Service Standards for Mother and Baby Units: Fourth Edition Editors: Peter Thompson and Keith Mahon Published: January 2014 Publication number: CCQI161 Publication Number: CCQI161 Publication Number: CCQI161 Introduction Background Following the inquiry into the deaths of Daksha and Freya Emson in October 2003 and the publication of the Confidential Enquiries into Maternal Deaths in 2001 and 2004, the Royal College of Psychiatrists made a commitment to promote perinatal mental health. Initial funding was provided for the College Centre for Quality Improvement (CCQI) to complete a national survey of Specialist Perinatal Mental Health Services and to set up a network. The Quality Network for Perinatal Mental Health Services was launched in June 2007, as part of this commitment, to develop and maintain standards for mother and baby units. The network engages with frontline staff and applies a clinical audit method within a peer-support network. Since 2013 100% of mother and baby units in the UK have participated in the process. In 2012 the network developed standards for community perinatal mental health services. The Review Process The standards represent just one part of the cycle: the real benefit for services is in taking part in the process of reviews. These reviews aim to gradually improve services using the principles of the clinical audit cycle (see figure below). Types of standards Each standard is rated as Type 1, 2 or 3. • Type 1: failure to meet these standards would result in a significant threat to patient safety, rights or dignity and/or would breach the law. • Type 2: standards that an accredited ward would be expected to meet. • Type 3: standards that an excellent ward should meet or standards that are not the direct responsibility of the ward. Important Note Data collection tools adapted from these standards will be provided with guidance notes to members before reviews take place. This document is provided for reference and not for data collection. These are best practice statements and consequently we would not expect services to meet every standard. While there are some statements that are based upon legal requirements, this document is not intended to act as a legal guide in any way. This is not intended to be a guide to any reviews conducted by regulatory bodies. Std No 1 1.1 1.1.1 Standard Type Access and Admission Provision and procedures ensure that inpatient care is available to those who need it. The unit has the ability to admit women with serious mental illness within the last weeks of pregnancy (based on individual need). The unit has the ability to admit women at risk of recurrence of serious mental illness in the early days after delivery. 1 1.1.3 The unit has the ability to admit women directly to the mother and baby unit, without prior admission to an acute adult ward 1 1.1.4 The unit is able to manage patients without them having to be transferred to a general adult psychiatric ward due to the severity of behavioural disturbance, unless there are exceptional circumstances 1 1.1.2 1 GUIDANCE: The unit should provide details of the source of all admissions (e.g. A maternity hospital or general adult psychiatric ward) and any patients transferred out of the mother and baby unit to another inpatient service within the past 12 months 1.1.5 1.1.6 All admissions to the unit (including out of hours and emergency) only take place after discussion with a senior member of the team GUIDANCE: This includes senior nursing staff Admissions for the purpose of mother and baby parenting assessments are only undertaken in the known or suspected presence of significant/complex mental illness 1 2 1.1.7 If a mother under the age of 18 is admitted to a mother and baby unit then: 1.1.7a A single room must be available 1 1.1.7b The local authority (or local equivalent) is informed of the admission 1 1.1.7c The Care Quality Commission, or local equivalent, is informed (if the patient is detained) 1 1.1.7d There is access to consultation from CAMHS clinicians throughout admission 1 1.2 1.2.1 1.2.2 1.2.3 1.2.4 Referrers and other related professionals have ready access to information about the unit Referrers can access a senior member of the unit team during working hours to discuss potential admissions and the care of women who are at risk of being admitted The unit has clear, written criteria for admission. The unit maintains a list of other mother and baby units with contact details to provide to referrers if the unit is full Information is available (in hard copy and electronically) for referrers and other related professionals e.g. an information booklet etc. 1 1 1 2 Std No 1.2.5 Standard Systems are in place to record, audit and evaluate transfers, refusals, waiting lists and the length of prior admission to an adult psychiatric ward 1.3 There is equity of access to units in relation to ethnic origin, social status, disability, physical health and location of residence 1.3.1 Staff work in a way that respects the individual needs of patients from different ethnic, cultural or religious backgrounds The environment has the ability to meet the needs of people with physical disabilities, and complies with current legislation on disabled access The unit works to overcome barriers to access for patients from remote areas 1.3.2 1.3.3 1.3.4 1.4 1.4.1 1.5 GUIDANCE: For example by providing information about local resources or providing accommodation The unit has access to interpreters and relatives are not routinely used in this role Type 2 1 2 2 1 Units are family-friendly The unit information leaflet clearly states that the participation of partners/family members is encouraged 2 Patients are made familiar with the unit as soon as possible after admission 1.5.1 Patients are introduced to a member of staff who will act as their point of contact for the first few hours of admission 1 1.5.2 Patients are shown around the ward within an hour of their admission, or as soon as they are well enough 1 1.6 1.6.1 All patients are assessed for their health and social care needs Patients have an assessment of their needs of their child which is regularly reviewed. of admission 1 1.6.5 Patients admitted in an emergency on a weekend or out of hours are reviewed by medical staff daily or more frequently if clinically appropriate Case notes show evidence of 2.1.15 Patients are cared for in the least restrictive environment, whilst ensuring appropriate levels of safety Type 2 GUIDANCE: This may change depending on the level of risk presented. 2.1.16 Ward-based staff have access to a dedicated staff room, either on or off the ward 2 2.1.17 All staff have access to a locker or locked area to store personal belongings 2 2.1.18 Patients can access a range of current culturally specific resources for entertainment such as board games, cards, a TV and DVD with DVDs The unit has a computer with internet access in a communal area for the use of Staffing The number of nursing staff on the unit is sufficient to safely meet the needs of patients and babies at all times On a typical six bedded unit, there is a minimum of two nursing staff on every shift to maintain appropriate observation, care and safety levels There is capacity to increase nursing staffing levels according to clinical need In an emergency, extra nursing cover is available from nearby wards 1 1 1 GUIDANCE: For example, there is access to additional on-call staff or staff from a nearby unit 3.1.4 The unit is staffed by permanent staff and bank and agency staff member of staff on shift are from an agency.
