JOB DESCRIPTION
POST:
Consultant Post in Neuro Anaesthesia
AREA:
Directorate of Anaesthesia, Critical Care & Operating Services
ACCOUNTABLE TO:
Clinical Director – Anaesthetics
SUMMARY OF DUTIES:
To provide services in anaesthesia and related specialities to
Sheffield Teaching Hospitals NHS Foundation Trust in
accordance with evidence based practice. Also to participate in
clinical audit, teaching of trainees and maintenance of high
standards of medical practice.
APPOINTMENT
Whole-time or part-time Consultant Anaesthetist, with an interest in neuro anaesthesia to the
Sheffield Teaching Hospitals NHS Foundation Trust.
This is a replacement post.
Any candidate who is unable for personal reasons to work full time will be eligible to be
considered for the post; if such a person is appointed, modification of the job content will be
discussed on a personal basis with the Trust in consultation with Consultant colleagues.
The successful candidate may be expected to work at both campuses in the Trust and on call work
being based at the Royal Hallamshire Hospital.
DUTIES OF THE POST
The appointee will, together with consultant colleagues, be responsible for the provision of general
surgical anaesthesia services.
RESPONSIBILITIES
1.
Provide safe, evidence-based anaesthesia and analgesia.
2.
Maintain and improve standards and quality of anaesthetic services.
3.
Supervision and training of junior staff in both elective and emergency situations.
4.
Maintain skills and up-date knowledge base meeting the Continuing Education and
Professional Development requirements of the Royal College of Anaesthetists.
5.
Participate in the audit programme of the Department.
6.
Lead tutorials and departmental meetings, in rotation with other Consultant staff.
7.
The appointee would be expected to have some special area of expertise/interest and to
have delegated responsibility for appropriate clinical management in that area.
8.
Participate in the Consultant on-call rota at Sheffield Teaching Hospitals.
9.
Prepared to work on all sites for elective work. However, lists will generally be on one site
in any one day.
10.
The successful candidate would inevitably have responsibility for resource utilisation and
would be expected to be aware of and take part in the Directorate business planning process
which governs new developments.
Some flexibility within duties dependent on the exigencies of the service will be expected from the
appointee.
The successful candidate will work according to the terms and conditions of the new consultant
contract (England).
The allocation of time between the main duties of the post will be open to some adjustment in

NOTES:
1.
This is a Specimen Job Plan only. Flexibility is available and final Job Plans can be
negotiated on appointment.

This is a 10 PA post with the direct clinical care sessions being described as 1.25 PA’s

SPA = Supporting Professional Activity. 1.5 PA will be allocated to the successful applicant
on appointment with up to 2.5 PAs being available by negotiation at job planning with the
Clinical Director.

Flexible Clinical Sessions are fixed in time but vary according to clinical necessity and aim
to provide comprehensive anaesthetic services appropriately. These are directed by the
anaesthetic service organiser responding to immediate workload demands.

Lists may be offered on all sites, however, generally only on one site in any one day

On-call will be based at the Royal Hallamshire Hospital and will accrue additional PA time.
An annual review of duties will take place with the Clinical Director as part of the Job Plan process.
The above work allocation is based on the new “Consultant Contract” of 10 Programmed Activities
divided into Direct Clinical Care, Supporting Professional Activity and On Call.
The successful candidate will undertake an appropriate share of out of hours Neuro Anaesthesia
Emergency care in rotation with other Neuro Anaesthetic Consultants. Similarly it is expected
there will be a sharing of the administrative duties within the Directorate.
The person appointed will take his/her equal share in providing emergency cover and will be
required to cover for colleagues during periods of absence. He/she will have continuing
responsibility for patients in his/her care and the proper functioning of the Department.
The appointee will be expected to participate in and actively contribute to the clinical governance
activities of the Directorate.
The appointee will be expected to take an active part in Undergraduate and Postgraduate teaching
and training and in this respect, as long as you remain a Consultant within the Trust, you will be
regarded as an Honorary Senior Clinical Lecturer in the University of Sheffield, subject to the
regulations of the University. Research and audit activity will be actively encouraged and
promoted.
MANAGEMENT ARRANGEMENT
Clinical Directorates because operational at the Royal Hallamshire Hospital on 1 April 1991 and are
now well established. Clinical Directors are appointed by the Chief Executive for a three-year
period, subject to annual review.
This is a large cross-city directorate which is challenged with shaping the future direction of much
of the clinical activity within Sheffield Teaching Hospitals.

