Insurance Information SPORTS MEDICINE - FORT SCOTT Community College 2108 SOUTH HORTON∙FORT SCOTT, KS 66701 Phone (620) 223-2700 ex.7050∙Fax (620) 223-4438 PLEASE KEEP THIS LETTER FOR FUTURE REFERENCE The Athletic Training Department of Fort Scott Community College is dedicated to the prevention and treatment of athletic injuries, serving the needs of each athlete in every sport. This explains FSCC’s policies and procedures concerning payment for treatment of athletic injuries. POLICIES & PROCEDURES CONCERNING MEDICAL EXPENSE COVERAGE All student athletes need to have their own primary insurance in their own name, through parent/guardian or be self-insured. Athletes must show proof of insurance at the beginning of each school year in order to be eligible for participation. All medical expenses for injuries sustained while participating in an athletic event at FSCC must first be submitted to athlete’s primary insurance carrier. The FSCC Athletic Department purchases a medical insurance policy which may cover some expenses not covered by your primary insurance policy. Claims submitted by FSCC to this policy must be accompanied by proof of payment or denial from your primary insurance carrier. The FSCC Athletic Trainer and/or Team Physician(s) will arrange for all medical treatments and services required for any athletic injury except during a medical emergency on the road or at practice and when no athletic trainer is present. Medical expenses incurred by a student athlete for medical treatment and services obtain without prior authorization of FSCC’s Head Athletic Trainer and/or Team Physician(s) will be the sole responsibility of the student athlete and/or his parent(s)/guardian(s). The athletes and/or their parent(s)/guardian(s) will be responsible for the payment of medical services regarding: o Pre-existing and congenital medical conditions, o Non-athletic related injuries, o Illness (colds, flu, etc.), o Medical conditions not related to participation in FSCC Athletics, o Medical expenses for athletic injury if the referral was not authorized by the Head Athletic Trainer or Team Physician(s), or o Medications for any non-athletic injury. All arrangements for the treatment of athletic injures must be made before the student athlete graduates or withdraws from FSCC. Responsibility for any medical expenses will not be accepted by FSCC or FSCC’s insurance representative after a period of one year following the date of injury. All insurance information, history and physical must be received by the Athletic Trainer before athletes are allowed to participate at Fort Scott Community College.
