IKHLASlink Comprehensive Critical Illness Benefit Secure Takaful Rider 1. IKHLASlink Comprehensive Critical Illness Benefit Secure Takaful Rider Securing life uncertainties. With IKHLASlink Comprehensive Critical Illness Benefit Secure Takaful Rider (CCIB), you are comprehensively covered through the most difficult stages of your life. The plan helps to ease your financial burden specifically covering the medical cost for life-threatening critical illnesses. IKHLASlink Comprehensive Critical Illness Benefit (CCIB) Secure Takaful Rider is a unit deducting rider which is attachable to the regular Takaful Contribution investment-linked Takaful plans. 2. What are the covers / benefits provided? Benefits CCIB Secure Takaful CCIB Diabetes Recovery Benefit CCIB Cancer Recovery Benefit i) CCIB Diabetes Recovery Benefit and CCIB Cancer Recovery Benefit are payable in addition to CCIB Secure Takaful amount. These benefits are not payable once 100% of CCIB Secure Takaful amount is paid for other covered critical illness. ii) Coverage for Critical Illness under Advanced Stage expires when Person Covered reaches the age of 100 at his/her next birthday. Whereas coverage for Critical Illness under Early and Intermediate Stages and Diabetic Related Disease expire when Person Covered reaches the age of 85 at his/her next birthday. a) Comprehensive Critical Illnesses Benefit (CCIB) the Person Covered is diagnosed of any one (1) of the CCIB listed in the table below, before If the Maturity or Expiry date of the Supplementary Takaful Certificate, a percentage (%) of the CCIB Secure Takaful amount based on the Critical Illness stages, shall be payable. The Supplementary Certificate shall terminate once 100% of the CCIB Secure Takaful amount has been exhausted. This benefit is payable once only. The CCIB Secure Takaful amount will be reduced by the claim amount paid under ‘Angioplasty and Other Invasive Treatment for Coronary Artery Disease’. c) CCIB Diabetes Recovery Benefit Category Diabetic Related Diseases Disease • Surgery for Type 2 Diabetic Retinopathy • Limb Amputation due to Type 2 Diabetic Complications • Severe Diabetic Nephropathy resulting in Kidney Failure The CCIB Diabetes Recovery Benefit amount is equal to twenty percent (20%) of the CCIB Secure Takaful amount. Only one (1) claim is allowed under CCIB Diabetes Recovery Benefit and this Benefit is paid on top of the CCIB Secure Takaful amount. d) CCIB Cancer Recovery Benefit The CCIB Cancer Recovery Benefit amount is equal to thirty five (35%) of the CCIB Secure Takaful amount. The amount for Early Stage Critical Illness is thirty percent (30%) of the CCIB Cancer Recovery Benefit amount, and the amount for Intermediate/ Advanced Stage Critical Illness is one hundred percent (100%) of the CCIB Cancer Recovery Benefit amount. Only one (1) claim is allowed for Early Stage Cancer under this benefit. The subsequent claim made under Intermediate / Advanced Stage Cancer will be paid based on the remaining CCIB Cancer Recovery Benefit amount. This Benefit is paid on top of the CCIB Secure Takaful amount. For further details on the coverage provided, please refer to the Supplementary Takaful Certificate. 3. How much Takaful Contribution do I have to pay? No additional Takaful Contribution applicable to this Rider since the Tabarru’ charge will be deducted from Unit Fund of the Basic Takaful Certificate. However, the Tabarru’ charge is NOT GUARANTEED and may be varied from time to time by giving three (3) months’ notice to the Takaful Participant prior to Takaful Certificate anniversary. 4. What are the fees and charges that I have to pay? a) Wakalah Fees - not applicable. b) Tabarru’ charge is deducted from the Unit Fund of the Basic Takaful Certificate on monthly basis (through cancellation of units at unit price) and Goods and Services Tax (GST) will be charged on the tabarru’. Tabarru’ will increase as the Person Covered grow older. Details of the tabarru’ amount and other charges for this rider are illustrated in the benefit illustration. 5. Terms and Age Limit Issue Age: Takaful Participant: 10 – 60 years old, age next birthday Person Covered: 30 days – 60 years old, age next birthday Note : Takaful Participant between age 10 – 16 years old are subject to consent from parents/ guardian. Sum Covered Limits: Minimum : RM10,000 Maximum : RM200,000 Expiry Age: • Coverage for Critical Illness under Advanced Stage expires when Person Covered reached the age of 100 years old, age next birthday. • Coverage for Critical Illness covered under Early and Intermediate Stages and Diabetes Recovery Benefit will expire when Person Covered reached the age of 85 years old, age next birthday. 