ANAESTHETIC ADMINISTRATION
Royal Hallamshire Hospital
The Anaesthetic Department is housed on C Floor and is currently undergoing extensive
refurbishment which will provide open plan office accommodation together with a large Seminar
Room and IT area. There are offices situated within this area for the Service Manager, Secretarial
staff, Clinical Director, General Manager and Pain Management Services. Facilities also include a
newly designed kitchen, photocopying area and individual meeting/interview rooms.
Northern General Hospital
The Anaesthetic administrative centre is housed at the end of the main operating theatre corridor.
Offices are at present used by the Anaesthetic Secretaries, Clinical Director, General Manager,
Business Manager, Service Manager, Nurse Director, Consultant Anaesthetists, Service Organisers
and the Acute Pain Nurses. Facilities also include a large seminar room, photocopying/printing
facility and kitchen.
The Chronic Pain Service is housed in Out-Patient Department 4 at the NGH following a successful
merger of Pain Services in June 2007.
KEY INFORMATION POINTS
1.
The Department of Anaesthesia at both the Royal Hallamshire and Northern General
Hospitals are both well established departments. Both departments have a range of
facilities including lecture and workshop discussion facilities together with good audiovisual aids including access to powerpoint software.
2.
There are a range of anaesthetic sub-specialities available for development within the Trust.
3.
The Directorate participates in a well-established rotational training programme with
Sheffield University Hospitals Trust as the administrative centre. All grades of junior staff
and medical students rotate through the Department.
4.
The Trust oversees the management of all sites via 9 major groups of clinical and non
clinical directorates; it is well organised and patient focused. The Trust offers most clinical
services expected in the modern NHS provides 2050 inpatient beds with 12,000 highly
committed members of staff. The Trust comprises of 5 hospitals sited on a central and
northern Sheffield campus.
The Royal Hallamshire Hospital is a 750 bedded general hospital which also houses the
medical school of the University of Sheffield. Speciality services provided are head and
neck, urology, communicable diseases, tropical medicine, haemotology, rheumatology and
dermatology.
The Jessop Wing on site provides the base for obstetrics, gynaecology, community
maternity services and neonatal intensive care.
Western Park Hospital provides specialist cancer services.
Charles Clifford Dental Hospital is a co-location of the University of Sheffield School of
Clinical Dentistry providing specialist dental services. As a Trust, we have very close
clinical teaching and research links with the local Universities.
The Northern General Hospital is a 1300 bedded unit 3 miles North East of the central
campus. The hospital houses the single accident and emergency department for the City of
Sheffield and also manages the emergency admissions unit at the Royal Hallamshire
Hospital.
Acute general medicine is managed on this site by a single directorate overseeing the
service at the Royal Hallamshire Hospital. Surgical services provide plastics, general and
orthopaedic surgery. The hospital also houses vascular, burns and cardiothoracic services,
renal, spinal and specialist rehabilitation and specialised medical services are also present.
Both campus sites provide a comprehensive range of clinical treatment and support services
for both general and highly specialised interests: Medical physics, imaging, pharmacy,
laboratory medicine and professional services along with Anaesthesia, Operating Services
and Critical Care.

