CC 08/10 v1 Credit Secure Benefit Details Credit Secure is an invaluable insurance benefit, covering your credit card outstanding balance for a nominal monthly charge. In the event of Permanent Total Disability, Critical Illness or Death due to any cause (subject to the Exclusions stated below), the outstanding balance on your credit card is waived, without causing any inconvenience to your loved ones. It also provides Reimbursement of hospital expenses in the event of Accident, Payment of monthly installment in the event of Involuntary Loss of Employment or Temporary Total Disability. As a Standard Chartered Bank Credit Cardholder you can enjoy the benefits of Credit Secure Protection provided by Alico Insurance Company. We request you to go through the Cover details in order to understand thoroughly the extent and terms of the Cover offered.The Cover is provided by Alico Insurance and Standard Chartered Bank (the 'Bank') has no liability to you for Credit Secure coverage. This booklet is only a summary of the principal provisions of the master policy. SCHEME HIGHLIGHTS • 24 hour cover for Credit Secure • Easy enrolment • No medical certificate is required • Hospitalization subsequent to an accident for up to 60 days applicable worldwide • Involuntary Loss of Employment benefit for a maximum of 6 months, applicable in UAE only or • Temporary Total Disability benefit for a maximum of 6 months • Low monthly rate of 0.99% of the total outstanding balance of the previous month's statement. The mentioned rate would be calculated on the sum of the current balance and any repayment facility balance. The Credit Secure amount charged would be reflected in the following month’s statement. SUMMARY OF POLICY CONDITIONS OF MASTER POLICY NO. CL-03 1. A nominal rate of 0.99% will be the total outstanding balance on each month's credit card statement. 2. The Cardholder may opt out of the scheme at any time. However, the Cardholder will only be allowed to re-join the scheme if he is approved by the Bank. 3. The cover under the policy is applicable only for Primary Cardholders and excludes Corporate Cards. However the outstanding balance on the supplementary card is also protected in case of death, permanent total disability, Accident, Critical Illness, Involuntary Loss of Employment * or Temporary Total Disability * of primary cardholder. 4. For cover under the policy the Cardholder should be aged between 18 to 65 years (ceasing at attaining the age of 65). 5. Death, Critical Illness, Permanent Total Disability, Accidental Medical Expenses and Temporary Total Disability covers are applicable worldwide. 6. Death or Disablement cover applies to any cause except those expressly excluded in terms of the policy (see below). 7. Involuntary Loss of Employment * applicable in UAE only 8. All the benefits payable under this policy shall be paid against the Cardholder's total outstanding balance on the credit card account with the Bank. 9. The Bank is not at any time considered as an agent of Alico Insurance Company, the 'Insurance Provider'. Any claims or contestations for any insurance coverage shall be negotiated directly with Alico Insurance Company, Dubai. 10. The Bank reserves the right, at any time, to change the terms, conditions, rates and/or reject, discontinue or cancel the Cover applicable without assigning reason. 11. Credit Card holder should be a resident in the UAE at the time of sourcing the Policy. The Credit Card holder shall be covered even in the instance where the Event of Claim occurred outside the UAE (except for ILOE cover which is restricted to the UAE). DEFINITIONS Accident shall mean a sudden, involuntary, violent and external event that directly and independently of disease, aging and degenerative conditions, results in visible bodily injury or death. Accidental Medical Expenses shall mean the expenses incurred by the Cardholder during hospitalization due to Accident during the currency of the Policy. Bank shall mean the Standard Chartered Bank, United Arab Emirates Critical Illness (CI) shall mean illnesses, the signs or symptoms of which first commenced more than 30 days following the respective Insured Credit Cardholder's Entry Date and shall include either the diagnosis of any of the illnesses or performance of any of the covered surgeries contained in the Benefit Schedule. Credit card shall mean the credit cards issued by the Policy holder to the primary or supplementary Credit Card holders. Credit Limit means the aggregate maximum debit balances permitted by the Bank on the Credit Card for the Primary Credit Card holder or the Supplementary Credit Card holders, if any, and notified to the Primary Credit Card holder from time to time. Credit card holders shall mean the persons who have been granted a Credit card and fulfill the Policyholder's Credit card requirements. Charges shall mean amounts payable by the Insured Credit card holder arising from the use of the credit card or the credit card number or the PIN or under the credit card agreement and includes without limitation all credit card transactions, fees, finance charges, additional