Foot Care Nurse (Part-Time) The Northumberland Family Health Team (NFHT) is a non-profit, primary health care organization providing over 26,000 patients with timely access to a comprehensive range of services and programs aimed at managing and preventing disease and improving health status. The inter-professional team includes 21 family physicians and 15 professional staff, including nurses and nurse practitioners, occupational and physical therapists, dietitians, social workers, pharmacist, chiropodist and respiratory therapist, who work together to provide person-centred, effective, collaborative and integrated care. For more information about our organization, please visit www.nfht.ca. An exciting opportunity is now available for 1 Foot Care Nurse (part-time – 2 days/week) to join our NFHT foot care program. Together with the Chiropodist, the program focuses on providing foot care to those requiring assistance with maintenance of overall foot health, prevention of complications such as diabetic ulcers, infections, osteomyelitis and amputations, and treatment of pathological conditions. Responsibilities Collaborate with the patient, chiropodist and the broader interprofessional team to provide comprehensive care and communication regarding foot care Conduct all aspects of foot care services in accordance with the highest quality of care and standards of practice established by the College of Nurses of Ontario (CNO), the policies and procedures of the NFHT and other relevant professional standards and guidelines Ensure all supplies and related equipment are properly maintained, cleaned and available Ensure infection control standards are followed in relation to the CNO, Ministry of Health and Long Term Care (MOHLTC), the Provincial Infection Diseases Advisory Committee (PIDAC) and the NFHT Maintain accurate and timely patient records and documentation according to CNO Practice Standards Provide health teaching for best possible foot care management and self-management Conduct outreach services and group education Participate in foot care program evaluation and quality improvement activities Qualifications, Skill and Experience Registration with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse Advanced Foot Care Certificate Recent experience as a practicing foot care nurse (preference to those with diabetes foot care experience) Current Ontario Drivers’ License, access to a reliable vehicle and willingness to travel Experience with clinical electronic medical record/scheduler an asset Demonstrated ability to work independently and collaboratively in an interprofessional environment Please visit www.nfht.ca for further information Interested, qualified candidates are asked to submit a resume to Adrienne Bell-Smith, Clinical Program Manager at [email protected].
