patients’ vital signs. You have been tasked to take vital signs and physical assessments on Mr.
Simataa, you note that his vitals are as follows pulse 108 b/min, SpO2 98%, respirations 24b/min
and a Temperature of 38.5 0 C

1. “Initial assessment—performed within specified time after admission to a health care
agency to establish a complete database for problem identification, reference and future
comparison” ( Berman & Snyder, 2014, p.197).
2. “Problem-focused assessment—ongoing process integrated with nursing care to

The American Nurses Association Code of Ethics (2001) states that “ the nurse has a duty to
maintain confidentiality of all patient information”( p.12), (as cited in Kozier & Erb’s
Fundamentals of Nursing Concepts, Process, and Practice, 2014, p.273).
According to A. Berman & S. Snyder (2014), “The client’s record is also protected legally as a
private record of the client’s care. Access to the record is restricted to health professionals
involved in giving care to the client. The institution or agency is the rightful owner of the client’s
record” (p.273).
“A personal password is required to enter and sign off computer files” (Berman & Snyder, 2014,
p.273).
“Do not leave client information displayed on the monitor where others may see it” (Berman &