expenses, damages, legal costs, and disbursements, which will be debited to the credit card and form part of the Current Balance. Commencement Date shall mean the date the Cardholder is enrolled for this policy by the Insured or the date of inception of this policy whichever is later. Company shall mean Alico Insurance Company Confinement: shall mean admission in a Hospital as an inpatient for a minimum period of one day as defined above upon the recommendation of a Physician for medically necessary treatment of a Covered Injury and continuous stay in the Hospital prior to his discharge. Confinement will be evidenced by a daily room/room & board charge by the Hospital. Current Balance shall mean the amount stated in the Statement of Account of the Insured Credit card holder as the 'Current Balance' being the total amount due from the Insured Credit card holder for the transactions shown on the Statement of Account. Date of Loss or Date of Event shall mean: (a) For claims under Death Benefit, the date of death of the Insured Credit card holder. (b) For claims under Permanent Total Disability Benefit, the date of diagnosis of total disability. It has to be certified by a Physician and approved by the Insurance Company as fulfilling the conditions herein for a disability claim. The Date of Loss for a disability claim shall not precede the Entry Date. (c) For claims under Accidental Medical Expenses, the date of accident of the Insured Credit card holder. (d) For claims under Involuntary Loss of Employment, the date of termination of employment (e) For claims under Temporary Total Disability Benefit, the date of diagnosis of temporary disability. It has to be certified by a Physician and approved by the Insurance Company as fulfilling the conditions herein for a disability claim. The Date of loss for a disability claim shall not precede the Entry Date. Permanent Total Disability (PTD) shall mean disability from bodily injury, Sickness or disease so as to prevent the Insured Credit card holders from performing any kind of work, occupation or profession for compensation, profit or gain for the remainder of his lifetime. Day shall mean a period of 24 consecutive hours for which the Insured is confined as an inpatient in a Hospital and for which a hospital room and board or Intensive Care Unit (ICU) charge is made. Policy shall mean this agreement, any supplementary contracts or endorsements herein, any amendments hereto signed by the Insurance Company and the Policyholder and Summaries of cover, which shall together constitute the entire contract between the parties. Eligible Credit card holders shall mean Credit card holders who have met the eligibility requirements set forth in this Policy. Policy Year shall mean a period of twelve (12) consecutive months beginning with the Policy Date and subsequent Policy Anniversary. Entry Date shall mean the effective date of coverage of Insured Credit card holders hereunder, which shall be the Policy Date or the date of them becoming Eligible Credit card holders, whichever is later. Pre-existing Condition shall mean illness, disease or sickness occurring or manifesting prior to the Commencement Date, for which advice or treatment was sought or obtained from a medical practitioner, chiropractor, naturopath, or any other practitioner of a similar kind within twelve months immediately prior to the Commencement Date. Hospital shall mean an institution licensed as a hospital and operated for the care and treatment of sick and injured persons, which institution provides 24-hour nursing care and has facilities for both diagnosis and except in the case of a hospital primarily concerned with treatment of chronic diseases, for major surgery. The term hospital shall not be construed to include a hotel, rest home, nursing home, convalescent home, place for custodial care, home for the aged, or a place used primarily for the confinement or treatment of drug addicts or alcoholics. Indebtedness shall mean the total amount outstanding payable by the cardholder arising from use of the Credit Card Facility including without limitation all Credit Card transactions, fees, finance charges, additional expenses, damages, legal costs and disbursements, which will be debited to the Credit Card and form part of the Current Balance as on the Date of Event subject to a maximum of the Cardholder's credit limit. Insured Credit card holders shall mean Eligible Credit card holders who, in accordance with the provisions of this Policy, are participating in the Insurance Plan under this Policy and who have fulfilled the automatic enrollment and did not choose to opt out. Installment shall mean the minimum amount due as defined in the Policyholder's general terms and conditions for the Credit card(s). Intensive Care Unit (ICU): one to one nursing care received in a hospital unit in which patients undergo specialized resuscitation, monitoring and treatment procedures. The unit must be staffed 24 hours a day with highly trained nurses, technicians and doctors, and be equipped with resuscitative equipment and monitoring devices that allow continuous assessment