Job Description
Job Summary
As part of the clinical nurse specialist/Palliative Care team, to provide specialist
care in symptom control, social, emotional and spiritual care to patients and
families.
Key Tasks
1.
To provide direct specialist care to patients in the community and hospital
ensuring that needs and choices are met and optimum care is provided,
acting as a key worker and patient advocate
2.
To provide specialist advice, support and education to the primary
healthcare team and hospital staff, patients and families where
appropriate ensuring consistently high quality end of life care
3.
To play a key role as a member of multi disciplinary teams presenting cases
clearly and professionally ensuring contribution to effective patient
management.
4.
To advise on a range of end of life care interventions in the community
ensuring patients are able to receive their care at home when this is
requested.
5.
To liaise effectively and collaboratively with community based healthcare
professionals from all sectors ensuring accessibility to appropriate levels of
care and expertise for patients.
6.
To make autonomous and individual clinical decisions on case load
management in line with clinical guidelines and procedures.
7.
To support the development of the Gold Standards Framework with GPs
and care homes ensuring consistently high standards of end of life care
throughout the community and hospital
8.
To make an effective contribution to clinical audit and service reviews
including engaging with patients and members of the public to ensure
patient focussed care
9.
To provide direct and signposted pre and post bereavement support up to
level 2 NICE guidelines ensuring that appropriate support is provided
10. To support the promotion and development of end of life care tools
ensuring consistently high standards of end of life care locally.
11. To assist with the development of policies, procedures and guidelines
ensuring that high standards of end of life care are applied and accessible
throughout the communities, hospital and for other stakeholders. To ensure
that accurate records of care are maintained according to standards,
with appropriate details and in line with legal requirements. To support and
contribute to data collection (qualitative and quantitative) in order to
provide evidence of quality, productivity and outcomes.
12. To actively keep up to date with developments and advances in palliative
care by participating in educational opportunities. To continue to develop
clinical expertise, specialist knowledge and skills to influence, enhance and
advance palliative care and end of life services.
13. In order to comply with the Health Act 2006 (Code of Practice for the
Prevention and Control of Health Care Associated Infections) it is the
responsibility of each member of staff to prevent and control the spread of
infection following the policies which apply at the location where they are
working. These are available in Policies and procedures folders at the
Hospice or on the Hospital intranet in the document library in the section for
clinical policies and guidelines – infection control
To undertake all duties in accordance with the law, hospice philosophy,
recognised professional guidelines, including compliance with cancer peer
review measures and the hospice commitment to equality of opportunity and
collective responsibility.
To participate in service reviews and annual reports and take advantage of
opportunities for development as necessary

Education and
Qualifications
NMC registration.
Diploma/Degree in Palliative Care or commitment to undertake post
registration study/qualification in palliative care
Advanced communication training
Mentorship training/education

Experience
Registered Nurse with minimum of 5 years post registration experience
Proven palliative care experience working at Agenda for Change
Band 6/equivalent or above
Experience of effective multi disciplinary working

the community
Working awareness of national requirements and local plans in
relation to cancer
Understanding of the Healthcare environment and changes in
