Foster Care Provider Reporting Portal
The Foster Care Provider Reporting Portal is a web application that has been
created in partnership with foster care providers. This web application supports
our collective effort to issue timely and accurate Foster Care and Medi-Cal
Program benefits to children in out-of-home care.
This web application helps you to do the following:
 Enter child demographic information
 Enter child placement information, i.e. child
entering or leaving your care
 Submit a Foster Care Provider Monthly Report
 Submit Foster Care Program benefit inquiries
 View and print On-Request reports
This web application is secure and requires a valid User Name and Password. If
you do not have a user name, please contact a local Foster Care Office. You may
obtain the local office information by clicking (add this link)