of vital body functions such as heart rate, blood pressure and blood chemistry. Involuntary Loss of Employment (ILOE): ILOE shall mean the unemployment of the insured credit card holder in the UAE arising out of the unilateral decision of the employer to terminate his employment contract for any reason other than those mentioned under exclusions in the policy. Medical Expenses shall mean all expenses incurred by the cardholder during hospitalization in respect of doctors (including general practitioners and specialists even if consulted for a second opinion), prescribed medicines, x-ray and all hospital and ward fees and expenses. Outstanding Balance The Outstanding Balance means the Current Balance owed to the Policyholder as stated in the Statement of Account of the Insured Credit card holder. The Outstanding balance is frozen on one given date called Date of Loss in order to exclude from insurance benefit calculations any use of the Credit card after the occurrence of the Claim. Pronouns: Masculine pronouns used in this Policy shall include both masculine and feminine gender unless the context indicates otherwise. Sickness shall mean any pathological state or state of abnormal function of bodily organs of the Insured, not caused by an accident, and objectively diagnosable. Statement of Account shall mean the monthly statement issued by the Policyholder to the primary Credit card holder of Charges made on his credit Card. Summary of Cover shall mean the accurate information about the Policy that must be provided by the Policyholder to every Insured Credit card holder. This information shall describe thoroughly the provisions, terms and conditions of the Policy as to be well understood by the Insured Credit card holders. The wording of the Summary of Cover is prepared by the Insurance Company. Temporary Total Disability (TTD) - shall mean insured Credit Cardholder’s total and temporary disability to perform his own work as a result of accidental bodily injury, sickness or disease. The state of TTD will be exclusively recognized by a physician designated by the Insurance Company. SCOPE OF COVER 1. Death Benefit 2. Permanent Total Disability Benefit 3. Accidental Medical Expenses 4. Critical Illness Benefit 5. Involuntary Loss of Employment, or 6. Temporary Total Disability Death Benefit if an Insured Credit card holder dies due to Sickness or Accident, the Insurance Company will, upon receipt of due proof in writing of the Death of the Credit card holder, pay the Outstanding Balance on the credit card, up to a maximum of AED 100,000, to the Policyholder. The Policyholder will subsequently allocate payments to the eligible Cardholders credit card accounts. Permanent Total Disability (PTD) If an Insured Credit Card holder becomes permanently and totally Disabled as defined in this Policy, the Insurance Company will pay the Outstanding Balance on the credit card, up to a maximum of AED 100,000, to the Policyholder. The Policyholder will subsequently allocate payments to the eligible Cardholders credit card accounts. The Insured Credit card holder must be able to justify a registered occupation, whether employed or self-employed, on the first day of the Sickness or on the date of the Accident, according to the cause of the PTD. The state of PTD shall be established based on a detailed medical report from a registered and reputable treating physician of a Hospital or Clinic. The Company reserves its right to seek a second opinion at its own cost in the above case. There must be a minimum consolidation period of 6 continuous months of total disability before a PTD claim can be reported to the Policyholder and recognized. However, in case of total and irrevocable loss of sight of both eyes, or of the loss by severance of two or more limbs (at or above wrist or ankle), the consolidation period of 6 months can be waived and the state of PTD be recognized immediately. The Insured Borrower shall have to provide the Insurance Company with the evidence that he was carrying an officially registered occupation on the commencement of his Total Disability. Accidental Medical Expenses (AME) If an Insured Cardholder is hospitalised directly resulting from an accident occurring anywhere in the world, the Insurance Company shall reimburse AED 200 per day towards the in-patient treatment for up to 60 days of hospitalisation. Critical Illness (CI) If a Credit Cardholder is diagnosed having one of the stated Critical Illnesses as defined in this Policy, the Insurance Company will, upon receipt of due proof in writing of the diagnosis of the Critical Illness, pay the Outstanding Balance on the credit card as at the claim date, of up to AED 100,000, to the to the Policyholder. The Policyholder will subsequently allocate payments to the eligible Cardholders credit card accounts. This benefit cannot be cumulated with any other benefit under the Policy/Cover. The benefit payment is subject to a waiting period of 30 days continuously. The waiting period is calculated from the first day of diagnosis of the Critical Illness. No benefit is paid for the first 30 days. No Critical