Rochdale Urgent Care Centre, Rochdale Infirmary. This centre is open 24 hours, 7
days a week and consists of a team of GP’s and Advanced Nurse Practitioner.
This practice is within the Heywood, Middleton & Rochdale CCG

Hopwood Medical Centre serves the whole of Heywood, Middleton and
Rochdale.
The area served by Hopwood Medical Centre is in the district covered by
Heywood, Middleton & Rochdale CCG.
Our team includes 3 General Practitioners, 1 Senior Practice Nurse, 2 Health
Care Assistants as well as our Practice Manager and Reception Staff.
Heywood, Middleton & Rochdale CCG is responsible for ensuring you receive
all the services you need.
We offer a full General Practice Service and run a range of specialist clinics for
Chronic Disease, Family Planning, Drug Addiction, Well Person and Travel
Clinics.
At Hopwood Medical Centre, we aim to treat all our patients promptly,
courteously and in complete confidence.
Hopwood Medical Centre is a ‘training practice’. This means hospital doctors
wanting to enter general practice spend 3 to 12 months with us in order to
gain the experience they need to become family doctors. The practice is also
involved in the education of medical students.
This leaflet is for both existing patients and those considering registering with
us. It tells you about our services, how to access them and some general
information about how our practice operates.
If you live in our practice area and would like to register with us, please
complete one of our registration forms that are available from our receptions
desk or on our practice website.
Complaints
Hopwood Medical Centre aims to give a friendly and professional service
to all our patients. However, if you have any concerns about any aspect of
our service, please let us know.
Hopwood Medical Centre operates a Practice Complaints Procedure
as part of the NHS system for dealing with complaints. Our system
meets National Criteria.
Our Practice Manager Mrs. A. Hardacre will give any further
information needed and a Patient Information Complaint Leaflet is
available at the Reception desk.
The NHS Greater Manchester Support Unit also operates a complaints
service.
Patients Services can help resolve problems with any service provider
such as your dentist, hospital or pharmacist. To speak to a Patient
services Officer please telephone 0800 121 4430.
Our Team
Dr. E. S. Osborne (Female)
MRCGP
Dr. Osborne has been with the practice since 1996. She offers a full range of
general practice care to her patients. Dr. Osborne is involved in the training and
supervision of medical students and junior doctors.
Dr. Osborne works Wednesday mornings, Thursdays and Fridays.
Dr. Z. Mohammed (Male)
MRCP MRCGP
Dr. Mohammed has been with the practice since 2006. As well as providing a
full general practice service he has a special interest in minor surgery and is
involved in the training of medical students and provides supervision for the
GP Registrar and is also the Medical Director for BARDOC.
Dr. Mohammed is a fluent Urdu and Punjabi speaker.
Dr. Mohammed works Monday, Tuesday, Thursday and Fridays.
Dr. Y. Shabbir (Female)
MRCCP
Dr. Shabbir has been with the practice since 2004. Dr. Shabbir offers a full
range of general practice care to her patients and specialises in sexual health.
She speaks fluent Urdu and Punjabi.
Dr. Shabbir works Monday, Tuesday and Wednesdays.
Other Local NHS Services
As well as our practice, there are many other local NHS services you can
contact for health advice, information or treatment. Before you do, remember
that you can treat many minor ailments such as colds, coughs and indigestion by
keeping a well-stocked medicine cabinet at home. We suggest you keep the
following:
■ Paracetamol and aspirin (children under 16 and people with asthma
should not take aspirin)
■ Mild laxatives
■ Anti-diarrhea medicines
■ Rehydration mixture
■ Indigestion remedy (for example, antacids)
■ Travel sickness tablets
■ Sunscreen – SPF15 or higher
■ Sunburn treatment (for example, calamine)
■ Tweezers and sharp scissors
■ A thermometer
■ A selection of plasters, non- absorbent cotton wool, elastic
bandages and dressings.
Remember
■ Keep the medicine chest in a secure, locked place out of reach of small
children.
■ Always read the instructions and use the suggested dose.
■ Watch expiry dates – don’t keep or use medicines past their
sell-by date.
■ Take all unwanted and out-of-date medicines back to the pharmacy.
Your Local Pharmacist
Patients with Particular Needs
Your local pharmacist will be able to give you free health advice at any time
and you don’t need an appointment. Many pharmacies operate extended
hours on a rota basis. Call NHS Direct for details.
Hopwood Medical Centre is accessible to patients using a wheelchair.
NHS Walk-in Centre
You can also see an experienced nurse for treatment of minor injuries and
illnesses, seven days a week

