ANIMAL CARE AND USE PROGRAM REVIEW FORM (for interim visits) SECTION 3. ANIMAL USE PROTOCOL FORMS Please append ALL FORMS (teaching, research, breeding colonies, field studies, invertebrates (protocols are not required for invertebrates other than cephalopods), genetically engineered animals) used in your institution for protocol review, protocol amendments and protocol renewals, along with any instructions to protocol authors. All protocol forms must be based on section 3c of the 2006 CCAC policy statement on: terms of reference for animal care committees, and the following elements must be clearly included: i) ii) iii) iv) v) vi) vii) qualifications/training of the personnel involved (section 3c)ii) of the 2006 CCAC policy statement on: terms of reference for animal care committees) merit of the protocol (scientific, pedagogical or regulatory, sections 3c)vi), 3c)vii and 3c)viii)) a summary of the protocol in lay terms (section 3c)ix)) detailed information on applying the Three Rs to the protocol (section 3c)xii)) detailed information on the definition and application of appropriate endpoints for the protocol (section 3c)xiv)) specifics related to field studies, where applicable (section 3c)xv)) specifics related to methods of euthanasia, and to the fate of the animals when they are not euthanized (sections 3c)xvi) and xvii)) With respect to protocol renewals, they can be submitted either on a separate, shorter form, or on the complete protocol form, as long as the following elements are included (section 3g of the 2006 CCAC policy statement on: terms of reference for animal care committees): i) ii) iii) iv) v) the number of animals used in the preceding year; the number of animals needed for the year to come, with a justification; a brief progress report, describing any complications encountered relative to animal use (unpredicted outcomes, and any animal pain, distress or mortality), any amendments to the original protocol, and any progress made with respect to the Three Rs of replacement, reduction and refinement of animal use; a brief report on the adequacy of the endpoints for the protocol, and on any complications encountered or refinements made relative to protecting animals from pain, distress or mortality; any other changes from the original protocol.