6. Additional terms and Conditions a) Waiting period for this plan is: i) Coverage for Critical Illness under Advanced Stage will only start sixty (60) days after the Issue Date or Reinstatement Date, whichever is later for Cancer – of specified severity and does not cover very early cancers (including Advanced Stage Critical Illness covered under Cancer Recovery Benefit), Heart Attack – of specified severity, Coronary Artery By-Pass Surgery, Serious Coronary Artery Disease, and Angioplasty and Other Invasive Treatments for Coronary Artery Disease and thirty (30) days for all other covered illnesses. ii) Coverage for Diabetic Related Diseases, Critical Illness under Early and Intermediate Stage (including Early and Intermediate Stage Critical Illness covered under Cancer Recovery Benefit) will only start sixty (60) days after the Issue Date or Reinstatement Date, whichever is later. d) Claims conditions: i) Any amount payable under CCIB Secure Takaful (any claim paid under Early Stage Critical Illness and Angioplasty and Other Invasive Treatments for Coronary Artery Disease) will reduce the CCIB Secure Takaful amount available for subsequent claims except if the claim is made under CCIB Diabetes Recovery Benefit and CCIB Cancer Recovery Benefit. Once a claim has been paid for a Critical Illness from any stages, other Critical Illness ii) within the same or earlier stages under that same category will no longer be covered. iii) A maximum of 2 Early Stage claims under different Critical Illness category according to the Critical Illness table can be made and the Supplementary Takaful Certificate will terminate thereafter. Diabetes Recovery Benefit and CCIB Cancer Recovery Benefit will not be payable iv) CCIB once 100% of the CCIB Secure Takaful amount has been paid. 7. What are the major exclusions under the Takaful Certificate? This Supplementary Takaful Certificate does not cover if: a) The Critical Illness is any illness or surgery other than diagnosis of or surgery for a Critical Illness as defined herein; b) The signs or symptoms of the Critical Illness defined under Advanced Stage that is manifested prior to or within the Waiting Period; c) The Critical Illness was caused directly or indirectly by the existence of Acquired Immune Deficiency Syndrome (AIDS) or by the presence of any Human Immuno-deficiency Virus (HIV) infection. The Company reserves the right to require the Person Covered to undergo a blood test for HIV as a condition precedent to acceptance of any claim. This exclusion does not apply to HIV Infection due to Blood Transfusion. For the purpose of the Supplementary Takaful Certificate, i) The definition of AIDS shall be that used by the World Health Organization in 1987, or any subsequent revision by the World Health Organization of that definition. ii) Infection shall be deemed to have occurred where blood or other relevant test(s) indicate in the Company’s opinion either the presence of any Human Immunodeficiency Virus or Antibodies to such a Virus. d) Any stage of Cancer caused directly or indirectly, wholly or partly, by any one (1) of the following occurrence: i) Any stage of Cancer or surgery other than diagnosis of or surgery for a stage of Cancer as defined herein; or ii) The signs or symptoms of Cancer that is manifested prior to or within sixty (60) days from the issue date or reinstatement date of this Supplementary Takaful Certificate, whichever is later; or iii) The Cancer arises directly or indirectly from a Pre-Existing Condition as defined in the Supplementary Takaful Certificate, which existed prior to the issue date or reinstatement date of this Supplementary Takaful Certificate, whichever is later; or e) Any Diabetic Related Disease caused directly or indirectly, wholly or partly, by any one (1) of the following occurrence: i) Any illness or surgery other than diagnosis of or surgery for Diabetic Related Disease as defined herein; or ii) The signs or symptoms of Diabetic Related Disease that is manifested prior to or within sixty (60) days from the issue date or reinstatement date of this Supplementary Takaful Certificate, whichever is later; or iii) The Diabetic Related Diseased arises directly or indirectly from a Pre-Existing Condition as defined, which existed prior to the issue date or reinstatement date of this Supplementary Takaful Certificate, whichever is later; or f) The Critical Illness is within the same or earlier stage of the previous Critical Illness claim category, save and except for Angioplasty and Other Invasive Treatments for Coronary Artery Disease; g) Any Critical Illness was diagnosed due, directly or indirectly, to a congenital defect or disease, which was manifested or was diagnosed before the Person Covered attains seventeen (17) years of age; h) Any of the Critical Illness is caused by a self-inflicted injury; i) Any Critical Illness resulting directly from alcohol or drug abuse; j) The Person Covered did not survive for at least seven (7) days after the diagnosis of an Early Stage Critical Illness; or k) The Person Covered did not survive for at least thirty (30) days after: i) The diagnosis of an Intermediate Stage Critical Illness; or ii) The diagnosis of an Advanced Stage Critical Illness; or iii) The undergoing of Angioplasty and other Invasive Treatment for Coronary Artery Disease; or iv) The diagnosis of a Diabetic Related Disease; or Note: This list is non-exhaustive. Can I cancel my Takaful Certificate? you find that this rider is no longer appropriate, you may cancel it by giving us a written If notice. 9.
