These texts should be included literally in the SmPC and PL. However, you are requested to check the SmPC and PL carefully to make sure that there is no overlap with the current texts. If there is such overlap, you should replace the current text with wording from the agreed texts. With regard to PL wording, further user testing by individual MA holders will not be expected on this occasion. For products registered via a MRP or DCP procedure, the RMS takes responsibility on behalf of CMS to request the variation from the MA holder and initiate the procedure. The applications do not require supporting information or expert statements and will be accepted by Member States Competent Authorities without further assessment or amendment. For nationally approved products there is no need to submit a Variation Application Form. Citalopram is contraindicated together with medicinal products that are known to prolong the QTinterval (see section 4.5). (…) Section 4.4 – Special warnings and precautions for use C B G M E B (…) QT interval prolongation Citalopram has been found to cause a dose-dependent prolongation of the QT-interval. Cases of QT interval prolongation and ventricular arrhythmia including torsade de pointes have been reported during the post-marketing period, predominantly in patients of female gender, with hypokalemia, or with pre-existing QT prolongation or other cardiac diseases (see sections 4.3, 4.5, 4.8, 4.9 and 5.1). Caution is advised in patients with significant bradycardia; or in patients with recent acute myocardial infarction or uncompensated heart failure. Electrolyte disturbances such as hypokalaemia and hypomagnesaemia increase the risk for malignant arrhythmias and should be corrected before treatment with citalopram is started. If patients with stable cardiac disease are treated, an ECG review should be considered before treatment is started. If signs of cardiac arrhythmia occur during treatment with citalopram, the treatment should be withdrawn and an ECG should be performed. (…) Section 4.5 – Interaction with other medicinal products and other forms of interactions (…) Contraindicated combinations QT interval prolongation Pharmacokinetic and pharmacodynamic studies between citalopram and other medicinal products that prolong the QT interval have not been performed. An additive effect of citalopram and these medicinal products cannot be excluded. Therefore, co-administration of citalopram with medicinal products that prolong the QT interval, such as Class IA and III antiarrhythmics, antipsycotics (e.g. fentiazine derviatives, pimozide, haloperidol), tricyclic antidepressants, certain antimicrobial agents (e.g. sparfloxacin, moxifloxacin, erythromycin IV, pentamidine, anti-malarian treatment particularly halofantrine), certain antihistamines (astemizole, mizolastine) etc., is contraindicated. (…) Influence of other medicinal products on the pharmacokinetics of citalopram Cimetidine (potent CYP2D6, 3A4 and 1A2 inhibitor) caused a moderate increase in the average steady state levels of citalopram. Caution is advised when administering citalopram in combination with cimetidine. Dose adjustment may be warranted. (…) Langzaam CYP2C19-metabolisme Voor de eerste twee weken van de behandeling wordt een startdosis van 10 mg per dag aanbevolen voor patiënten van wie bekend is dat ze een langzaam CYP2C19-metabolisme hebben. Afhankelijk van de individuele respons kan de dosis worden verhoogd tot maximaal 20 mg per dag (zie rubriek 5.2). (…) Rubriek 4.3 - Contra-indicaties (…) Citalopram is gecontra-indiceerd bij patiënten met een bekende verlenging van het QT-interval of het aangeboren lange-QT-syndroom. Citalopram is gecontra-indiceerd in combinatie met geneesmiddelen waarvan bekend is dat ze het QT-interval verlengen (zie rubriek 4.5). (…) C B G M E B Rubriek 4.4 - Bijzondere waarschuwingen en voorzorgen bij gebruik (…) Verlenging van het QT-interval Citalopram blijkt een dosisafhankelijke verlenging van het QT-interval te veroorzaken. In de postmarketing-periode is melding gemaakt van QT-intervalverlenging en ventriculaire aritmie inclusief torsade de pointes, voornamelijk bij vrouwelijke patiënten, patiënten met hypokaliëmie en patiënten met een reeds bestaande verlenging van het QT-interval of andere hartziektes (zie rubriek 4.3, 4.5, 4.8, 4.9 en 5.1). Voorzichtigheid is geboden bij patiënten met significante bradycardie, evenals bij patiënten met een recent acuut myocardinfarct of niet-gecompenseerd hartfalen. Verstoringen van de elektrolytenbalans zoals hypokaliëmie en hypomagnesiëmie, verhogen het risico op maligne aritmieën en moeten worden gecorrigeerd voordat de behandeling met citalopram wordt gestart. Als patiënten met een stabiele hartaandoening worden behandeld, verdient een ECG overweging voordat de behandeling wordt