Family enterprise questionnaire Maize Action Research, August - September 2011 Introduction - - - - This questionnaire focuses on the activities and experiences of family maize enterprises member to one of the PO’s participating to the self-assessment. All the participants of the self-assessment are asked to fill in the actual sheet after having participated to the self-assessment. The questions focuses on the production, storage-conservation, processing and marketing of maize as well as on the collection and use of economical data by the family enterprises. This survey enables FUPRO to have data on 360 family enterprises (24 PO x 15 members) You find two types of questions: a. Questions formulated like statements – to answer YES or NO b. Open questions – to be answered with quantitative data (quantity, price, percentage) The data of this questionnaire are to be transferred to the Excel sheet 3 – Data on maize family enterprises. The data entry enables to develop a big database and do analysis Facilitators: Keep the questionnaire and hand over later to FUPRO Check whether all the sheets indicates the region and the name of the organisation Number the sheets from 1 to 15. That makes each sheet unique (region-name of the OI and number). This is also aligned with the database structure.
