Documentation Guidelines
Students are required to provide documentation in order to participate in fieldwork and community
experiences throughout the duration of the program. Students are to have all items completed by the
expected deadlines. Failure to complete the requested documentation by the due dates will delay the
student’s ability to participate in fieldwork, community experiences, and associated coursework.

Incoming 1st year OTD students must have all documentation completed and up to date no later
than August 1st, 2022. We will review this information again prior as needed throughout the
program to ensure students are prepared for all fieldwork experiences.
When applicable, students are to take all required forms and documentation to their respective
appointments with fieldwork educators/fieldwork sites.
Students are advised to keep all originals; students may be required at any time during their time in the
program to produce the updated and original documents to their fieldwork sites. All documents must be
added to CORE in order to be accessible by AFWC and student at any time.
The following is an overview of the Fairleigh Dickinson University OTD program documentation
requirements:
1. Student Personal Data Form for Fieldwork Placement (update each semester)
2. OT Immunization and Health Report (update at least annually or as needed)
3. Immunization record (one copy required throughout enrollment in the program, unless the record
changes)
4. PPD/Mantoux test (update annually, or as required by fieldwork site) – Student must have PPD test
completed prior to beginning of fieldwork participation. Test date, product manufacturer and Lot
#/expiration date must be recorded on Immunization and Health Report. If a student has a history of
positive PPD the following information must be recorded: date of conversion, size of reaction, last
chest X-ray, and any treatment received. Please note that some of our clinical sites are requiring
that PPD testing be done no more than 3 months prior to starting a clinical rotation. It is the
student’s responsibility to have the testing done or repeated if necessary.
5. Flu Vaccine (update annually during each active flu season) Maintain proof of vaccination to provide
to fieldwork sites as needed.
6. CPR – must have current certification through American Red Cross or American Heart
Association prior to matriculation into the program (update as needed throughout duration of
program based on initial certification date). Certification must include adult and pediatric CPR, AED
and First Aid. Online or partial online certifications are not acceptable.

7. Criminal Background Check (every year) – At the request of the fieldwork site students must have
a background check completed at their expense, this must include both state of permanent residency
as well as state where fieldwork placement is to occur. A web-based site that has been used and
accepted by many of our fieldwork partners is Certiphi Screening Inc. (https://www.certiphi.com/)
Some fieldwork sites may require you to use a specific company or website for you background
check completion.
8. Child Abuse History Clearance – Each student is required at the request of the fieldwork site to
obtain a New York State clearance and fingerprinting for all pediatric Level I FW experiences as well
as clearance for any state where a pediatric rotation may occur during a Level II FW experience.
These clearances will also be at the student’s expense.
9. Health Insurance – Each student must provide proof of continuous health insurance coverage while
enrolled in the program.
10. Malpractice Insurance – Each student must provide proof of continuous malpractice insurance
coverage while enrolled in the program. Malpractice insurance can be purchased through Mercer
(Proliability) and must be in the amount of 1mill/3mill aggregate and cover the entire two year
duration of the program.
11. HIPAA and Infection Control Training – Will be provided through an online training course.
Students will be provided the link by the AFWC toward the end of the summer semester.
12. Drug Screens & FBI Fingerprinting– May be required by the Department of Occupational Therapy
or clinical sites in order to provide safe working and learning environments for students, faculty, staff
and patients. It is the student’s responsibility to complete these as required, at their expense.