gestart. Als tijdens de behandeling met citalopram tekenen van aritmie optreden, moet de behandeling gestaakt worden en een ECG worden gemaakt. (…) Rubriek 4.5 - Interacties met andere geneesmiddelen en andere vormen van interactie (…) Gecontra-indiceerde combinaties Verlenging van het QT-interval Er zijn geen farmacokinetische en farmacodynamische studies verricht naar interacties tussen citalopram en andere geneesmiddelen die het QT-interval verlengen. Een additief effect kan bij gecombineerd gebruik niet worden uitgesloten. Daarom is er een contra-indicatie voor gelijktijdig gebruik van citalopram en geneesmiddelen die het QT-interval verlengen, zoals klasse IA- en IIIantiaritmica, antipsychotica (zoals fentiazinederivaten, pimozide, haloperidol), tricyclische antidepressiva, bepaalde antimicrobiële middelen (zoals sparfloxacine, moxifloxacine, erytromycine iv, pentamidine, behandelingen bij malaria, met name halofantrine), bepaalde antihistaminica (astemizol, mizolastine), enz. (…) Invloed van andere geneesmiddelen op farmacokinetiek van citalopram Cimetidine (een sterke CYP2D6-, 3A4- en 1A2-remmer) veroorzaakte een matige stijging van de steady-state-waarden van citalopram. Voorzichtigheid is geboden bij toediening van citalopram in combinatie met cimetidine. Mogelijk is een dosisaanpassing nodig. Rubriek 4.8 - Bijwerkingen Toevoegen in de tabel: Frequentie onbekend: Ventriculaire aritmie inclusief torsade de pointes Toevoegen onder de tabel: (…) In de postmarketing-periode is melding gemaakt van QT-intervalverlenging en ventriculaire aritmie inclusief torsade de pointes, voornamelijk bij vrouwelijke patiënten, patiënten met hypokaliëmie en patiënten met een reeds bestaande verlenging van het QT-interval of andere hartziektes (zie rubriek 4.3, 4.4. 4.5, 4.9 en 5.1). C B G M E B Rubriek 4.9 - Overdosering (…) Controle van het ECG wordt geadviseerd in geval van overdosering bij patiënten met congestief hartfalen/bradyaritmieën, patiënten die gelijktijdig andere geneesmiddelen gebruiken die het QTinterval verlengen, en patiënten met een veranderd metabolisme, bijvoorbeeld door een leverfunctiestoornis. C B G M E B BIJSLUITER – citalopram 10, 20, 30 en 40 mg tabletten 2. WANNEER MAG U DIT MIDDEL NIET GEBRUIKEN OF MOET U ER EXTRA VOORZICHTIG MEE ZIJN? Wanneer mag u dit middel niet gebruiken? (…) - Als u een aangeboren hartritmestoornis hebt of hier last van hebt gehad (vastgesteld met een ECG: een hartfilmpje); - Als u andere geneesmiddelen gebruikt voor het behandelen van hartritmeproblemen of geneesmiddelen die invloed hebben op het hartritme. Zie ook de rubriek ‘Gebruikt u nog andere geneesmiddelen?’. Wanneer moet u extra voorzichtig zijn met dit middel? (…) - Als u last hebt of hebt gehad van hartproblemen of kort geleden een hartaanval hebt gehad. - Als u in rust een trage hartslag hebt en/of u weet dat u een zouttekort hebt als gevolg van langdurige diarree en braken of door gebruik van diuretica (plaspillen). - Als u last hebt van een snelle of onregelmatige hartslag, flauwvallen, toevallen of duizeligheid bij het opstaan, omdat dit kan wijzen op een afwijkende hartslag. Gebruikt u nog andere geneesmiddelen? (…) GEBRUIK DIT MIDDEL NIET als u ook geneesmiddelen gebruikt voor hartritmeproblemen of geneesmiddelen die invloed hebben op het hartritme, zoals klasse IA- en III-antiaritmica, antipsychotica (zoals fentiazinederivaten, pimozide, haloperidol), tricyclische antidepressiva, bepaalde antimicrobiële middelen (zoals sparfloxacine, moxifloxacine, erytromycine iv, pentamidine, behandelingen bij malaria, met name halofantrine) of bepaalde antihistaminica (astemizol, mizolastine). Als u hierover nog vragen hebt, neem dan contact op met uw arts. 3. HOE GEBRUIKT U DIT MIDDEL? (…) Hoeveel moet u gebruiken? Volwassenen Depressie De gebruikelijke dosis is 20 mg per dag. Uw arts kan dit verhogen naar maximaal 40 mg per dag. Oudere patiënten (vanaf 65 jaar) Er moet begonnen worden met een halve startdosis, d.w.z. 10-20 mg per dag. Oudere patiënten mogen doorgaans niet meer dan 20 mg per dag krijgen. Patiënten met extra risico Patiënten met leverklachten mogen niet meer dan 20 mg per dag krijgen. (…) 4. MOGELIJKE BIJWERKINGEN (…) Als u een van de volgende symptomen krijgt, moet u stoppen met het gebruik van dit middel en onmiddellijk contact opnemen met uw arts: (…) C B G M E B Een snelle, onregelmatige hartslag en flauwvallen, wat tekenen kunnen zijn van een levensbedreigende aandoening die torsade de pointes heet. C B G M E B ANNEX 3 Summary Assessment Report of the PhVWP October 2011 Citalopram – Risk of QT interval prolongation Key message Citalopram may cause QT prolongation and the product information will be updated, in particular to reduce the maximum daily dose to 40mg/day in adults and 20 mg/day in the elderly and patients with impaired liver