Illness benefit is paid if death occurs within these 30 days from the claim date (i.e. the claim may be paid therefore as a Life claim). The benefit payment is subject to a deferral of coverage period of 90 days continuously. The deferral of coverage period is calculated from the entry date. If the claim takes place within these 90 days, the claim is automatically turned down. The Covered diseases are: Cancer: a malignant tumour characterised by the uncontrolled growth and spread of malignant cells and the invasion of the tissue. This includes leukaemia (except chronic lymphocytic leukaemia), lymphomas and malignant melanomas, but excludes non-invasive cancers in situ, all skin cancers and tumours in the presence of a human-immune-deficiency virus. Stroke: any cerebrovascular incident producing neurogical sequel lasting more than 24 hours and including infarction of brain tissue, haemorrhage from an intra cranial vessel and embolisation caused by an extra cranial source. Evidence of permanent neurological deficit must be produced. Transient ischaemic attacks are specifically excluded. Coronary artery disease requiring surgery: open heart surgery undergone to correct narrowing or blockage of two or more coronary arteries by the use of saphenous vein grafts or internal mammary grafting, but excluding all non-surgical procedures such as balloon angioplasty or laser techniques. Angiographic evidence of the underlying disease must be provided. Major organ transplantation: the actual undergoing as a recipient of a transplant of a heart, lung, liver, pancreas or bone marrow. Multiple sclerosis: unequivocal diagnosis of multiple sclerosis made by a consultant neurologist holding such an appointment at a major hospital. The claimant must exhibit neurological abnormalities that have existed for a continuous period of at least 6 months or have had at least one relapse of such abnormalities. The typical symptoms of demyelination and impairment of motor and sensory function must evidence this. Involuntary Loss of Employment (ILOE) - applicable for salaried customers only If an Insured credit card holder loses employment (as defined in the policy), the Insurance company will pay to the Policyholder one monthly minimum payment due as set out in the previous month's credit card statement of account for each period of 30 days of unemployment after the waiting period. Only unemployment arising in UAE and according to a UAE contract of employment can be covered. ILOE insurance shall be subject to a deferral of coverage period of 90 days accounted only once from the date of enrolment in the policy. A claim shall be automatically turned down if redundancy or dismissal is notified to the insured credit cardholder within these 90 days. The benefit payment shall be subject to a waiting period of 30 days continuously applicable to each and any ILOE claim. The waiting period is calculated from the first day of ILOE commencement. No benefit shall be paid for the first 30 days. The benefit payment terminates at the earliest of the following dates: • • • • • • • • Heart attack: the death of a portion of heart muscle as a result of inadequate blood supply, which must occur with prolonged chest pain, new electro cardio graphic changes and an elevation in cardiac enzyme levels. • Kidney failure: end stage renal disease due to chronic irreversible failure of both kidneys to function as a result of which the person insured must be required to undergo regular peritoneal dialysis or haemodialysis or renal transplantation. • if the claim is no longer justified as required, in case of Death/Disability, when the card holder resumes work (even if it is only a part-time work), on the date of retirement or early retirement, when the maximum benefit payment term is reached, on the maximum age at claim date, when the credit card is cancelled, When the maximum benefit payment term of 3 monthly installments has been reached for any one claim; or 6 monthly installments in aggregate for several ILOE claims during the period of coverage. The period of coverage is renewed every year with the renewal of the contract between Alico and the policyholder. when the insured card holder no longer claims the Tanmia unemployment benefit (applicable to UAE Nationals only) if insured does not have a valid UAE/Gulf resident or employment visa (ex-pats), (Insured staying during the grace period post cancelation will be considered for the claim) There must be at least 6 months of continuous employment after the term of a first claim for which benefits have been paid to re-qualify for a new one. In this event only, these 6 months can be achieved either under a permanent contract of employment, or under a temporary or a fixed term contract. To report a claim, the insured card holder must meet the following conditions: o Being less than 65 years old on the notification of unemployment, o 6 continuous months of permanent gainful contract of employment of not less than 30 hours per week with the same employer in the private/public sector prior to the claim date [selfemployed are not entitled to claim under this ILOE