Syracuse University is pleased to offer eligible students the opportunity to enroll in an affordable,
comprehensive student health insurance plan for the 2016-2017 policy year.
• Aetna’s national Preferred Provider network: Students will be able to access participating providers
whether at home, school, or throughout the country
• Fully Affordable Care Act (ACA) compliant Platinum Plan with an unlimited policy year maximum
Enhancements have been made to the 2016-17 Plan, such as:
• Out-of-Pocket maximum for Preferred Care is $4,000 Per Person, Per Policy Year.
• In-Network Per Insured, Per Policy year deductible of $100 or $500 Out-of-Network*
• In-Network Coinsurance at 10% Preferred Allowance & Out-of-network Coinsurance at 40%
Reasonable & Customary. Deductible applies.
• $0 copay for In-Network Ambulatory Surgery Center, Outpatient Hospital Surgery Facility
• In-Network Prescription drug coverage with copays of $15 generic / $30 brand name / $50 noformulary brand
• $0 Emergency Room copay for both In-Network and Out-of-Network
Additional features:
• In-Network Physician’s Office visits covered at 100%, after $25 copay, deductible does not apply*.
• Covered services provided at the on-campus Health Services are covered at 100%; Copays apply to
prescription drugs dispensed at Health Services.
• In-Network Only - Preventive care services* with no deductibles, copays or coinsurance.
• Pediatric Dental & Vision coverage up to age 19.
• Worldwide Travel Assistance including Medical Evacuation, Repatriation, & Political & Natural
Disaster coverage*
• $50,000 Accidental Death & Dismemberment Coverage*

For 2016-17, all full-time, matriculated undergraduate, graduate and law students will have the charge of
$2,230 for the Student Health Insurance Plan added to their bill from the Bursar’s Office, but it can be
removed if the students provide proof of qualifying coverage through MySlice by September 6, 2016. Log in
using your Net ID and password to access the enrollment and waiver forms.
