current Health Trainer Programme.
The contract is a combined health trainer and lifestyle hub service. It commenced
01/04/2015, contract duration is initially 2 years, with the option to extend for a
further 3 years. The current contracted provider is Parkwood Healthcare Limited.
The total contract value is £399,000 per annum (£100,000 from CCG).
Typical outcomes are to provide an effective, culturally appropriate and reliable ‘one
stop’ referral service for GPs and other health and social care professionals to
access lifestyle support services. Provide the Health Trainer service as additional
support, when necessary, to clients from the priority areas specified to make
positive behavioural changes to improve their health and wellbeing.

current Weight Management Programme.
Leicester City Council have 2 weight management programmes, a universal service
– that commenced on 01/04/2015 till 31/02/2017. The contract value is £98,655
per annum. The current contracted provider is Weight Watchers UK Limited. Also
a targeted and enhanced adult weight management service that commenced
01/04/2015 initially for 2 years, with the option to extend for a further 2 years. The
contract value is £229,790.00 per annum, the current contracted provider is
Leicestershire Partnership NHS Trust.
Typical outcomes for both services are to implement an innovative and accessible
tier 2 universal adult weight management service for overweight and obese adults
aged 16 and over within the locality targeting those who do not typically access
weight management services or may need additional support. This forms an
integral part of the weight management care pathway.

current NHS Health Check Programme.
Leicester City Council commission NHS health checks as part of the community
based services framework since 1st April 2015. The framework lasts for 4 years till
31st March 2019 and the current commissioners are Leicester’s 62 City GPs. The
maximum contract value for this service is £548,680 and typical outcomes are to
reduce levels of Cardiovascular Disease (CVD) risk and associated premature
mortality in Leicester City through primary care; ensuring systematic and proactive
identification and management of patients.