coverage](*), o being unemployed as a consequence of redundancy or dismissal (other than due to a reason of misconduct), (•) the eligibility to ILOE depends on conditions that have to be met on the claim date, irrespective of what the occupation of the card holder was when he applied for insurance. Maximum benefit payment will be 3 monthly installments per ILOE claim or 6 monthly installments in aggregate for several ILOE claims during the period of coverage. Temporary Total Disability (TTD) - applicable for self employed customers only If an Insured Credit Cardholder becomes Temporary and Totally Disabled as defined in this policy, the Insurance will pay to the Policyholder one monthly installment for each period of 30 days of proved TTD after the waiting period of 60 days, subject to a maximum of 6 months. No benefit is paid for the first 60 days. The benefit payment terminates at the earliest of the following dates: • when the claim is no longer justified as required from a medical point of view, • in case of death or PTD of the insured Credit Cardholder, • on the 65th birthday of the insured Credit Cardholder, • when the insured is able to resume work (even if only partially), • in case of retirement, including early retirement • when the maximum number of TTD benefit payments are settled. ,when the maximum benefit payment term of 6 monthly installments has been reached for any one claim during the period of coverage. The period of coverage is renewed every year with the renewal of the contract between Alico and the Policyholder. The insured credit cardholder has to justify that: • He was carrying a registered occupation on the first day of disability and was not disabled on the first day of enrolment hereunder, whether under a permanent gainful contract of employment (or under a fixed term contract of employment,) or self employed • He had to stop working totally as a result of sickness or accident and that he is still unable to resume work when the claim is reported. In case less than 2 months are elapsed between two TTD claims, the two claims are assumed to make only one (but the waiting period is not applied then to the second TTD claim occurred). SUMMARY OF POLICY EXCLUSIONS 1. Any Death, Disability or illness caused by or resulting, directly or indirectly, wholly or partly, from any of the following factors will not be covered by this insurance: a) Illness occurring within 30 days of the commencement date; or b) Illness due to pre-existing condition but this exclusion shall not apply in respect of Death occurring after a continuous period of 6 months from the Commencement Date in respect of the particular Cardholder. However, this exclusion is applicable only if the Company is able to substantiate 'pre-existing condition' within 1 month from the date of submission of all required claims documentations to the Company; or c) Death by suicide during the first 12 months from date of joining the scheme; or d) Self-destruction or self-inflicted injury, while sane or insane; or e) Normal pregnancy, childbirth, abortion or miscarriage, or any complications thereof ;or f) Chronic alcoholism or, abuse of alcohol or, abuse or addiction to drugs; or g) Civil war, war, invasion or warlike operations, act of foreign enemy, hostilities, revolt, mutiny, riots, strike, civil commotion, rebellion, revolution, insurrection, acts of terrorism to such a degree and extent of the involvement or engagement of the Insured in these conditions without any cause; or h) Participation in any professional sport or any sport involving a motor engine (including rallies), boxing , scuba / skin diving, parachuting or hang ñ gliding, horse racing and mountain climbing or i) Military Service in the Armed Forces or Security Forces of any country or any authority; or j) The condition of Acquired Immune Deficiency Syndrome (AIDS), as defined below, or any AIDS Related Illness or virus HIV; or k) The commission of or attempted commission of an assault or any unlawful act, or being engaged in any illegal activity or felony. l) Any accident occurring on or in or about any aircraft other than an aircraft in which the Cardholder was travelling as bonafide passenger and which is operated by a licensed commercial or chartered airline. However this exclusion is not applicable to Cardholders who are air crew and pilots of airlines unless the event occurs in or on or about any aircraft. 2. And specifically, regarding Critical Illness: Same as for Death & PTD plus • critical illness which is diagnosed or undergone during the deferral of coverage period, • critical illness which the Insured Credit Cardholder was known to have suffered, or surgery which has been diagnosed as being required or has been undergone, prior to the application date (the suffering or undergoing of a heart attack, coronary artery disease requiring surgery, heart transplant or stroke is considered to be the same condition), • critical illness or surgery which arises directly or indirectly from any condition from which the Credit Cardholder was known to be suffering prior to the application date, 3. And specifically regarding Hospitalization, the consequences of: • Motor cycling as a driver or passenger on machines with more than 150 cc engine capacity or as a driver if the cardholder does not have a valid motor cycle license; or • Mountaineering or rock climbing that uses ropes or guides, bungee jumping, scuba diving, pot holing or organized team sports. 