Contact information for your referring doctor, if you have one ! Insurance information, insurance cards or Medicare card ! Photo ID card ! Calendar to keep track of appointments ! Items to help pass time (books, crossword puzzles, computer) ! Assistive devices: canes, walkers, crutches, etc. When You Arrive Parking and Wheelchair Assistance Plan to arrive 30 minutes before your appointment time. • • • Free Parking – Lee Street garage – 11th Street garage – Registration will validate your ticket Valet Parking – Cost $6 (cash only) – Monday – Saturday, 7 a.m. - 7 p.m. – Free for handicapped or disabled veteran license plates, tags, or placards. Wheelchair assistance is provided by the front door team. Registration You will register on the 1st floor for most of your appointments. Remember to bring: • Photo ID • Insurance cards • Prescription cards • Co-pays • Parking ticket for validation • Social Security Number Patient Experience Program (PEP) Badge You will receive a badge at Registration. The badge is used to improve your experience at UVA Cancer Center. It helps: • Your care providers to find you • Reduce your wait times • Improve communication with your care team Return your badge to Registration after your last appointment. If so, what type of cancer do I have? • What is the stage or extent of my disease? • What treatment do you recommend? • What is the goal of treatment? • What are the possible risks or side effects? • How will you know my treatment is working? MyChart® – Access Your Medical Record Online Your health record online or on the go. Cancer Center Amenities Café Located on the 1st floor, the small café offers a variety of healthy options: • • • • • Hot breakfast items Soup, salads, and sandwiches Snacks Beverages Free Fruit Infused Water They accept all major credit cards and cash payment options. Hours: 7:30 a.m. – 2:30 p.m. There is a full-service cafeteria across the street in the main hospital. Flourish Positive Image Boutique The boutique offers products to help with your treatment side effects. Items include: • • • • • Skin care products Wigs Scarves Jewelry Other gifts Patient & Caregiver Learning Resource Center Located on the 3rd floor, the Learning Resource Center offers: • Access to computer, printer, Internet, phone • Free print and electronic materials about diagnosis, treatment, caregiving and more! • Research assistance from a librarian • Help signing up for MyChart • Office hours to get information and help from our Health Educator Cancer Center Amenities Quiet Spaces • Meditation Room on the 1st floor • Tranquility Garden at the corner of Lee Street and Jefferson Park Avenue Patient & Caregiver Support Services UVA Cancer Center Support Services team helps you and your caregivers through your journey with cancer. Most services are located on the 1st floor. A member of the team may also meet with you in the clinic. Most of the services are free to patients and caregivers. American Cancer Society Patient Navigator In partnership with the American Cancer Society, UVA Cancer Center offers free support from an ACS Patient Navigator. She helps cancer patients and caregivers with the practical concerns of diagnosis and treatment. Practical Concerns - Will I have a hard time getting to and from treatment? - Am I worried about paying medical bills? - Am I worried about paying for medications? - Do I need lodging information? - Do I want to connect with others in a community support group? - Do I need to know about hair loss resources? - Do I want to talk about how my diagnosis impacts my work? - As a caregiver, do I want to talk about how the diagnosis of my loved one impacts my work? American Cancer Society Patient Navigator Free Resources • American Cancer Society programs and services • Resources in your community • Other reliable and relevant national organizations Meet with the ACS Patient Navigator • During a clinic or treatment appointment • In a separate visit in the Support Services clinic • Over the phone You do not need an appointment. It can be helpful to make an appointment to make sure you have plenty of time to discuss your concerns and problem solve together. Social Workers A cancer diagnosis can bring emotional and psychological distress. Our Clinical Social Workers help with adjusting to the illness, anxiety, and depression. They can help with difficult family or social situations. They draw from your strengths to find solutions, increase coping skills, and seek out resources. Their services are free. Services Include • Individual, family and group counseling and support • Advance Directive guidance and support. An Advance Directive informs healthcare professionals of your wishes for medical care in the event that you are unable to. Everyone over the age of 18 should have an Advance Directive. • Support groups Health Educator Educational Resources There is a lot of information available on the Internet. It is important to get your information from reliable sources to ensure it is accurate, easy to understand and up-to-date. Events may include: • • • • • ACS Look Good Feel Better Program Art Therapy Caregiver Café Educational Classes Support Groups Financial Coordinators We may be able to help: • Enroll you in a drug assistance program to reduce your co-pay for certain medications. • Combine bills and arrange payment plans. • Review and explain your insurance benefits. • Work with you to get a cost estimate for the services you need if insurance does not cover them. • Help you apply for financial assistance. This service is free. Their offices are in the Registration area, 1st floor. Flourish Positive Image Boutique Our certified mastectomy fitter provides free fittings for • • • • Bras Prostheses Camisoles Wigs Flourish is also home to Ramon’s Room, which provides free wigs, scarves, and caps. Nutrition Counseling Good nutrition is an important part of your cancer treatment. It can help reduce side effects and speed up your recovery. Our dietitian offers free services that can help you with: • • • • Weight gain or weight loss Loss of appetite Taste changes Meal planning Meeting with the dietitian as early as possible into treatment is ideal. She can see you at your clinic or treatment visits or meet one-on-one with you. Tell your healthcare provider you want to meet with the dietitian or call to make an appointment. Psychologist Our Clinical Psychologists help you and your loved ones adjust to cancer and cancer treatments. They can clinically assess and treat: • • • • • Addictions Anxiety Depression Eating disorders Fatigue • • • • Insomnia Pain Sexual difficulties Other psychological disorders They provide psychotherapy for individuals, groups, and/or couples and families. They can also work with Psychiatrists to help facilitate the use of medications as needed. There may be a co-pay required for this service. Spiritual Care and Guidance The chaplain is available to help support you and your caregiver during this stress-filled time. Her services are free and include: • Listening to you • Helping you reflect on your own emotional or spiritual concerns • Provide support whether you are part of a specific faith tradition or not • Offer prayer • Simply sit and talk Tobacco Treatment This free service helps tobacco users address their addiction to tobacco. Our tobacco treatment specialist can meet with you in person or by phone to help you create a custom plan to quit. She can help you address: • • • • • Nicotine withdrawal Anxiety Coping with stress Recovery from relapse Maintaining your weight Palliative Care Palliative Care is a special team approach to managing symptoms such as: • • • • • • Pain control Shortness of breath Sleep problems Mood changes Fatigue Emotional stress Palliative care eases suffering for patients and caregivers. The goal is for you to have the highest quality of life possible. Your doctor can refer you to our Palliative Care clinic. There is a copay required for this service. Caregivers: A Community of Support Family and caregivers play an important role in a patient’s cancer journey. Caregivers can be spouses, children, family members, friends, and others. Counseling and educational materials are available to provide support to stay emotionally strong and healthy. Caregiver Café – Session for caregivers held twice each month. Caregivers can stop by to get free support and educational materials. Our social workers can provide resources for your children, or help you to talk to your children/teens about cancer.
