Represented by Full name and title Mailing address Company name Full name and title Street City Postcode Contractual Stipulations The Parties have agreed that T-Mobile will set up a connection to the Customer’s IP network for the Customer, using the access point as specified in detail in the instructions provided by the Customer in an annex to this Contract within 20 days following the date on which this Contract is entered into. The T-Mobile’s obligation to set up the connection corresponds to the Customer’s obligation to pay the fee specified in the annex to this Contract for the set-up of this service as well as to meet other obligations specified in this Contract. The provision of data services shall be governed by the provisions of the Subscriber Contract. Matters that are not explicitly regulated by this Contract shall also be governed by the provisions of the Subscriber Contract (e.g. the provisions related to entering into and terminating the contract, maturity period, delivery of correspondence, and all further applicable provisions). The Customer expressly confirms that he/she has read and agrees also to the General Terms and Conditions of T-Mobile Czech Republic a.s. and the Terms and Conditions for Processing of Subscribers' Personal, Identification, Traffic and Location Data effective from 1 July 2012, which will form an integral part of the Subscriber Contract from 1 July 2012. The Customer shall ensure that all the technical prerequisites for proper data interconnection are fulfilled on its part. In the event that the Customer does not use the services (e.g. due to an interruption in their provision – the suspension of a SIM card, etc.), the Customer is obliged to remove the relevant SIM card from the terminal equipment. At the same time, the Customer is obliged to prevent the generation of excessive traffic within the T-Mobile electronic communications network that would be caused by faulty set-up of its terminal equipment and other technical components. Any breach of these obligations shall be considered misuse of the services in terms of the General Terms and Conditions of TMobile Czech Republic a.s. The Customer undertakes to inform the Users of the conditions related to the use of the Mobile Intranet service. The Customer appoints a Systems Solutions Administrator to act on behalf of the Customer in all matters related to the Mobile Intranet service. T-Mobile undertakes to provide the Customer with 24/7/365 technical support on the above specified telephone number, and to remove any defects in the shortest time possible.