function. Safety concern and reason for current safety review The PhVWP assessed new data from a randomised, multi-centre, double-blind, placebo-controlled, crossover pharmacokinetic and thorough QT-study undertaken in healthy volunteers given daily doses of 20 and 60 mg citalopram. The FDA issued an announcement on 24 August 2011, notifying healthcare professionals in the US that citalopram should no longer be used at doses higher than 40 mg/day. The reason was identification of an increased risk for QT interval prolongation. As part of its review, the PhVWP assessed the data which formed the basis for this actionwhich included the results of the above mentioned study. Clinical setting Citalopram is indicated for the treatment for depression, panic disorder and obsessive-compulsive disorder (OCD). QT interval prolongation is an indication of abnormal heart rhythm, which can lead to ventricular arrhythmia, including torsade de pointes, and sudden cardiac death. Information on the data assessed The PhVWP assessed the results of a randomised, multi-centre, double-blind, placebo-controlled, crossover pharmacokinetic and thorough QT-study undertaken in healthy volunteers given daily doses of 20 and 60 mg citalopram. The PhVWP additionally evaluated data from the marketing authorisation holder of the originator citalopram-containing product in the EU provided in response to a list of question from the PhVWP. Finally, spontaneously reported cases of suspected reactions which could be related to QT interval prolongation were reviewed. Outcome of the assessment Following review of the thorough QT-study, the following can be concluded: A dose-dependent increase in QT interval was shown in this thorough QT study, particularly with 60 mg/day.The change from baseline in QTc (Fridericia-correction) was 7.5 (90%CI 5.9-9.1) msec with 20 mg/day and 16.7 (90%CI 15.0-18.4) msec with 60 mg day. The study was considered of good quality, although testing of a higher dose would have been desirable (in accordance with the ICH E14 guideline). A positive control was included, i.e. moxifloxacin 400 mg/day, which showed expected results, thus confirming assay sensitivity. Further, the PhVWP considered that cases of QT interval prolongation and ventricular arrhythmia including torsade de pointes were spontaneously reported, predominantly in female patients, with hypokalemia, pre-existing QT interval prolongation or other cardiac diseases. Most of the reported cases of torsade de pointes had a positive dechallenge. Positive rechallenge was also reported. In addition, a number of cases of ventricular arrhythmia, sudden death, loss of consciousness and syncope might also be associated with torsade de pointes. The data from spontaneous reporting also had limitations, for instance underreporting was considered as potentially large for events such as torsades de pointes, cardiac arrhythmia or sudden cardiac death. Thus, those data could not be used C B G M E B to exclude the existence of a risk. Overall, the PhVWP considered that the results from the study indicated that citalopram causes dosedependent QT interval prolongation. In considering the appropriate risk minimisation strategy, the PhVWP also noted that elderly patients and patients with reduced liver function achieve higher systemic exposure than younger patients with normal hepatic function. Furthermore, it was also noted that cases of QT-interval prolongation have been reported also in association with some other selective serotonin re-uptake inhibitors (SSRIs) including escitalopram, the S-enantiomer of citalopram. Based on a review of the available data, the PhVWP concluded that the summary of product information for citalopram-containing medicinal products in the EU should be updated to - remove the dose recommendation of 60 mg/day; - limit the maximum daily dose for adults to 40 mg/day; - limit the daily dose in the elderly and in patients with hepatic impairment to 20 mg/day; - contraindicate use of citalopram with other medicinal products known to prolong the QT interval; - include relevant contraindications and warnings; and - include ventricular arrhythmia including torsade de pointes as adverse reactions. The package leaflet should be updated accordingly and include advice to patients to - contact a healthcare professional immediately if they experience signs and symptoms of an abnormal heart rate or rhythm while taking citalopram; - not to stop taking citalopram or change or reduce the dose without first consulting their healthcare professional, as withdrawal symptoms may occur when citalopram treatment is discontinued, particularly if this is abrupt; Healthcare professionals are advised to review patients on doses that are above the now recommended maximum dose and gradually reduce the dose accordingly.
