Safety Checklist for Educational Supervisors in GP Training Trainee Name: 1 Prescribing Safely Knowledge of safe prescribing in context of primary care: including high risk medicines, DOACs, DMARDS, unlicensed medications; and high-risk patient groups renal impairment, pregnancy, medication dosing in children. Knowledge of controlled drugs storage, dose adjustments, prescription format. Knowledge of practice policies and systems for drugs of abuse and addiction including methadone/ opiate replacement therapy if applicable. Knowledge of practice prescribing systems for both acute (special) and repeat requests. Knowledge of how to hand write prescriptions. Knowledge of practice systems for monitoring prescriptions (e.g. antidepressants or analgesics) 2 Dealing with Medical Emergencies Knowledge of emergency care procedures including drugs, equipment, emergency detention paperwork and protocols. Aware of emergency contacts- ambulance, police, mental health, social work, homecare etc 3 Home visiting Safely Essentials for doctors/ visit bag- drugs, equipment, essential forms and paperwork. Consideration of personal safety- who, how and when to access help. Partners mobile/ bypass phone systems, procedure for late house calls. Understand importance of timely documentation of encounter and prescription in electronic record. 4 Dealing Effectively with Results of Investigation Requests Knowledge of practice systems for results, including any covering arrangements for absent clinicians. 5 Dealing Effectively with other parts of the health care system Knowledge of system for making referrals including how and when to refer urgently or routinely. Local and national pathways for referrals. Awareness of safe clinical handover, successful admission conversations, safe delegation of tasks and dealing with inappropriate requests. Knowledge of systems for dealing with discharge documents, clinic letters, clinical email etc. 6 Effective and Safe Communication Knowledge of communication processes within the practice, email, message systems, contact details and communication route for staff Understanding of safety netting and follow up planning, including clinical handover Comments 7 Ensuring Confidentiality Understanding of GMC guidance on confidentiality Knowledge of the appropriate storage of medical and personal information in line with GDPR and practice protocols. 8 Keeping Effective Records Understand the need for confirmation of patient identity during all contacts (face-to face and telephone consultations).