Recent review of General Surgery provision in Sheffield has seen this service reconfigured,
Upper GI, Breast and Endocrinology based at the Royal Hallamshire Hospital, Colorectal at
the Northern General Hospital. Unification of other inpatient base specialities has been a
major principle across the Trust.
6.
The hospitals are easily accessible. Good public transfer routes place us in the centre of the
M1, M62, M18 and A1(M) network and London is only 90 minutes away by Inter City train.
An international airport opened in Doncaster in Spring 2005 on the site of the former RAF
base at Finningley.
7.
There is a whole range of housing choices within the Sheffield area including suburban or
town centre developments and a number of village settlements, all within the required
travelling distance of the hospitals.
8.
The major industries in the area historically have been coal mining and engineering,
however the decline in these activities has seen significant diversification and introduction
of new industries. The Sheffield area allows a high quality of life, boasting leisure and
lively cultural activities in the centre and being surrounded by the scenic countryside of the
Peak District with Meadow Hall shopping centre is only 30 minutes away – one of Europe’s
larges indoor shopping malls. Leisure and recreation facilities are well provided for,
especially at Ponds Forge and the Don Valley Stadium. Sheffield also has a dry ski slope
with excellent facilities.
MAJOR CLINICAL SERVICES – SHEFFIELD TEACHING HOSPITALS NHS FOUNDATION
TRUST
Royal Hallamshire Hospital Theatres
There are 12 main operating theatres, 3 obstetric theatres, 5 theatres within the day case unit (with
inpatient capability).
Neuro Anaesthesia.
Care is provided for elective and emergency neuro surgical procedures in dedicated theatres,
interventional neuro radiology, MRI and CT scanning suites. There is a dedicated Neuro
ITU/HDU which is to expand its provision in the near future. This unit has full daytime Neuro
Consultant and trainee anaesthetic cover.
Obstetrics & Gynaecology
The Obstetric Jessop Wing is located at the back of the tower block and is supported by a team of
dedicated specialist anaesthetists. The unit currently supports 7000 deliveries per year.
Gynaecology has dedicated facilities located in the main block.
Urology
Provides facilities for highly specialised surgery and a full stone service.
General Surgery – Upper GI
Specialises in hepato biliary surgery.
Head & Neck
Specialises in general, specialist oncology and trauma surgery. Full ENT service and a
comprehensive neurosurgical facility with trauma access 24 hours per day.
Emergency Theatre
A dedicated theatre is available 24 hours a day for emergency surgical cases. The anaesthetic cover
is provided by a team of anaesthetists led by a Consultant Anaesthetist.
Post-Operative Surgical Unit
There is an 8 bedded unit that will expand to 12 beds by 2009/10.
Acute Pain Team
A dedicated team of Acute Pain nurses are based within the Department of Anaesthesia supported
by a Lead Consultant Anaesthetist. Regular visits to the wards are made and audits conducted, but
the main roles are education and support and hands on treatment for patients.
Day Surgery
The Day Surgery Unit at the Royal Hallamshire Hospital is incorporated into the main RHH
building. It therefore has the flexibility to provide services for both day case and inpatients.
Charles Clifford Dental Hospital & Weston Park Hospital
Theatres are also located within the Charles Clifford Dental Hospital predominantly providing
dental services under sedation. A theatre is also located in the Weston Park Hospital to
accommodate on demand care for cancer patients and also provides a local anaesthesia ophthalmic
service.
Northern General Hospital Theatres
There are 17 main operating theatres, 1 spinal injuries theatre, 2 day case theatres and local
anaesthetic treatment room in the Bev Stokes unit, 5 cardiac theatres and also cardiac and vascular
angiography suites.

General Surgery – includes colorectal and renal access and donation.

Orthopaedics – the largest single site in the country providing dedicated specialist orthopaedic,
arthroplasty and 24 hour trauma facilities.

Vascular – a comprehensive mix of acute and highly specialised services are provided for South
Yorkshire.

