Aims of the
Module
This module is designed to enable the student to develop an evolving knowledge
and practice base in the care of adults with complex and long term health needs

Module
Content
The concepts, policies and implementation of continuing care as discussed in
contemporary government policy e.g. in the NHS improvement plan including Self management (expert patients). Disease management – Primary care.
Prevention/monitoring. Case management, complex long-term. Complex
conditions – role of community matrons. Planning transfer of care. Concepts,
policies and implementation of palliative care including - Pain and symptom
management. Complementary therapies.

1. Critically analyse the attitudes, values and beliefs of society, the
community, colleagues and self regarding adults with a variety of
complex/long term needs
2. Critically discuss the principles of care in relation to adults who may
have chronic/long-term disability
3. Critically discuss the range of care philosophies attributed to the care of
adults with chronic physical and/or psychological needs
4. Investigate the possible effects of chronic physical and/or psychological
health problems on the family, carers and significant others
5. Critically discuss the importance of collaborative working with the adult,
family/carers together with statutory and non statutory agencies

Notes
When this module is taken by students via FDL:
- interactive tutor-led sessions will be replaced by directed learning using the
VLE
- tutorials will be by a combination of asynchronous discussion via the VLE
and telephone tutorial (or similar).
