With today’s shorter hospital stays, many patients benefit from the services of home health care, which may include nursing services, rehabilitation services, social work, or home health aide services. Under the direct supervision of a physician, an individualized plan of care is developed for each patient. Duke HomeCare & Hospice is accredited by the Joint Commission. Our expert clinicians collaborate with the patient and their physician to develop an appropriate and comprehensive plan of care. Specialized nursing services include wound care, ostomy care, and infusion therapy. Nursing assistants provide help with personal care in collaboration with nursing and therapy services. Rehabilitative Therapy Rehabilitative services are provided by physical, occupational, and speech language therapists. Our therapists educate patients and help them regain their maximum physical capabilities and achieve their highest level of independence. They may assist patients in day-to-day activities and educate them on creating a safe home environment. Speech therapists may also assist patients in regaining their communication ability. Occupational therapists help patients achieve independence in their activities of daily life. Social Work Our social workers help patients and families identify appropriate community resources that may assist in their recovery and their ability to remain in their homes. Service Area Duke Home Health serves patients within 60 miles of Durham, including all or parts of the following counties: Alamance, Chatham, Durham, Franklin, Granville, Orange, Person, Vance, and Wake. Paying for Care Duke Home Health care may be paid for through private insurance, Medicare, Medicaid, or self pay. How to Make a Referral To make a referral, call 919-620-DUKE (3853) or 800-599-9339. Referrals are taken 24 hours a day. The following information is helpful at the time of referral: • Patient name • Physician’s name and phone number • Insurance information • Caregiver or family contact name and phone number
