Position Description Position Title: Community Lactation Services Coordinator – Pouawhi Ora Business unit: Bay of Plenty Location: Based in Area Office, Tauranga Reports to: Clinical Leader Last review date: 04 October 2016 Business Unit overview The Operations Business Unit ensures Plunket’s relevance to all communities, in line with Plunket’s organisational goals, vision and strategic direction. It holds collective accountability for the delivery of all Plunket Services, the leadership of national and local initiatives and contribution to the wider leadership team of Plunket to ensure the achievement of Plunket’s vision and strategic goals. This will be achieved through engagement, service delivery, service development and innovation of service, funding stream development and continuous improvement. The business unit is responsible for working with the relevant Plunket Boards, Plunket Groups and external stakeholders to provide relevant services to improving the health outcomes for children and their families. Purpose of position To increase rates of breastfeeding, particularly amongst key population groups Maori, Pacific and low socio-economic To establish, lead and coordinate the BOP community breastfeeding initiative and community breastfeeding clinics, in high-need communities To support and promote breastfeeding through community and peer based support, and engagement with wider family/whanau relationships To work with health providers and community workers to increase knowledge and support for breastfeeding in the community To provide a community-based lactation consultant service that targets Maori woman To provide clinical oversight and support for the Mother-to-mother Breastfeeding Peer Support programme To facilitate bonding and attachment between baby and mother, and wider whanau Dimensions of the position Responsible to: Clinical Leader Responsible for: Nil Financial delegations: Nil Functional Relationships Internal: All Plunket staff, Community Volunteer and Clinical External: Women, partners and families/whanau SUDI Project Manager Portfolio Manager Maternity, Child and Youth Lactation Consultant / BFHI Coordinator Clinical Maternity Manager Professional Midwifery Advisor Director of Nursing and Midwifery Clinical Midwife Educator GP services Iwi Services Lead Maternity Carers Well Child / Tamariki Ora providers Primary Care / General Practice Community groups, voluntary and support agencies Health and non-health providers of services to women and babies Key accountabilities (Some will be unique to the position; others will be common across the organisation and level within the organisation.) Business as usual Establish and coordinate community-based drop in breastfeeding clinic(s) at various locations that target Maori women and babies and women living in low socio-economic areas Provide and coordinate lactation consultant services at the clinic(s). Provide clinical oversight and coordinate training to support the mother-to-mother Peer Support Administrators. Support the mother-to-mother Peer Support Administrators to: - Identify, recruit, train and support appropriate Peer Supporters. Establish and coordinate local support groups for mums with infants and toddlers. Engage and involve the wider whanau (particularly grandmothers and fathers) in supporting breastfeeding. Build the capacity of and provide on-going training and development to the Peer Supporters. Develop toolkit/resources for the mother-to-mother Peer Support Administrators and Peer Supporters. Establish, lead and support a mother-to-mother breastfeeding peer support 2 programme aimed at Maori, Pacific and low socio economic families. Maintain systems that support the mother-to-mother Peer Support Administrators and Peer Supporters (i.e. regular meetings, clinical supervision, reporting templates, work plans). Provide Community Lactation Coordination Services Provide support for complex breastfeeding cases, breastfeeding clinic(s), in mother’s homes or other appropriate venues. Provide updated breastfeeding training for the mother-to-mother Peer Support Administrators and Peer Supporters. Maintain annual practicing certificate and IBCLC certification. (Or obtain IBCLC) Record Keeping Complies with all Plunket policy to create and maintain full and accurate records. Leadership Workforce development Provide workshops, education and resources to health providers, primary care, Well Child, Maori health providers, and others working in the community to increase knowledge and build capacity to support women with breastfeeding Quality & Risk Patient safety is paramount to the service we deliver at Plunket. This is achieved in a clinical governance framework identifying and managing risk and opportunities to improve. Proactively encourage all staff and lead by example implementing a culture of continuous quality. Identify all risks that will prevent Plunket from achieving their goals. Report and manage risks appropriately. Put the client at the centre of all improvement. Use evidence based improvement methodologies when appropriate for all improvements. Promote Certification where appropriate to showcase quality. Health & Safety Each individual is responsible for ensuring the safety of themselves, their colleagues, patients and their visitors and to comply with all organisational health and safety policies, procedures and guidelines. Relationships are established and maintained with Iwi services in the planning and delivery of services. Demonstrates knowledge of, and practices in a manner that is consistent with, the Treaty of Waitangi in the provision of health care services and support to Maori clients and their whanau. Assists in the facilitation of safe services to Maori, including access to Maori treatment options and involvement of whanau in planning and delivery of care. Scope of duties The job holder is expected to perform such other duties as can reasonably be regarded as incidental to the position description, and such other duties reasonably within their experience and capabilities as may be from time to time assigned following consultation. 4 Once appointed a programme of work will be developed with the appointee that will set out the specific tasks and time frames to achieve the key deliverables for this position. Progress will be monitored through Plunket’s Performance and development programmes. Person Specification Knowledge, Skills and Experience (including Technical Competencies) Qualifications Holds, or studying towards a qualification or commitment to complete IBCLC (International Board Certified Lactation Consultant) that will be complete within two years of employment Relevant nursing qualification, Midwifery, or work history in WCTO Current drivers licence If nurse current APC Evidence of an interest in post registration study, or to undertake further study as applicable Skills & Experience Ability to build and maintain relationships Basic computer skills Excellent interpersonal skill Experience in adult education Innovative / proactive change agent Ability to communicate effectively at all levels Ability to work independently Demonstrated ability to own professional development Current full NZ driver’s license Non-smoker preferred Other Requirements Knowledge of Treaty of Waitangi and its application to the health setting Privacy Act (1993) and Health Information Privacy Code (1994) Quality & Risk Patient safety is paramount to the service we deliver at Plunket.