Plastic Surgery and Burns – provision is for general trauma and highly specialised surgery. The
unit also supports the Sheffield Children’s Hospital.
Day Surgery
Much of the day case surgery occurs at the purpose built Bev Stokes Unit. This is a twin theatre
unit with local anaesthetic accommodation. It provides facilities for all specialities.
Pre-operative assessment for all specialities is nurse led. Staff are trained at Masters level.
Theatre Admissions Unit
The Trust has developed an admissions unit adjacent to main theatres allowing all patients
admitted on the day of surgery to be prepared by dedicated theatre staff allowing effective and
efficient perioperative care.
Pain Management Unit
The Trust has established a Pain Management Unit at Sheffield Teaching Hospitals serving the
Sheffield populations and referrals taken from the surrounding district generals. A wide range of
techniques for managing chronic pain are available and a multi-disciplinary practice is promoted.
Post Operative Surgical Unit
The directorate has a 7 bedded post-operative unit housed in the new Critical Care Department
adjacent to the main theatre suite. The unit has 24 hour/5 day week F2 doctors and critical care
trained nursing to support the higher dependent post-operative patient requiring pain relief, fluid
management and increased monitoring and medical care
Acute Pain Team
A dedicated team of Acute Pain nurses are based within the Department of Anaesthesia supported
by a Lead Consultant Anaesthetist. Regular visits to the wards are made and audits conducted, but
the main roles are education and support and hands on treatment for patients.
Pre-Operative Assessment Unit
The Directorate manages a centralised pre-operative assessment unit for all patients attending main
theatres at the Northern General Hospital. The unit is nurse led with daily anaesthetic Consultant
surgeries to support complex cases.
Emergency Theatre
A dedicated theatre is available 24 hours a day for emergency surgical cases (predominantly
general and vascular). The daytime anaesthetic cover is provided by a team of anaesthetists led by
a Consultant Anaesthetist.
THE MAIN CONDITIONS OF SERVICE
The appointment will be in accordance with the Terms and Conditions of Service of Hospital
Medical and Dental Staff (England) as amended from time to time. The arrangement of duties will
be such as may be agreed from time to time between the employing Trust and the person
appointed. Whether the successful candidate chooses to accept a whole-time or part-time contract,
it is agreed that any private practice undertaken, whether limited or not by the Terms and
Conditions of Service, will in no way diminish the level of service that may be expected from
him/her in carrying out the duties specified above (Schedule 9 Terms and Conditions of Service).
This Trust supports the concept of Continuing Medical Education and in line with Clinical
Governance Senior Clinicians are expected to participate in continuing professional development
(CPD).
MEDICAL EXAMINATION
This appointment is subject to medical fitness and the appointee may be required to undergo a
medical examination and chest X-ray.
Potential applicants should be aware of the Department of Health and GMC/GDC requirements
with regard to HIV/AIDS, Tuberculosis, Hepatitis B and Hepatitis C viruses.
Candidates are advised that they will be asked at interview whether they are aware of anything
preventing them from fulfilling their professional commitments and this includes health.
The successful candidate must be immune to Hepatitis ‘B’ and Tuberculosis. They will be required
to provide, in advance of appointment, evidence of immunity or have a local blood test (as deemed
appropriate by the Occupational Health Department).
RESIDENCES AND REMOVAL EXPENSES
The successful candidate will be required to maintain his/her place of residence in contact with the
public telephone service and not more than 10 miles by road from the Royal Hallamshire Hospital,
Sheffield, unless prior specific approval to a greater distance is given by the Trust.
Removal expenses on appointment will be paid in accordance with the Sheffield Teaching
Hospitals NHS Trust Terms and Conditions of Service. These documents can be seen at the offices
of the Medical Personnel Department. It should be noted that it is not Trust policy to reimburse
interest payment on bridging loans.
POSTGRADUATE EDUCATION & CONTINUING MEDICAL EDUCATION
Sheffield Teaching Hospitals NHS Foundation Trust
There is an active Postgraduate Centre at Sheffield Teaching Hospitals with teaching programmes
for all grades of staff. There are regular seminars where Consultants and junior staff present cases
to hospital colleagues and General Practitioners, and occasional evening meetings to which
eminent guest speakers are invited.
There is a purpose built Medical Library within the Trust under the charge of a full-time Librarian.
There are over 15,000 volumes in stock and access is available by joint scheme with other libraries
and with the National Lending Library. The Library subscribes to over 150 journals. Computer
facilities with Internet Access and CD Roms are available in the Library.
The Trust actively encourages Directorate members to attend local and national meetings of
relevance to their speciality and is fully supportive of Continuous Medical Education. Clinical
skills laboratories are available for all staff on both sites.
FUTURE TRUST ISSUES
The Trust has been highly successful in meeting activity and financial targets, whilst meeting the
demands placed upon it from its stakeholders.
As with other hospitals, activity and financial pressures are not likely to diminish, but the Trust
feels it is in a strong position to cope with future expectations.
National tariff, patient choice, national programme for IT and the integrated care record system
along with integrated care pathways are ongoing initiatives presenting the Trust with huge
opportunities and challenges.