4. And specifically, regarding TTD: same as for Death & PTD, plus • any mental condition, including but not limited to nervous breakdown, depression, anxiety or other stress related condition, • backache and related conditions, unless there are definite clinical findings, such as radiological medical evidence of abnormality. 5. And specifically, regarding ILOE: same as for TTD, plus • ILOE which is notified during the deferral of coverage period, • The severance or the natural expiry of a fixed term contract of employment or of an interim contract, Early retirement, If the insured credit cardholder is not able to claim the Tanmia unemployment benefit (for UAE Nationals only), Dismissal or redundancy when the employer is a next of kin of the claimant, or when the claimant is a shareholder of the company or a member of its board, Resignation or voluntary redundancy, including cases of rejection of other suitable job in company due to organizational changes, Self-employment, When the insured no longer holds a valid UAE/Gulf Residency or Employment Visa (expatriates) Dismissal by reason of misconduct, strikes, lockouts or other organized labor disputes or any unlawful acts, partial, seasonal or casual employment, Emiratization for • • • • TERMINATION OF INSURANCE The insurance of an individual shall automatically terminate at the earliest time below: 1) The Insured Credit card holder reaches age 65 or Critical Illness; or 2) Upon payment of the Death or Permanent Total Disability Benefit; or 3) Upon cancellation by the Policyholder of the Credit Card agreement of the Insured Credit card holder, whatever the reason; or 4) If the Credit Card agreement is not renewed, or early terminated, by the Insured Credit card holder; or 5) Credit Card holder ceases to be enrolled in the insurance program; or 6) Cancellation of the Policy; or 7) The transfer of the Insured Credit card holder's indebtedness to another person; or 8) The failure of the Policyholder to collect premium from the Insured Credit card holder; or 9) The date on which payment of the Installment under the Credit Card become overdue by 180 days or a complaint or suit is filed with regard to non-payment or the Credit Card Holder is declared as absconding or a judgment is entered in any court with respect to his indebtedness under the Credit Card. 10) If the Credit Card holder so chooses to do so, he/ she can opt out of the insurance program during the first 14 days of enrollment and have the entire initial paid premium refunded. for • • • • • • • • • • • • • • GOVERNING LAW The provisions and terms of this policy shall be construed in accordance with the laws applicable in the United Arab Emirates and the UAE courts shall have the sole jurisdiction in respect of any controversy / dispute arising there from. CLAIMS PROCEDURE 1. Upon happening of an event giving rise to a claim under the policy, the Cardholder / Cardholder's Legal Representatives shall follow the following procedure: (i) Give immediate written notice to the Company but not later than 60 days from the Date of Event; In the event of accidental hospitalization the company should be notified within 48 hours from the time of event. (ii) The Cardholder or the Cardholder's legal representative shall complete the standard claim form issued by the Company and submit the same with such evidence to substantiate the claim to the satisfaction of the Company as the Company may reasonably require; (iii) The Cardholder or the Cardholder's legal representative shall submit the following documents (and any additional documentation that may be requested by the Claims Department of the Insurance Company).: • Death Claims Death Certificate. Post Mortem Report (wherever legally required). Police Report (in case of accidental death). Medical Report from an authorized medical practitioner with Detailed Diagnosis and Cause of Death if required by the Company when Actual Cause of Death is not clearly mentioned in the Death Certificate. Copy of Passport with Visa page. for Temporary and Permanent Total Disability Claims' • Disability Certificate from an authorized medical practitioner to assess disability. • Police Report (if disability is due to an accident). • Medical Report from an authorized medical practitioner with Detailed Diagnosis, Cause of Disability and Details of Treatment given. • Copy of Passport with Visa page. Accidental Medical Expenses Claims Certificate of Insurance and Declaration form Police Report (if any) Medical Report Discharge Summary with the final invoice. Any other documents as may be required for Critical Illness Claims • Medical report (from an Authorized Medical Practitioner) diagnosing critical illness • Police report (if critical illness is due to an accident) • Copy of passport with visa page.