For patients with hearing problems, we have a hearing loop located on
reception.
We can arrange interpretation and translation services in person or by phone
for patients who do not speak English. Please let us know if you need this
service when booking an appointment.
NHS 111 is a new service that's being introduced to make it easier for
you to access local NHS healthcare services.
Patient Confidentiality
You can call 111 when you need medical help fast but it’s not a 999
emergency. NHS 111 is a fast and easy way to get the right help,
whatever the time.
We respect your right to privacy and keep all your health information
confidential and secure.
NHS 111 is available 24 hours a day, 365 days a year. Calls are free
from landlines and mobile phones.
It is important that the NHS keeps accurate and up-to-date records about
your health and treatment so that those treating you can give you the best
possible advice and care.
Accident and Emergency/999
Whatever the day or time, if you or someone else experiences severe chest
pain, loss of blood or suspected broken bones, go to your nearest accident
and emergency department or call 999. Accident and emergency
departments are open 24 hours a day, 365 days a year and can assess serious
injuries and provide emergency treatment.
This information is only available to those involved in your care and you
should never be asked for personal medical information by anyone not
involved in your care.
We aim to treat our patients courteously at all times and expect our
patients to treat our staff in a similarly respectful way. We take
seriously any threatening, abusive or violent behavior against any of
our staff or patients. If a patient is violent or abusive, they will be
warned to stop their behavior. If they persist, we may exercise our
right to take action to have them removed, immediately if necessary,
from our list of patients.
Appointments
Please contact the practice on telephone number: 01706 369886 to book an
appointment with one of the GP’s, Practice Nurse or Health Care Assistant’s.
■ Urgent cases are seen on the day.
■ The Health Care Assistants and Practice Nurse based at the practice treat patients
for a wide range of common conditions. You can expect to see a nurse within 2
working days.
■ If your condition is non-urgent, you can expect to see a GP within two working
days, though you may have to wait longer if you want to see a particular GP.
■ You can book to have a telephone consultation with a doctor. He or she
will ring you at the end of morning surgery.
Please let us know:
■ If more than one person in the family needs to be seen. We can give
you a longer appointment if necessary.
■ Tell us if you want someone to accompany you during an
examination or a private room to discuss any matters. Remember
that the results of tests can only be given to the patient due to
patient confidentiality.
Home Visits
Our doctors typically see four patients in the practice in the time it takes to
do a single home visit. For this reason, we ask our patients to come to the
practice if at all possible. However, we can visit you at home if your
condition means you cannot attend the practice. Please ring before
10am to arrange a visit and let us know if your condition is urgent.
Evening, Weekends and Public Holidays
BARDOC offers all patients registered with our practice a full GP service when the
practice is closed. For urgent advice and treatment please dial the surgery
number 01706 369886 or alternatively you can call 0161 763 4242.
How you can help us:
■ Be on time for your appointment
■ Tell us if you need to cancel
■ Call for a home visit or urgent appointment before 10am
■ Ring for a repeat prescription between 9.30am to 11.30am
■ Ring for test results after 11.30am
Prescriptions
Where the doctor thinks it is suitable patients on regular medication may
obtain repeat prescriptions. Please telephone, fax or bring your request slip
into the surgery between the hours of 9.30am to 11.30am. Repeat
prescriptions can now be ordered via the surgery website on
www.hopwoodmc.nhs.uk.


Please allow 48 before collection from the chemist or surgery.
If you wish the prescription to be posted, please supply a stamped
address envelope.
Investigations
Specimens should be handed into the Reception Desk before 10am for
delivery to the laboratory. When telephoning for results please contact the
surgery after 11.30am when the telephones are not so busy.
Specialist and Hospital Care
If a GP or another member of our health care team believes you need
hospital treatment or specialist care elsewhere, they will ask you where and
when you would like to go. They can then book your appointment
electronically while you wait.

We run a range of clinics. For an appointment or further details, please call the

 Minor Surgery
Minor operations can be done in our treatment room. Please discuss this with your
doctor who will then arrange for you to be given an appointment.
 Stopping Smoking
Run by our Health Care Assistants, this clinic gives advice and support to
patients trying to give up smoking.
 Antenatal
All appointments to see a midwife now take place in the Local Sure Start
Centre’s. To make an appointment please telephone:
 Woodlands Children’s Centre: 08456019107. Clinics are held Monday &
Wednesday mornings.
 Derby Street Children’s Centre: 01706 369889 (press option 1). Clinics
are held every Tuesday morning.
 Hollins Children’s Centre, Staycliffe Road. Mondays 9.30am – 12.00noon.
 Baby Assessments Thursday 11am
All new babies are invited for regular check-ups from eight weeks old.
 Baby Clinics (Health Visitors)
 Tuesday 10am to 12 noon - Woodlands Children’s Centre.
 Wednesday 10-am to 12 noon - The Phoenix Centre.
 Thursday 1.30pm to 3pm - Derby Street Children’s Centre.
 Chronic Disease Management Clinics
Tuesday 9am-11 noon - Led by Senior Practice Nurse, this clinic offers
advice and general health check-ups to patients diagnosed with any chronic
disease.
 Travel Clinic
Appointments Only
Led by Senior Practice Nurse, this clinic offers Vaccinations & General Advice
when traveling abroad.
Our Nursing Team
Our highly qualified nurses deal with a range of conditions and health
concerns. They examine patients, make a diagnosis and plan care,
including prescribing certain products. They are experts in many areas of
disease management such as diabetes and asthma.
Mrs. Rosanna Cohen
RGN & RM
Rosanna is our Senior Practice Nurse and provides care for patients with
chronic conditions such as asthma, diabetes and hypertension.
Health Care Assistants
Miss Kelly McGrath (AP)
Miss. Angela Pearson (HCA level 3)
Kelly and Angela important members of the practice team who work under the
supervision of a qualified nurse. They can take blood, check blood pressure,
test urine and undertake simple dressing and removal of sutures.
Practice Manager
Mrs. Adele Hardacre’s role involves managing the smooth running of the
practice ensuring that all patients receive a professional and efficient service.
Reception Staff
Paula Beanland, Melanie Greenalgh and Julie Thomson are here to help you.
They answer the phone, deal with enquiries and take repeat prescriptions. Their
job is very demanding so please be patient.
