I hereby certify that I am the owner of ________________
_______, the pet(s) which will participate in grooming
services at PawFection. PawFection has my permission to do whatever it feels is humane for the well-being of my pet. PawFection
will not be responsible for any conditions or problems discovered during grooming. If my dog gets injured during grooming services
and veterinary care is needed, my dog will be taken to Animal-Am Emergency Clinic of Pasadena, PawFection will notify you of such
injury. Should veterinary care be necessary after as a result of any grooming services rendered by PawFection, you must notify
PawFection within 24 hours of veterinary care, I agree to pay all veterinary costs. Reimbursement for such care will be at the sole
discretion of PawFection.
PawFection is not held responsible for before and after skin infections/irritations including but not limited to clipper burn,
shampoo allergies, brush-burn, minor nicks, hand stripping, ear hair plucking, scrapes or cuts from a matted pet or pre-existing scabs
and or moles, de-matting, clipping procedures or problems uncovered on a badly matted or otherwise neglected coat including, but
not limited to: itchiness, skin redness or self-inflicted irritations/abrasions from excessive external rubbing; nor will we be held
responsible for stressful effects that grooming may have upon a geriatric pet.
I understand that time and costs associated with de-matting are unpredictable and subject to the particular condition of my
pet; consequently, I agree to pay whatever fees are incurred as a result of de-matting. Whether a pet is “matted,” furthermore, is at
the sole determination of PawFection. I agree to pay all costs for the grooming of the pet including, but not limited to, special
handling requirements, de-matting, and or flea and tick prevention. I hereby certify that my pet(s) has been fully vaccinated against
Rabies, I alone assume the responsibility of such exposure to all canine/feline infectious diseases. I agree to keep PawFection up to
date on current vaccines by via-email, fax or bringing in the vaccine certificates.
I agree to inform PawFection prior to grooming if the pet has ever bitten any human or another pet or has any aggressive
tendencies whatsoever. I understand and agree that I will be held solely liable for any harm, injury, or property damage caused by
my pet and pay for all costs. PawFection reserves the right to refuse to groom any pet for the health and safety of the groomer and
the pet. A soft muzzle may be used, or services discontinued or refused if PawFection determines that a pet presents an unsafe
condition. Such conditions include, but are not limited to: behavior issues, health problems, fleas or parasites.
I agree to be ON TIME or up to 10 min. early for my appointment. If I am over 10 min late, I understand that I may be
required to re-schedule. I agree to pick my pet up at the arranged time. There is a $7 early drop off charge for every hour before
your scheduled appointment time. You are given 1 hour after we call to pick up your pet, after the hour there is a $7 charge for
every hour after that has passed. I fully intend to pick up my pet on time. I will notify PawFection of any changes to the pickup and
assume full responsibility for any additional charges that will apply. Abandonment is determined after 14 days from schedule pick up
date and or the refusal to provide or perform the legal obligations for care of the pet by its owner or agent. Abandonment shall
automatically relinquish all rights and claims by the owner to the pet.
I have read and understand this release form and I will honor and abide by the terms and conditions set forth above. I agree
not to initiate any legal proceedings of any sort against PawFection as a result of illness, injury, damage or death to owner’s pet
while staying at PawFection in absence of gross negligence and agree that, should the owner bring a legal action against PawFection,
the correct result of such a legal action should be a summary dismissal in favor of PawFection and the owner will also agree to pay
PawFection costs, including, but not limited to attorney’s fees.
