the medication or solution
has been compromised (eg, discoloration, particulate formation);
f. using sterile transfer devices (eg, sterile vial spike, filter straw, plastic catheter) to transfer medications or solutions to the sterile field;
g. dispensing the entire contents of the container slowly into a
receptacle that is placed near the sterile table’s edge or held by a
scrubbed team member and labeled immediately;
h. considering the edge of the container contaminated after the
contents have been poured;
i. not replacing the cap on opened medication or solution
containers, and discarding any remaining fluids;
j. dispensing medications and solutions to the sterile field as close
as possible to the time they will be used;
k. not removing stoppers from vials for the purpose of pouring
medications unless specifically designated for removal and
pouring by the manufacturer; and
l. discarding unused, opened irrigation or IV solutions at the end of
the operative or other ivasive procedure.
11. Verbalizes a review of facility or health care organization policies
and procedures related to opening, dispensing, and transferring sterile
items.
12. Participates in quality assurance and performance improvement programs
related to opening, dispensing, and transferring sterile items as assigned.

