Disease management – Primary care. Prevention/monitoring. Case management, complex long-term. Complex conditions – role of community matrons. Planning transfer of care. Concepts, policies and implementation of palliative care including - Pain and symptom management. Critically analyse the attitudes, values and beliefs of society, the community, colleagues and self regarding adults with a variety of complex/long term needs 2. Critically discuss the principles of care in relation to adults who may have chronic/long-term disability 3. Critically discuss the range of care philosophies attributed to the care of adults with chronic physical and/or psychological needs 4. Investigate the possible effects of chronic physical and/or psychological health problems on the family, carers and significant others 5.
